Overview

This position will be a part of the City of Peoria Social Services Unit (SSU). SSU personnel are tasked with assisting those who come into contact with, or are referred to, the SSU. One of the goals is to promote safer encounters between citizens in need of crisis services and police department personnel while maintaining a high level of professional service. The other main goal is to refer to, and encourage participation in, appropriate mental health, substance use, or other needs-based services that may fit the individual. These duties are critical to an overall healthier, safer community and are aimed at facilitating more stable community members. This position provides crisis intervention services/assessments/evaluation and stabilization/case management to people in a primarily community-based environment in collaboration with the Peoria Police Department (PPD). This role will include field response into the community in collaboration with the PPD, likely several times per day, to provide mobile mental and behavioral health services to citizens within the city of Peoria, Illinois. This role will consist of supervision of Licensed Social Workers and Licensed Clinical Social Workers, clinical oversight for staff, continued follow up and case management to connect citizens to ongoing services and supports.

Responsibilities

Assist provider and other clinical staff with obtaining basic vital information preparing patient for examination, treatment and minor surgical procedures* Demonstrates competence in the clinical skills necessary to carry out assigned job duties May be required to draw and collect blood and other samples from patients and prepare specimens for laboratory analysis. Apply Nursing Process (assessment, diagnosis, planning, intervention and evaluation within licensure guidelines). Telephone triage by providing and documenting advice for home care of illness/conditions; conducting follow-up phone calls regarding patient’s health and progress as necessary. Administer vaccines and ordered medications by oral, IM,SQ or ID routes. Place urinary catheters and IV per protocol and license. Schedule referrals for patients with specialist or outpatient as requested by the provider. Complete appropriate forms or requisitions needed for referrals or lab testing, including appropriate CPT/ICD-9 coding. Understand impatient and outpatient specialty procedures. Responsible for compliance with precertification requirements. Responsible for tracking system for follow-up appointments related to office visits, laboratory tests and procedures to regularly follow patients monitoring disease status. Provide appropriate and timely documentation in the patient’s electronic health record using standardized workflows and processes. Supervision of delegated duties to LPN and CMA on approved clinical protocols. Maintain compliance with CLIA, OSHA, Safety and Risk Management guidelines. Maintain compliance and behave in a manner consistent with all policies and procedures, including but not limited to Compliance, HIPAA and personnel workplace rules. Demonstrate initiative to improve quality and customer service by striving to exceed customer expectations. Provide education to patients and families using the Teach-back method per protocols and guidelines.

Compensation and Benefits

The compensation range for this position is $30.39per hour - $52.27per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
