Andover Manor Rehab and Nursing is seeking a compassionate Dementia Care Coordinator to fill a full-time position in our skilled nursing facility. This is an exciting and energetic position to provide a variety of meaningful activities for our elders daily.
As the Dementia Care Coordinator, you will:
- work with the Director in creating programs that are tailored to the needs and preferences of our residents
- provide hands-on involvement in activities and events
- ensure support and safeguard the dignity of those in our care
Don't forget.
- Social, recreational, activities experience of at least two years required this experience must be within the last 5 years
- Prior experience working with patients/elderly required.
Degree in Recreation or previous experience as a Recreation Assistant or Director is preferred.
- A positive, outgoing, upbeat personality is a must.
Strong organizational & planning skills; ability to multi-task.
This is a direct hire position; we do not go through an agency.

A Day in the Life as a Transport Support Worker
As a Transport Support Worker, you will provide transportation and visitation support for children and families involved in the child welfare system. You'll play a critical role in promoting the well-being of children, participate in staff meetings and trainings, and coordinate with other agencies providing Permanency Services .

POSITION TITLE: Adult Attendant Care/PRI Worker
FLSA CLASSIFICATION: Non-Exempt
REPORTS TO: Case Management and Peer Team Lead
POSITIONS SUPERVISED: N/A
POSITION OVERVIEW: The Adult Attendant Care/Psychosocial Rehabilitation, Individual Worker is responsible for assisting skill building opportunities with consumer. The goal is for the consumer to become more self-sufficient in daily living skills. This position works one-on-one with consumers to provide crisis support, assistance with the utilization of community resources, assistance with symptom management, and with other activities which promote the recovery of mental health consumers.
ESSENTIAL POSITION RESPONSIBILITIES:
- Provides services in order to maintain required productivity/billing standards set by the department.
- Meets deadlines and ensures accuracy of various reports/paperwork, mileage records, and electronic timesheets.
- Maintains accurate and timely documentation of service provision. Completes progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.
- Submits required progress notes/billing information in a timely manner as per Agency, Medicaid, and COMCARE requirements.
- Provides AC/PRI services in accordance with the consumer's treatment plan goals.
- Provides motivation, support and instruction for consumers to achieve self- sufficiency in the activities of daily living necessary to maintain personal stability in their current living arrangements.
- Provides support in crisis situations.
- Observes and notifies supervisors & treatment team of changes in consumer condition.
- Notifies supervisors of a consumer's non-compliance with treatment plan goals or the need to change goals.
- Assists consumers with utilizing resources within the community.
- Provides consistency in services to consumers by arriving on time to scheduled appointments and/or notifying consumers in the event of tardiness.
- Collaborates with the treatment team & Care Manager to ensure better health and appropriateness of services.
- Maintains acceptable overall attendance record, to include department staff meetings, agency meetings, and trainings as required. Ensures appropriate notification to supervisor for absences, and ensures that work is covered. Flexible in work schedule when needed.
- Exhibits appropriate level of technical knowledge for the position.
- Produces quantity of work necessary to meet job requirements.
- Works well with a team, keeps others informed of information needed. Treats others with respect, maintaining a spirit of cooperation.
- Maintains effective and professional verbal and written interactions with peers, customers, supervisors and other staff. Uses diplomacy and tact in dealing with difficult situations or people. Demonstrates effective listening skills. Is receptive to constructive feedback.
- Demonstrates the ability and willingness to handle new assignments, changes in procedures and business requirements. Identifies what needs to be done and takes appropriate action.
- Completes assigned work, meets deadlines without reminders/follow-up from supervisor or others.
- Performs work conscientiously with a high degree of accuracy.
- Meets goals and objectives as mutually agreed upon during last performance review (if applicable).
POSITION REQUIREMENTS: The worker in this position must have a high school diploma or equivalent and be at least 18 years old. Prefer post high school education and/or experience in the human service field equal to one year. (Experience in the provision of services to persons with a mental illness or the life experiences of a person who is recovering from a mental illness may be substituted for other requirements.) The worker must be reliable and responsible, with the ability to respond appropriately to emergency situations and will preferably have experience with the SPMI population. Effective communication skills, both verbal and written, and the ability to develop and maintain rapport with consumers, constituents and staff are essential. The AAC worker must possess basic computer and typing skills. A valid Kansas driver's license, current vehicle insurance, and access to a personal vehicle are required.
PHYSICAL REQUIREMENTS:
* Driving (for purposes of community mobility)
* Extensive writing, note taking
* Lifting/carrying up to 30 pounds
* Bending/Stooping/Climbing stairs
* Sitting/typing for extended periods of time
