SUMMARY: Under general supervision of the Program Manger is responsible for independent living skills training on a one-on-one basis.
QUALIFICATIONS:
Education/Experience: High school diploma or GED with a preference give to persons who have experience working in day or residential programs serving persons with disabilities.
Language Ability: Must possess ability to read, write and understand English. Strong oral and written communication skills are a must. Must be able to understand and follow verbal and written instructions from supervisor.
Math Ability: Knowledge and ability to work with basic mathematical concepts.
Computer Skills: Must have working knowledge of personal computers. (Highly desirable)
Driving Skills: Must possess a valid CDL and have clean driving record as well as operate a vehicle in good working order for employee transportation. Must have proof of ongoing auto insurance on personal vehicle.

Purpose:
The Skills Trainer will work with adults diagnosed with severe and persistent mental illness (SPMI) as part of an Integrated Team Based Care (ITBC) team. The primary role is to support participants in developing and strengthening their ability to thrive, as defined by the participant. This may include assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (iADLs). Working in an outpatient mental health clinic and out in the community, the Skills Trainer will collaborate with other team members to ensure participants receive holistic, person-centered care that enhances their overall independence and quality of life.
Accountabilities:
- Provide direct support to participants in ADLs and iADLs, including but not limited to:
- Navigating public transportation and community navigation
- Grocery shopping and meal preparation
- Medication management
- Scheduling and attending appointments
- Managing finances and budgeting
- Maintaining personal hygiene and household upkeep
- Collaborate with the ITBC team to develop individualized care plans and goals for each participant
- Offer guidance and coaching to participants, helping them develop skills that promote autonomy and confidence
- Empower participants to define what thriving looks like for them and work toward those goals
- Monitor and document participant progress, updating service plans as needed
- Facilitate access to community resources and services that support participants in achieving their goals
- Provide consistent support for program operations, including de-escalation and participant engagement
- Offer milieu support, modeling person-centered social interactions with participants
- Engage in regular communication with team members, participants, and their support networks to ensure consistency and continuity of care
- Participate in team meetings, case reviews, and trainings to maintain a high standard of care and professional development
- Maintain a base understanding of available community resources
- Develop individual service plans in partnership with the participant and members of the treatment team, which includes measurable objectives
- Facilitate skills training groups
- Transport participants to necessary appointments as indicated
- Model assertive engagement strategies and advise staff on personalized approaches for participants.
- Ensure clinical charts are complete and in compliance with federal, state and county rules, including service plans and LSI/LOCUS
- Meet productivity and engagement expectations to ensure participants are being seen adequately.
- Complete documentation within 72 hours of services being rendered through EHR (or sooner if crisis risk indicates need for same day completion).
- Complete supportive documentation in timeframes indicated by OARs
- Utilize feedback-informed treatment tools and review regularly
- Participate in one-on-one supervision regularly to review interventions, documentation, and professional goal setting
- Engage in both structured and ad hoc consultation within the team
- Attend and positively participate in required meetings, including team meetings and care coordination meetings with LMPs to support collaboration and efficiency
- Adhere to any applicable ethics codes and New Narratives' mission and values
- Follow all New Narrative policies, procedures, and protocols
