The Master's Social Worker (Masters in SW) participates in the interdisciplinary care provided to home health patients. The Medical Social Worker functions to evaluate and develop a plan of care personalized to fit the patient’s emotional and social needs. The Medical Social Worker provides direction and supervision of the Social Worker Assistant as required and when involved in the patient’s plan of care. The Medical Social Worker works within CenterWell Home Health's company-specific policy and procedures, applicable healthcare standards, governmental laws, and regulations.
  • Assesses the patient’s social and emotional state as it relates to his or her illness or injury, needs for care and his or her response to such treatment, and adjustments to care.

  • Assesses any relationships of the patient’s medical and nursing needs in the home setting, financial resources, and available community resources.

  • Provides any appropriate action to obtain available community resources to assist in resolving issues that may be impeding the patient’s recovery.

  • Instructs patients and families in treating and coping with social and emotional response connected with Provides ongoing assessment of patient and family needs and responses to teaching

  • Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health Participates in the development and periodic re-evaluation of the physician's Plan of Care for the patient.

  • Observes, records, and reports changes in patients’ condition and response to treatment to the Clinical Manager and the Participates in the discharge planning process

  • Participates as a member of the interdisciplinary care team in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs

  • Supervises instructs and evaluates the performance of the Social Work Assistant (BSW) to assure that all medical social services are provided to patients in compliance with Company, government, and professional standards

  • Maintains and submits documentation as required by the company and/ or facility including any case conferences, patient/physician community contacts, visit reports progress notes, and confers with other health care disciplines in providing optimum patient care.

Required Skills/Experience

  • Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education.

  • Social Worker licensure in the state of practice; if required by state law or regulation.

  • A valid driver’s license, auto insurance, and reliable transportation are required.

  • Proof of current CPR certification

  • Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice.

  • Knowledge of and the ability to assist with discharge planning needs, and to obtain community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility.

  • Excellent oral and written communication and interpersonal skills.

  • Knowledge of medications and their correct administration.

  • Ability to organize tasks, develop action plans, set priorities, and function under stressful situations.

  • Ability to be flexible in work hours and travel locally.

  • Ability to communicate effectively with patients and their family members and at all levels of the organization.

  • Maintains current licensure certifications and meets mandatory continuing education requirements.

  • Must read, write and speak fluent English.

  • Must have good and regular attendance.

Summary:

The Travel Cath Lab RN provides specialized care for patients undergoing cardiac catheterization and interventional treatments. The RN prepares patients for procedures, assists during interventions, and monitors immediate post-procedure recovery. Working closely with cardiologists and cath lab teams, this role requires advanced cardiac care skills, cardiac catheterization skills, strong communication, and the ability to adapt quickly while maintaining strict safety and regulatory standards.

Essential Work Functions:
  • Conduct assessment of patients prior to procedures, including reviewing medical history and performing physical exams
  • Assist in positioning patients correctly for the procedure and ensure EKG electrodes and cardiac leads are properly placed
  • Assist physicians during procedures, including the placement of catheters, wires, balloons, stents, structural heart devices, and other cardiac interventions
  • Operate and monitor hemodynamic equipment, recognizing and responding to changes in cardiac rhythms and hemodynamic waveforms
  • Assist in arterial and/or venous sheath removal using closure devices, manual pressure, or mechanical compression, and transport patients to the appropriate recovery area
  • Monitor vital signs, ECG, and hemodynamic parameters during cardiac catheterization and interventional procedure
  • Support post-procedure care, including monitoring patient recovery and ensuring a smooth transition to the next phase of care
  • Maintain and troubleshoot cath lab equipment, ensuring everything is functioning properly and fully stocked for procedures
  • Communicate effectively with team members, patients, families, and physicians to provide seamless care
  • Adhere to hospital safety protocols, infection control guidelines, and regulatory standards
  • Perform other duties as assigned within the scope of practice
Required Essential Skills:
  • Critical thinking, service excellence, and good interpersonal communication skills
  • The ability to read, write, and communicate in the English language
  • Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills
  • Physical Abilities - Must be able to remain in a stationary position, move about, move equipment (50-100lbs), push, pull, and bend
  • Interpersonal Skills - Must be able to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism
  • Technical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipment
  • Mental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure. Must be able to work under close supervision occasionally, as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail
  • Sensory - Must possess visual acuity and ability to effectively communicate

Phelps Health Medical located in Rolla, MO is a Med Surg Unit. Phelps Health, which includes a hospital licensed for 240 patient beds, serves a six-county area, with its main campus and several clinics located in Rolla, Missouri. Phelps Health also has clinics in Salem, St. James, Vienna and Waynesville, Missouri. Our Unit is a very fast-paced yet fun environment with endless opportunities to learn and grow!

What we're looking for: We are looking for a dynamic Occupational Therapist who is passionate about helping others and is a team player!

What you will do in this role :

· Develop a specialized treatment plan to help him/her achieve as much as possible in regaining his/her well-being through exercise and/or use of prosthetic and orthotic devices. You and your staff will play an important role in his/her recovery

· Communicate patient progress or problems to supervisor and other team members; instruct patient's family or nursing staff in follow-through programs

· Document patient care in accordance with regulatory, licensing, payer and accrediting requirements

· Maintain equipment and work area in a safe and clean condition

· Handle job responsibilities in accordance with the standards of the Company's Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standards and applicable state/federal laws

· Other duties as assigned

Qualifications:

· Degree from an accredited Occupational Therapy program

· Current and unrestricted Occupational Therapy license in the state where services are rendered

· Current CPR certification

· Strong organizational and communication skills

· Accepting new Grads
