+ Develops comprehensive care plan to address objectives and goals as identified during assessment.
+ Supports member access to appropriate quality and cost-effective care and modifies plan(s) as needed.
+ Coordinates with internal and external resources to meet identified needs of the member in terms of integrated (physical and behavioral) whole person care.
+ Coordinates social determinants of health to meet the needs of the member and incorporates that into care planning.
+ Works closely with various state agencies.
+ Maintains knowledge of the system of care philosophy; a spectrum of effective, community-based services and supports for those with or at risk for mental health or other challenges and their families, that is organized into a coordinated network.
+ Builds meaningful partnerships with designated populations and their families, and addresses cultural and linguistic needs, in order to help them function better at home, in the community, and throughout life.
+ Evaluates health needs and identifies applicable services and resources in conjunction with members and their families.
+ Provides important information including patient education, medication reconciliation, and identification of community resources and assists with arrangement of follow-up care.
+ Works on strategic projects related to specialty populations.
+ Assists with onboarding new associates.
**Minimum Qualifications:**
+ Requires MS/MA in social work, counseling, or a related behavioral health field or a degree in nursing and minimum of 5 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience, which would provide an equivalent background.
+ Requires an active, current and valid license as an RN, LCSW (as applicable by state law and scope of practice), LMHC, LPC (as allowed by applicable state laws), LMFT, or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.
+ Certified Case Manager Certification is also required
**Preferred Skills, Capabilities, and Experiences:**
+ Knowledge of resources, supports, services and opportunities required for safe community living for populations receiving in-reach and transition services, including LTSS, Behavioral Health, therapeutic, and physical health services.
+ Experience working with Children, Youth, and Families who are being served by Local Departments of Social Services through Foster Care and Adoptive Assistance programs is very strongly preferred.

Job Description

The Clinical Social Worker II provides social work services to patients and their families to assist in meeting psychosocial and clinical needs. Using expertise and experience, this role identifies and serves the needs of patients, both inside and outside the clinical setting utilizing therapeutic and other clinical techniques.

Determines which patients to focus clinical social work efforts on to help staff and patients alike be in the correct environment with appropriate social work support.
Analyzes information from both the patient and clinical staff to understand the patient's situation and is able to communicate this information to all parties to better prevent issues that arise from information gaps.
Links patients with community resources and community health opportunities based on clinical knowledge and experience to ensure that patients get the highest quality of necessary care.
Crafts a care plan for patients with minimal assistance from clinical staff to enable the patient to get the treatment that is right for them.
Educates staff, patients and families alike about critical information related to care to help all parties involved better understand the situation, serving as a liaison.
Provides crisis support and leadership in high-tension situations for staff, family and patients using clinical expertise to ease all parties in high-tension situations, giving explanations that help de-escalate the worries of patients and their families.
Provides clinical work services to both patients and their families to meet current and future psychosocial and medical needs.
Completes psychosocial assessment for patients to understand their situation and be able to link them with the option of care that makes the most sense for the patient's medical and social status.
Utilizes various counseling and therapy techniques to help place the patient and their family at ease with regards to clinical and medical work.
Performs other duties as assigned.
EDUCATION: Required: Masters of Social Work
EXPERIENCE: Required: Minimum of two (2) years of recent experience in a health-related environment
LICENSURE/CERTIFICATION/REGISTRY/LISTING: Required: Licensed Clinical Social Worker
