As a Home Health Physical Therapist , you will:
Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities to attain highest level of physical function.
Test/screen the patient’s physical strengths to assist the physician in evaluating the patient’s level of function and records findings to develop or pursue treatment programs and establish measurable training objectives.
Develop/implement a conditioning/rehabilitation program consistent with physician’s Plan of Treatment and the overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.
Confer with physician and clinical team members to obtain additional patient information and assist in developing, implementing and revising the therapy treatment program and Plan of Treatment.
Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plan that reflects current plan of care as related to therapy, supervise/evaluate staffs' performance. Monitor the appropriate completion of documentation by physical therapy assistants and home health aides/personal care workers as part of the supervisory/leadership responsibility.
Accurately, promptly and thoroughly document patients' care observations, interventions and evaluations. Assure that interim (verbal) orders received from the physician and physical therapist are promptly and accurately documented, submitted for physician signature and implemented
Report patient's progress to the patient's physician, Clinical Manager, staff, patient and family. Submit evaluation, treatment plans, progress reports and discharge summary to the supervisor and care management staff.

Minimum of one year physical therapy experience preferred

Join our busy 20-bed ED with an additional 6 patient processing bays and a 6-bed Crisis Stabilization Unit for Behavioral Health patients. With a nurse-to-patient ratio of 1:4/5, our diverse staff includes RNs, LPNs, CNAs, Paramedics, Unit Secretaries and Providers whose goal is to create a collaborative environment to deliver exceptional care. We take pride in our facility being a Certified Chest Pain and Primary Stroke Center. With an average daily census of 106 patients, this role offers the opportunity to contribute to high-quality emergency medical care in a fast-paced environment.

Join our team in a 7-bed Operating Room (OR) with 1:1 patient ratio. Our OR specializes in General Surgery, Urology, GYN, Ortho, ENT and Cataract services. RNs typically work 8-hour shifts, can vary based on cases. Night, weekend and holiday call is required. Must reside within a 30-minute radius for call or stay onsite, on-call room is available. Join our team in a 90-bed Skilled Nursing & Rehabilitation Unit, with 20 Rehab beds and 70 LTC beds. Staffed with Registered Nurses (RNs), Certified Nursing Assistants (CNAs), and Medication Aides. Operating with a patient ratio of 1:15 days, 1:19 evenings and 1:25 nights, our unit cares for patients with Congestive Heart Failure (CHF), COPD, Diabetes, ESRD, Renal Failure, Dementia, Stroke, and Heart Disease. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs.

Join our busy 20-bed ED with an additional 6 patient processing bays and a 6-bed Crisis Stabilization Unit for Behavioral Health patients. With a nurse-to-patient ratio of 1:4/5, our diverse staff includes RNs, LPNs, CNAs, Paramedics, Unit Secretaries and Providers whose goal is to create a collaborative environment to deliver exceptional care.
