When unit is full, LPN will work with RNs with highest acuity or will be paired with an RN who has a heavy patient load. When the UNL has patients, the LPN will be paired with the UNL to assist with the UNL patient load. LPN tasks may include oral meds, IV piggyback meds, trach care, dressing changes, documentation on assessments and care plans after the RN has completed the first assessment and initiated the care plan.

Ensures patient visit string(s) are added to the record to make sure visits are not missed.Ensures proper acronym is evident on all physician orders.Completes Home Health Aide supervisory visits.Acts as the coordinator of the health care team in order to maintain the proper linkages within the continuum of care.Collaboratively communicates and initiates case conferencing as necessary and documents interacts (other disciplines, MD, care coordinator, insurers, etc.). Communicates pertinent information for patient care conferences on all admits done by him/her.Practices in a manner sensitive to the needs of patients and families. Provides care according to plan of care and orders.Directs the activities of the licensed practical nurse.Makes home health aide assignments, prepare written instructions for the aide and supervises the aide in the patient home.Assess patients for appropriate placement in Hospice per Medicare guidelines Make home visits to monitor for signs and symptoms associated with end stage disease and end of life Implement interventions to palliate symptoms in concert with attending physicians Triage phone calls when on call Make home visits to assess and pronounce death, do post mortem care, notify County Coroner of patient passing, notify Funeral home and physician Provide comfort for the family and make appropriate referral to Bereavement Coordinator

The compensation range for this position is $33.62per hour - 57.83per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model. The Diabetes Nurse Educator utilizes a systematic approach to nursing by incorporating assessment, planning, implementation and evaluation in the practice of nursing. Provides healthcare to individuals, families, and communities within the RN scope of practice as defined by the Illinois Board of Nursing.
Qualifications

Certifications: Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR); Certified Diabetes Care and Education Specialist (CDCES) within 3 years (1) - Certification Board for Diabetes Care and Education (CBDCE); Basic Life Support (BLS) Instructor within 30 days - American Heart Association (AHA), Education: Bachelor's Degree: Nursing, Work Experience:
Responsibilities

Provides and facilitates competent health care of patients and families in area of practice. Ensures adherence to established standards of care and scope of practice. Ensures documentation of care is complete and accurate. Provides health and wellness education to individuals and groups. Acts as preceptor and supports the development of other staff and healthcare students. Provides leadership for initiatives designed to improve the quality of care or clinical outcomes. Completes all documentation required for the ADA Recognized program Assists with communicaty and organziation projects Completes clinical and educational assessment on all patients. Develops individualized teaching plans for self-management (Diet, Medication, Self-care) Implements evidence based clinical practice guidelines to provide diabetes education Provides education to all patients utilizing an insulin pump. Collaborates will all providers and members of the health care team
