Job Description
The Patient Care Coordinator is responsible first and foremost for setting the tone of customer care when patients are being processed through the department. Monitors customer service in the front office. Responsible for admitting new patients, incoming calls, daily attendance records, data entry, departmental transcription, inter-departmental communications, scheduling for the department, and clerical responsibilities for the department and the Director.
- Familiar with operating procedures and specific department procedures.
- Fills in for the office manager as needed.
- Schedules appointments from all referral sources in a timely and efficient manner.
- Provides proper admission/readmission of patients.
- Computer-generated correspondence has minimal errors.
- Continuously prioritizes tasks at hand.
- Ensures information requested is routed to the proper source.
- Ensures daily schedule stays full with patients.
- Departmental data such as admitting patients to the Audiology system is complete and accurate.
- Confirms next-day appointments accurately and in a timely manner.
- Checks mail and package deliveries.
- Handles entering cash transactions.
- Tallies all transactions electronically and routes to patient accounting daily.
- Receives all incoming phone calls to the department and forwards calls to the appropriate person or takes messages.
- All phone calls are handled promptly and responsibly.
- Works with other departmental staff to coordinate job functions between other clerical staff, technicians, and receptionists as necessary.
- Performs minor hearing aid repairs.
- Sells hearing aid supplies and related accessories.
- Performs other responsibilities as assigned.
This position requires endurance to perform tasks over long periods of work hours and the ability to perform essential functions without posing a "direct threat" in the workplace. Must be able to lift 25 lbs.
A minimum of two years experience in clerical work, which requires knowledge of generally accepted office procedures; ability to effectively and efficiently use, or demonstrate the ability to learn all office machines required to accomplish the department's activities. Familiar with medical terminology. Ability to type dictation or handwritten material. Must be able to handle multiple tasks, and have the ability to function in a very busy environment. Experience in using Google Workspace products is required.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
As a member of the nephrology healthcare team, you will provide safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse.
• Sets up, tests, and operates hemodialysis machines for patient treatments.
• Obtains and documents necessary pre- and post-treatment vital signs and weight for each patient.
• Evaluates vascular access pre-treatment and performs vascular access cannulation.
• Evaluates intradialytic problems and provides intervention as prescribed by physician order or as directed.
• Monitors patients’ response to dialysis therapy.
• Evaluates patient prior to termination of venous access and discontinues dialysis treatment according to established procedures.
• Reports any significant information and/or change in patient condition directly to supervisor.
• Enters all treatment data into the designated clinical application in an accurate and timely manner.
• Collects, labels, appropriately prepares, and stores lab samples according to required laboratory specifications.
• Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
• Assists other healthcare members in providing patient education.
• Prepares, organizes, and efficiently uses supplies and equipment to prevent waste.
EDUCATION AND LICENSES :
• High school diploma or G.E.D. required.
EXPERIENCE AND REQUIRED SKILLS :
• Qualities and traits: dependable, compassionate, caring, supportive, collaborative, reliable.
• Previous patient care experience in a hospital setting or a related facility (preferred but not required).
• Continued employment is dependent on successful completion of the Fresenius Medical Care dialysis training program and successful completion of CPR certification.
