As a Home Health Occupational Therapist , you will:

  • Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.  Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient’s level of function.

  • Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care Treatment.

  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.

  • Review/expand the retraining programs in a manner consistent with the behavioral goals of each patient and within the guidelines of the rehabilitation program as a whole.

  • Consult with other vocational team members, as appropriate, to develop and implement vocationally oriented plans consistent with the needs and capabilities of patients.

  • Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient and family members.

  • Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's needs.

  • Design community reintegration activities to assist the client in the physical reconditioning effort, and/or the psychological adjustment and coordinate the plan with members of the interdisciplinary team.

  • Recommend and/or design special adaptive equipment for clients to improve residence or working environments or improve their participation in the rehabilitation program and/or community.

  • Design/train staff and family members to carry out the retraining program including dressing, feeding, grooming and hygiene skills, participate in team and family educational meetings.

  • Coordinate/oversee/supervise/instruct and evaluates Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.

  • Accurately and thoroughly document patients' care observations, interventions and evaluation on the day services are rendered.  Ensure that interim (verbal) orders received from the physician are accurately documented and implemented.  Submit evaluation, treatment plans and discharge summary to the supervisor and care management staff.

Required Experience/Skills:

  • Degree from an accredited Occupational Therapy Program

  • A minimum of six months of occupational therapy experience preferred

  • Home Health experience a plus

  • Current and unrestricted OT licensure

  • Current CPR certification

  • Good organizational and communication skills

  • Valid driver’s license, auto insurance and reliable transportation.


Pay Range
•    $49.00 - $9.00 - pay per visit/unit

Assesses, plans, implements, and evaluates the individualized occupational therapy care provided to a patient. Responsible for the coordination of care of all assigned patients.   • 1 year of clinical experience as an Occupational Therapist in an acute or Rehab setting

In this position, you make it possible for patients to recuperate from illness or injury in the comfort of their own home.

Full-Time visits needed Monday - Friday for Citrus County

Essential Job Functions:

  • Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
  • Responsible for administration, evaluation, and interpretation of tests and measurements to determine functional goals and outcomes.
  • Provide a written evaluation and plan of care defining physical therapy needs, goals, progress and expected outcomes.
  • Delivery of physical therapy services to comply with prescribed physician orders.
  • Maintains plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determine discharge care plans as appropriate.
  • Responsible for discharge planning, including collaborating and educating the patient and his/her caretakers on home exercise plan to follow discharge.
  • Participates in coordination of services with health care team, physicians, parent/guardians, nurses, etc.
  • Documents according to policy and procedure and requires minimal supervision or instruction.
  • Documents effectively resulting in no technical or clinical denials subsequent to review of documentation by payers
  • Maintain effective communication between staff, healthcare team members and family. Serve as a resource consultant for clinical staff.
  • Supervises other personnel (PTA, HHA) as applicable.
  • Completes OASIS assessments where allowed by state professional practice and regulation.
  • Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
  • Maintain effective communication between staff, healthcare team members and family. Serve as a resource consultant for clinical staff.
  • Supervises other personnel (OTA, HHA) as applicable.
  • Completes OASIS assessments where allowed by state professional practice and regulation.

The role you’ll contribute:



Executes physician's orders and notifies physician of necessary updates or changes in a home health patient's condition. Assesses, plans, implements, and evaluates the individualized occupational therapy care provided to a patient. Responsible for the coordination of care of all assigned patients.   • Performs comprehensive initial evaluation on all patients assigned requiring occupational therapy.
  • Perform comprehensive home safety evaluation in relation to activities of daily living and communicates specific needs to the physician and the appropriate team members.
  • Establish a plan of care with goals based on the patient’s current level of functioning and potential for improvement in conjunction with the patient/family/caregiver and in accordance with the physician’s orders .
  • Implement the occupational therapy assessment and care plan, continuously evaluating the patient’s response to program and need for modification.
  • Report pertinent observations to attending physician; changing patient care plan as directed.


The expertise and experiences you’ll need to succeed:



Minimum qualifications:



  • Graduate of an occupational therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association or its equivalent.
  • Registered by the American Occupational Therapy Association.
  • 1 year of clinical experience as an Occupational Therapist in an acute or Rehab setting
  • Occupational Therapist – Registered within the state of FL BCLS
  • Active American Heart Association BLS
  • Valid in state Driver’s License with current car insurance


Preferred qualifications:
