Qualifications:
Qualified candidates need to be fully and independently licensed as a LCSW in Texas
Clinicians need to be licensed and living in the state in which they will be practicing
Compensation: Up to $105,400 depending on licensure, sessions, and bonus opportunities

Individuals willing to see a minimum of 25 sessions per week

As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation.
What you will be doing
Advocate & Support. You will be part of an interdisciplinary team working to ensure patients receive the best care, including conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state/CMS regulations. You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies.
Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community.
Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes. You will also participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations.

Position Summary
The Guia (Licensed Master Social Worker) is responsible for a panel of patients and, in collaboration with other members of a multidisciplinary primary care team, helps patients meet their preventive, chronic, and acute care needs.
The Guia will engage patients and encourage them to take an active role in their health by
providing them with the tools necessary to make healthy lifestyle choices and adopt lifelong healthy behaviors. This individual's primary responsibilities center around establishing trusting, supportive, collaborative relationships with patients and their families. Our Guias build relationships with patients in a clinical setting and in the community by working alongside medical providers, nurses, other Guias, medical assistants, and a behavioral health specialist in a collaborative and empathetic team to improve patient outcomes.
Responsibilities
- Actively engage a panel of assigned patients within Suvida Healthcare
- Onboard patients to the Suvida model and their medical/social care visits
- Function as a patient advocate by attending patient appointments when requested/needed two.
- Document relevant information about the patient's health and healthcare experience and perform health risk and social needs assessments.
- Collaborate with a multidisciplinary team to care for a population of patients and monitor and track whether patients are up to date on preventive measures.
- Provide patient education on acute and chronic disease management.
- Provide guidance to patients and families.
- Establish healing relationships with patients and families.
- Learn to employ confidence-promoting techniques in patient communication and develop patient self-efficacy to better manage health.
- Communicate with patients in-person and by phone, video conference, and text messaging.
- Collaborate with other members of the multidisciplinary care team including but not limited to the Guia manager, Transitions of Care manager, Medicaid case managers, and medical assistants.
- Coach patients using an action-planning model based on motivational interviewing techniques.
- Assist with the coordination of care across the continuum, such as: scheduling appointments with providers, coordinating referrals, and sharing or transferring information with the patient's internal and external care team.
- Participate broadly in the daily operations of a primary care practice, such as: Answering incoming phone calls and messages and ensuring general upkeep of the clinical space.
- Track patient enrollment and progression through care programs.
- Other duties as assigned by the Guia Manager
Knowledge, Skills, and Abilities
- Experience in a customer service environment• Some clinical experience is a plus, such as LMSW or CHW-Instructor.
- LCSW Preferred.
- Experience working with geriatric population.
- Able to articulate Suvida Healthcare's mission in relation to patient satisfaction and patient outcomes.
- Compassionate, kind, and open-minded
- Teamwork experience.
- Proficient with a computer, mobile devices, medical devices, and Microsoft Office Suite
- Experience utilizing electronic medical records or social service referral management software.
- Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components.
- Ability to work independently and exercise sound judgment in interactions with providers, payors patients, and their families.
- Ability to communicate and effectively interact with people across cultures, ranges of ability, genders, ethnicities, and races.
- Effective oral and written communication skills
- Able to care for patients in-home, in-clinic, and remotely.
- Bilingual/Bicultural (English and Spanish)

As an AHF Managed Care Division Social Worker Supervisor, experience and skills in psychosocial and complex social determinants in collaboration with the entire Care Management Team, i.e., Medical Provider, RN Care Manager, Community Health Worker, HCC Team and other support AHF staff focusing on socioeconomic services that influence health outcomes.
As part of the Care Management Team, you will provide supervision and support to the community health workers who are focused on retaining patients in care, regardless of payment source to provide access to services impacting their ability for better health outcomes. Utilizing your Social Work and supervisory experience and skill set, you will maintain current information on community resources, provide referrals to appropriate resources and guidance designed to address and mitigate patient social determinants of health which impact patient attainment of their health goals.
You will collaborate with the RN resource manager, providers and the Healthcare Center team so that the team is aware of all patient/client goals and challenges. You will conference with the provider, Care Management Team, RN resource manager, patient and family so that treatment plans, referrals and individual care plans are shared and understood with focused interventions designed to maintain patient eligibility for services and engagement, improve health outcomes and address issues as they pertain to the social determinants of health. You will provide access to services and resources in the community that can assist in the goals of your patient/client. You will be responsible for reviewing charts of the community health workers and ensuring daily productivity. Radical Collaboration is essential in this role as you will work across all divisions of AHF that impact your patient/client.
Utilizing your Social Work experience, supervisory experience and Care Management skills you will meet our patients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting and access to resources so that their psychosocial needs are met.
