The role you’ll contribute:
The Home Health Occupational Therapist (OT), under the general supervision of the Care Manager, provides occupational therapy to assigned patients and directs therapy assistants and Home Health Aides to provide quality care in accordance with physician orders under a plan of care. Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with therapy standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after-hours work. Coordinates and directs the home care patient's care based on individual patient needs.

- Measures and monitors effects of treatment at various stages and confers with occupational therapist to adjust treatments to achieve maximum benefit and outcome.
- Assures that patient treatment outcome is directed towards development and restoration of functional skills and returning to or maintaining maximum potential.
- Recommends and teaches the use of assistive devices and equipment to patients and families.
- Instructs nurses, Home Health Aides, and family members to provide for continuous Occupational Therapy program.
- Observes records and reports on patient progress, reaction to treatment and any changes in patient’s condition.
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
- Minimum of six-months relevant occupational therapy
- Occupational Therapist (OT)
- Active American Heart Association BLS
- Valid in state Driver’s License with current car insurance
Preferred qualifications:
- Recent, relevant experience in a Medicare-certified home health agency as a Occupational Therapist
- Master's in Occupational Therapy

What You'll Do
As a Clinic Manager - Physical Therapist (PT) at CORA, you’ll:
Make a powerful impact on your local community through inclusive physical therapy treatment.
Develop and deliver a personalized plan of care for your patients -- a diverse patient population with both orthopedic and neurologic diagnoses.
Objectively measure patient outcomes using cutting-edge software.

What You’ll Need
Degree from a CAPTE-accredited Physical Therapy program.
Licensed or license eligible as a Physical Therapist (PT).
At least one year of experience as a Physical Therapist.

ESSENTIAL FUNCTIONS:
- Performs CT procedures at a technical level not requiring constant supervision of technical detail.
- Obtains patient history, explains standard procedures and addresses patient concerns.
- Produces computerized tomographic scanner radiographs of specific areas as required by the departmental procedures.
- Performs CT procedures at a technical level not requiring constant supervision of technical detail.
- Performs those duties directly involved with a variety of technical procedures applying ionizing radiation for the purpose of detecting pathology.
- Regulates the equipment used to expose the x-ray film, develops and documents PACS imaging.
- Operates equipment safely and maintains SimonMed standards while performing call types of procedures.
- Establishes and maintains a good rapport and professional relationship with fellow employees, other departments, and facility staff.
- Familiar with standard concepts, practices and procedures.
- Relies on experience and judgment to plan and accomplish goals.
- Works under general supervision.

PHYSICAL DEMANDS: This position may require duties including lifting and carrying up to 40 pounds, sitting for prolonged periods of time, with frequent standing and walking.
