Under the direction of the Chief Behavioral Health Officer, the Licensed Clinical Social Worker is responsible for performing psychological assessments and providing brief evidenced-based treatment using the primary care behavioral health model to adults, older adults, children and their families in a primary care setting. This licensed clinician works closely with primary care providers to develop and implement sound treatment plans. Responds to and provides crisis intervention services when needed; Ensures behavioral health services are delivered in a culturally appropriate fashion and is consistent with State of California laws and regulations, professional standards of care, agency guidelines, and professional ethical guidelines.
Essential Functions of the Job :
- Evaluates, triages and provides evidence-based, brief psychological/behavioral health interventions to patients with physical health, mental health, and/or substance abuse diagnoses.
- Provides consultation to clinical staff and primary care providers (PCPs) as needed regarding the patient's care, treatment recommendations and progress via documentation in the client's record, face to face or telephonic consultation.
- Performs brief assessments of patient needs and, with patient and PCP, develops a negotiated care plan. Evaluates and/or revises care plan as indicated with appropriate documentation in progress notes and communicates this to relevant multi-disciplinary team members.
- Maintains accurate and timely documentation in the electronic medical record of all patients seen, completes forms and documents as required by the department, company, state, and federal agencies, and produces reports of evaluation and/or treatment, as required.
- Performs crisis interventions as needed including but not limited to conducting suicide assessments and makes recommendations to medical providers and patients determines if crisis response services are appropriate for patient safety.
- Works in coordination with the Chief Behavioral Health Officer and Director of Behavioral Health Operations to provide information to healthcare providers about mental illness, behavioral health services, and other topics as requested.
- Participates in meetings and committees, including but not limited to case conferences, treatment team meetings, peer review, multidisciplinary team meetings, etc. Participates in committees and work groups as directed.
- Actively participates in quality assurance activities of the department, including regular peer review and development and revision of policies and procedures.
- Maintains productivity expectations in accordance with clinic and program guidelines.
- Works closely with all providers to establish full integration of services and team-based approach to care.

Under the direct supervision of the BH Case Manager Supervisor and general oversight of the Rural Telehealth Program Manager, the Behavioral Health Case Manager is responsible for providing direct services and coordinating care for behavioral health patients with particular focus on those with complex needs. Specifically, the Case Manager works to ensure patients have access to the care and referral services needed to achieve and maintain optimal functioning. The Case Manager will also serve as the on-site liaison at the Mountain Health Family Medicine clinic for the rural telehealth grant program. The Case Manager is responsible for providing case management and community engagement services to patients engaged in or referred to Behavioral Health Services. The Behavioral Health Case Manager will participate in all-staff meetings, treatment team meetings, conduct presentations, and represent the Rural Telehealth program in internal and external community engagement.
Please note - this is a grant funded position. The grant is due to end September 30, 2028.
Essential Functions of the Job:
- Provides direct services to patients, including case management, referral support, warm handoffs, and psycho-educational groups.
- Conducts a comprehensive assessment of the patients' health and psychosocial needs, including health literacy status and deficits to identify appropriate level of care and create a case management plan in collaboration with family and department staff.
- Begins developing the therapeutic alliance with patients by providing an orientation to the behavioral health department and the services available.
- Identifies and supports patients and families who are eligible for services and in need of case management services. Coordinates patient care with behavioral health provider, Primary Care Physician (PCP), and/or psychiatrist via consultation, collaboration, follow up, and recommendations. Completes warm handoffs for patients identified by PCP as needing behavioral health.
- Collaborates with other SYHealth Departments (e.g. Referrals, Social Services, Health Education, etc.) to facilitate and monitor referral requests and processes to ensure timely receipt of services.
- Closely collaborates with Rural Telehealth Program Manager and clinic staff to support scheduling, initiation, and completion of telehealth visits with behavioral health providers at other sites.
- Assists patients and families in accessing resources and overcoming barriers to care.
- Manages internal and external referrals to the behavioral health department, including follow-up phone calls, sending letters, and communicating with referring staff.
- Educates patients, families, caregivers, and members of the health care delivery team about treatment options, community resources, insurance benefits, psychosocial concerns, case management, etc. so that timely and informed decisions can be made.
- Represents San Ysidro Health and the Behavioral Health Department in the community to create and maintain partnerships with other programs and to improve referral outcomes and quality of care for patients.
- Maintains timely, complete, and accurate records with detailed assessment results, action plan, and progress notes. Clearly documents plans discussed with the patient, family, behavioral health clinicians, PCPs, etc. in the electronic health record system to ensure that communication is available to all team members.

