We are seeking to hire a Social Worker .
$23 to $27 per hour based on circumstances including skills, qualifications, degrees and certifications, prior job experience, training, market data, and other relevant factors.
Social Worker Qualifications:
- Bachelor's degree in Social Work or a related field required.
- Minimum of one year of Social Work in a healthcare setting.
- Must have Skilled Nursing Facility (SNF)/Subacute experience.
- Knowledge of Point Click Care or other Electronic Medical Records (EMRs).
- Assist with documentation for the resident admissions: Social Service Evaluation, Discharge plan, Initial Admission Progress, Geriatric Depression Scale.
- Assist with interviewing applicants and families to determine suitability of residency.
- Interview Resident with input of family to determine psycho-social strengths/weaknesses and appropriate adjustments through planned interventions.
- Update resident files/charts, Progress Notes, Plan of Care and Assessment.
- Participate in the Interdisciplinary Care Conference Meetings.
- Assist resident and/or family members with emotional and financial issues.
- Assist in communication with families.
