Provides after-hour care of patient needs, supports critical patient communications, and maintains working knowledge of specialized processes including but not limited to: Receiving and communicating critical lab value reporting, heart monitor alerts, hospital discharges, AMS patients, patient and provider appointment management, and other miscellaneous tasks as outlined within onboarding and orientation. Performs essential Charge RN functions in rotation with other team members that directly supports immediate team needs, operational flows, and patient care support. Cross training to all Patient Contact Center departments to assist with patient care needs requiring medication refills, telephone triage, and patient portal electronic communications.

The RN Healthy Beginnings is a professional caregiver who is a member of the interdisciplinary team and is responsible for providing comprehensive nursing services to women and their families eligible for the Nurse-Family Partnership Program(NFP). Nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines, and standards of NFP and of the Carle Health System. Additional responsibilities include the coordination and provision of direct and indirect patient care using the nursing process to meet the physical, psychosocial, environmental, and spiritual needs of Healthy Beginnings specific patient populations and families throughout the geographical area.

Responsibilities

Provides home visits to women and their families eligible for the NFP Program.

Coordinates the care/services for each client; carries a case load of up to 25 clients.

As requested/required, participates in community coalition in support of NFP as well as promotes public awareness of NFP and the Healthy Beginnings program.

Develops and maintains community relationships to support client referrals.

Follows nursing process in implementing NFP into nursing practice; Develops therapeutic relationships, utilize concepts of reflection and motivational interviewing with women and their families in a home visiting environment.

Assesses physical, emotional, social, spiritual, and environmental needs of women and their families as they relate to the NFP domains.

Assists women and their families in establishing goals and outcomes; Provides education, support and referral resources in assisting women and their families in attaining their targeted goals.

Participates in review and analysis of reports for achievements and areas for improvement as well as quality improvement efforts.

Formulates nursing diagnosis based on nursing assessment and client goals and then evaluates client progress toward stated goals and NFP outcomes.

Plans home visits as well as joint home visits with Healthy Beginnings Nurse Supervisor in accordance with client goal and NFP outcomes and expectations.

Consults and collaborates with other professionals involved in providing services to women and families.

Understands, supports, represents, and coaches others in the NFP vision, mission, and model in actions and verbally to both internal and external customers and colleagues.
