The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.  

  • Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.  

  • Evaluates the patient’s functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care.  

  • Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition. 

  • Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.

Northern Dutchess Hospital, affiliated with Nuvance Health, is seeking a full-time outpatient Endocrinologist to join our established and busy employed practice in Kingston, NY with an APRN.


 


The ideal candidate will be board certified in Internal or Family Medicine and board certified/eligible in Endocrinology and looking to join a dedicated and collegial team with some teaching responsibility to residents and medical students. The position is completely outpatient.  This position will have no call responsibilities.  This is an adult only, Monday-Friday practice. There will also be opportunity to expand coverage to other area office locations as we continue to grow the practice to meet the needs of our large primary care referral base. There may be flexibility to work a few days of the week in Kingston and a few days in Poughkeepsie if mutually agreeable.

Job Requirements:

  • MD or DO with NY State and Federal Narcotic Use Registration.

We are seeking a motivated and compassionate Travel Physical Therapist Assistant (PTA) to join our team for 13-week contract assignments nationwide! In this role, you will work under the supervision of a licensed Physical Therapist to help patients regain mobility, strength, and independence.   • Provide education to patients and caregivers on exercises and rehabilitation techniques. 
  • Ensure a safe and supportive therapy environment by maintaining equipment and assisting with patient mobility. 
  • Collaborate with healthcare teams to deliver comprehensive, patient-centered care. 

Requirements:  

  • Associate’s degree from an accredited Physical Therapist Assistant (PTA) program. 
  • Active PTA license in the state of practice or eligibility for licensure in multiple states (licensing assistance available). 
  • Minimum of 1 year of clinical experience preferred; new grads with strong skills are encouraged to apply. 
  • Ability to quickly adapt to new settings and patient populations. 
  • Strong communication, documentation, and teamwork skills. 
  • Passion for patient care and a willingness to travel for assignments.
