During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits.  The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities:

Matrix Provider – What to Expect :

  • Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers and any ancillary medical personnel as appropriate
  • Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up
  • Provide services in a variety of venues to include: Home Visits, Skilled Facility Visits, 

Nurse Practitioner/Physician Assistant  - PRN - Home Risk Assessments

Type: PRN - Flexible

PAY:  Earn from $2,200 up to $1,000 per month (Based on an average of 20-100 completed visits per month, and pay of 110.00 per completed assessment).  Compensation range is based on availability and assessment completion.

Location: Home, SNF and other community environments in Lexington, Virginia 

Hours : Days, Evening, Nights, weekend, Flexible Scheduling

Benefits : PRN providers are non-benefited except for 401(k) and limited Voluntary Benefits 

Mileage Reimbursement plan

Qualifications:

Must Haves:

  • Master’s Degree required OR commensurate experience and satisfactory completion of NP licensure
  • Master’s Degree required OR commensurate experience and satisfactory completion of PA licensure
  • Current RN and NP/PA licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority
  • Board certified by the NCCPA, AANP, ANCC or the AACCN in a Matrix approved specialty
  • Have a current BLS, ACLS or CPR certification
  • 1 year experience as a Nurse Practitioner/Physician Assistant preferred, new grads encouraged to apply. Have the ability to travel either in state or out of state
  • Have strong computer skills and familiarity with EMR software
  • Possess excellent verbal and written communication skills with patient, clients and colleagues

Responsibilities:

-Conducts State Safety and Emissions Inspections on vehicles in an ethical manner, and following all Virginia State Police, Virginia DEQ, and company guidelines

-Observes general vehicle condition while performing basic services, and reports any obvious repair needs or safety concerns to the Service Manager

-Maintains shop cleanliness for a clean and inviting appearance

-Follows all safety procedures and reports any concerns to the Service Manager

-Comply with established safety programs, including wearing protective eyewear

Job Summary and Qualifications The CRT will perform all respiratory care related duties. Sets up and administers all routine and complex respiratory therapy procedures to patients of all ages in all areas of the hospital as prescribed by a physician. Respiratory technicians are responsible for the application and operation of acute care ventilators along with non-invasive ventilation including BiPAP and CPAP. Maintain all department and hospital records and enters accurate information into the hospital information system for the therapy given. Respiratory technicians evaluate, treat and care for patients with breathing or other cardiopulmonary disorders.  The respiratory technician is also responsible for performing arterial punctures and the handling and analysis of blood gas specimens. The technician is responsible for the daily maintenance of the blood gas analyzers.  
What qualifications you will need:

Must maintain CPR, ACLS, and PALS certification (initial certification within 90 days of hire) 
Must be able to follow verbal and written instructions. 
Ability to communicate verbally and in writing. 
Participate in continuing education and learning to enhance and maintain current knowledge and skills for continued competence. 
Perform all duties using proper body mechanics. 
Frequently move/lift up to 80 pounds, depending on assignments. 
Stand for long period with frequent squatting, twisting, bending, kneeling and reaching to prepare equipment, materials in order to provide patient care or maintain the patient environment. 
Ability to work in stressful environment. 
Consistent attendance is required to assure that the department, hospital, and patient’s needs are met. 
Manual dexterity and fine motor coordination. 
To comply with all facility policies, procedures and practices. 


EDUCATION  
Required: Graduate of an accredited school of Respiratory Therapy. 
Required: Successful completion of the NBRC Certified Respiratory Technician exam.

CERTIFICATE/LICENSE 
Required: Currently licensed as a professional Respiratory Therapist in state of Virginia