- Works closely with Rural Telehealth Program Data Analyst in monitoring and tracking program deliverables and outcomes.

As an Air Interdiction Agent (AIA) you will perform aviation-based law enforcement operations for the detection, prevention, interdiction, and apprehension of terrorists, terrorist weapons, and other contraband and persons from illegally entering or attacking the United States. Conducting air patrol, surveillance, and pursuit activities related to the interdiction of smuggled contraband via land vehicles, aircraft, or vessels.

Qualifications
Experience: You qualify for the GS-11 grade level if you possess one (1) year of specialized professional aviation experience performing duties such as:
Flying as a Pilot-in-Command or sole manipulator in an airplane and helicopter in all environments of flight, including night, poor weather, unfavorable terrain, low altitudes, or speed.
Evaluating information rapidly and making judicious decisions promptly during in-flight operations.
Developing strategies and coordinating aircraft and ground assets.
Using information systems and databases to conduct information surveys, queries, update files and disseminate information.
Experience may include Operation of an aircraft as Captain, Pilot in Command, Aircraft Commander, First Officer, Certified Flight Instructor, Second in Command, or Co-pilot.
Hiring Minimums:
Certification & Ratings: A current FAA Commercial or ATP Pilot Certification with one of the following ratings:
Dual Rated: Airplane (Single-engine land or multi-engine land) with instrument rating AND Rotorcraft Helicopter with instrument rating.
Airplane Rated: Airplane (Single-engine land or multi-engine land) with instrument.
Helicopter Rated: Rotorcraft Helicopter with instrument rating.
Equivalent military rating of the above at the time of application (eligibility based on military flight experience must provide official orders, forms or logbooks showing their status as a rated military pilot).
Flight Hours: Pilot Enter on Duty minimums are 1500 flight hours; (up to 500 hours can be waived, reducing the pilot enter on duty minimums to 1000 hours.
250 Pilot-in-Command hours; 75 Instrument hours; 75 Night hours (Flight Hour Waiver available, see Required Documents); and 100 Flight hours in the last 12 months This qualification requirement is currently being waived by OPM through August 5, 2025.
FAA Class 2 medical required for assessment dated within the last 12 calendar months; FAA Class 1, FAA Class 2 or Military medical flight clearance dated within the last 12 months qualifies to start the pre-employment process for the AIA position.
Apply at 750 hours total time: Applicants applying at 750 flight hours are required to obtain at least 1,000 flight hours (depending on the number of hours approved for a waiver) at their own expense before being able to attend the 3-part flight assessment. Applicants must still meet 250 Pilot-in-Command, 75 instrument, and 75-night hours (100 flight hours in the last 12 months. This qualification requirement is currently being waived by OPM through August 5, 2025)
UAS Flight Hours: Applicants may include UAS Predator A (MQ-1), Predator B (MQ-9) flight hours and Predator A (MQ-1), Predator B (MQ-9) or predator-based flight hours. These hours may be credited towards satisfying the ‘Total flight time‘ 1,500 flight hour requirement only. UAS hours do not count towards the 250 flight hours as a Pilot in Command, 75 flight hours Instrument, and 75 flight hours Night. Flight hour logbooks will be required at the time of your Flight Assessment for verification.

Duties and Responsibilities
As an Air Interdiction Agent (AIA) you will perform aviation-based law enforcement operations for the detection, prevention, interdiction, and apprehension of terrorists, terrorist weapons, and other contraband and persons from illegally entering or attacking the United States. Typical duties include:
Conducting air patrol, surveillance, and pursuit activities related to the interdiction of smuggled contraband via land vehicles, aircraft, or vessels.
Monitoring behavior patterns and activities of suspect persons, vehicles, or vessels believed to be engaged in illegal activities.
Collecting, refining, and analyzing strategic and tactical intelligence.
Supporting search and rescue and humanitarian efforts.
Salary and Benefits
Begin your career as an Air Interdiction Agent (AIA) and make up to $106,000 -$27,000 per year based on your qualifications for the GS-11 grade level and possible extra compensation based on your duty location (see below) and overtime pay, up to 25% of your salary.
Your starting salary will include Special Salary Rate (SSR); Law Enforcement Availability Pay (LEAP); Recruitment Incentive; and Retention Incentive.
This career ladder position has a grade level progression of GS-11, GS-12, and GS-13. You may be eligible for a promotion to the next higher grade level automatically (without re-applying) once you complete 52 weeks at each grade level (with supervisor approval).
