This nursing position will join a collaborative team of outpatient behavioral health professionals committed to Carle's mission and values. Will work in a department with 10 physicians and APP's plus 10 psychologists and counselors). Outpatient Behavioral Health population ages 5 and up. Provides phone triage, education and support; multi-tasking with returning phone calls/portal messages/helping during clinic/returning messages to PSR staff trying to schedule patients.

-Positions patients correctly, using proper immobilization techniques-Performs optimal scans using correct technical factors, shielding and protocols, based on the patient diagnosis, history and other relevant clinical information.-Use correct technical factors in accordance with the American College of Radiology (ACR) to use the lowest amount of radiation to get a diagnostic scan.-Performs CT angiography, 3D, MIPS, VR, Vessel Analysis, and MPR's, post processing of computer data to enhance exams for radiologist interpretation, using reconstruction software appropriate to the pathology or region of interest.-Follows correct CT Department Technologist workflow ( select correct scan room, identifies and documents two forms of identification on each patient, acquire history, verify orders, scan patient, send images to PACS, scan all paperwork and data sheets (eg. history sheet, med-reconciliation sheets), complete the case in Meditech, address Q A issues, charge for contrast media and supplies, Verify that all images, series and data sheets have been sent to PACS make sure the number of images is correct, verify exams with images and archive images on scanner-Operates support equipment for CT such as: Independent workstation, pressure injector, PACS and EPIC systems.-Initiates IV for administration of contrast media for CT in accordance with department policy and utilizing standard precautions - Must be IV certified within 3 months of hire.-Prepares patient for exams by explaining procedure and by encouraging questions and answering any questions the patient may have-Transports patients to and from departments when necessary. Assist in transfer of patients on and off the CT table. Continually monitors patient condition during CT procedure.-Accountable for documentation of and charging for supplies used during the performance of exams or procedures.-Documents patient charge data, modifying and initiating charges as procedure modifications are made per the radiologist or ordering physician-Documents any allergic reaction, iv extravasations or unusual occurrences. Initiates incident report regarding staff or patient situations-Maintains appropriate inventory levels of medical supplies. • Patient population (age, diagnosis, specifics about unit): 18 years and old. Cardiac arrhythmia, Chest pain, CHF, Pacemakers, Ablations, Heart attacks (MI), and Cardiac interventions.

• Department culture and resources: CVCU is a fast paced, high patient turnover unit. We admit and d/c multiple patients each day. • New Graduate Nurse Residency Program
  
  Carle BroMenn Medical Center and Carle Eureka Hospital provide a Nurse Residency program for new graduate Registered Nurses during their first several months of practice. New Graduate Nurse Residency assists the nurse in the transition from student to professional nursing practice. The program offers facilitated learning sessions, discussions, and reflective self-care sessions in a group setting. The content is structured to help new graduates apply best practices, evidence-based care guidelines, and to use proactive nursing strategies.
  
  All nurses receive a comprehensive orientation which includes classroom orientation followed by unit-based orientation with a unit preceptor. During residency, the new graduate nurse receives additional classroom, simulation, virtual reality, and skill sessions presented by a team of expert level RNs and Clinical Development Specialists. Topics cover core concepts that assist in the nurse’s professional career development and are designed to meet the needs of each group. In addition, the residency program also includes attending a unit shared governance meeting and an opportunity for observational experiences. Demonstrates understanding of Medicare Home Care benefits including covered services, recertifications, transfers, and non-recertifications. Follows regulatory requirements regarding documentation standards.Comprehends department specific indicators.Performs case management activities of assessment, coordination, planning, monitoring, implementation, and evaluation. The Clinical Resource Unit (Float pool) is committed to our Nursing Professional Practice Model; we believe in caring relationships with our patients, families, community, and colleagues to provide exceptional care. In the clinical resource unit, our dedicated nursing staff, nurse leaders, interdisciplinary team members, and physician partners practice on providing specialized medical and nursing care to various inpatient units including Medical- Oncology, Surgical -Orthopedic, Cardiovascular care unit, Progressive Care unit, Mental health unit, and Acute Rehab. Our highly-trained staff utilizes the most current technology and equipment. Medical diagnoses include but are not limited to coronary artery disease, angina, myocardial ischemia, myocardial infarction, dysrhythmia, syncope, congestive heart failure, strokes, cancer, cellulitis, post surgicals, and chest pain. Overview

Responsible for the oversight, coordination, and management of the functional and financial outcomes during acute illness requiring hospitalization for patients of Carle Health. Responsibilities

Act as a liaison working with patient/family and physician to determine next level of care and manage LOS. Coordinates the transition from inpatient care to post-hospital care, collaborating with physicians, consultants, internal and external referrals to facilitate appropriate next level of care to meet patient goals and outcomes. Documents plan of care and utilization issues in appropriate locations, including but not limited to: case management/utilization review software and the multidisciplinary plan of care document on all assigned patients. Evaluates effectiveness of plan of care and collaborates with physicians to ensure the progression toward desired patient outcomes while managing risk for readmission and length of stay. Proactively investigates coverage for post-hospital needs and presents options to the patient/family and provider. Initiates intervention, both pre-hospital, in-hospital, and post-hospital, for patients and families identified from a proactive initial admission assessment, as well as through referrals from members of the healthcare team. Participates in multidisciplinary rounds and initiates timely referrals to other internal healthcare team members (quality improvement, risk manager, social workers, pharmacy, therapy services, etc.) Performs case management activities of assessment, coordination, planning, monitoring, implementation, and evaluation. Interacts with clients, caregivers and families to assess, plan care, arrange services, monitor, and provide support and education. Provides oversight of acute setting plan of care to ensure coordination and completion of services to meet post-hospitalization needs. Conducts case review presentations to educate peers on unique or challenging cases and scope of practice issues. Track avoidable days on inpatient stays. Readmission assessment of inpatient stays. Assess patients for post discharge needs within 24-48 hours of admission and ongoing through out the stay. Assess patients risk for readmission and navigate strategies with patient and family to reduce risk through post acute planning. Arrange DME, Home Care, outpatient infusion and coordinate with social work for Hospice, ECF Placement, and Transportation Deliver or delegate delivery of regulatory letters for Medicare and observation patients.
