The Geisinger Skilled Nursing Program is seeking an experienced Physician Assistant for the Embassy of Wyoming Valley, Wilkes Barre and Embassy of Scranton, Scranton, PA.
Job DutiesThis position is as a Geisinger provider in a non-Geisinger facility and requires an experienced advanced practitioner, prior experience working in a skilled nursing facility, family medicine, internal medicine or ICU. The advanced practitioner (AP) provides direct (hands-on) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.

Responsibilities:
- Review reports
- Communicate with SNF staff for patient needs
- Treat patients for acute needs, admissions, discharges, etc.
- Manage extremely complex patients, see every day or at least once a week
- Approximately 12 patients a day
- Length of time per patient and number of patients is per need
