BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country.

The Central Referral Center (CRC) for BJC Home Care Services is where the patient referral process starts. There are Intake Coordinators in each BJC hospital who coordinate services for those patients that need home care. The patients who choose BJC Home Care Services are then channeled through the CRC. The CRC also consists of nurses who take physician office referrals for patients not currently hospitalized. There is someone on-call 24/7 for the CRC to facilitate home care coordination for patients in emergency rooms, etc.

Role Purpose

Registered Nurse evaluates the client and furnishes services requiring substantial and specialized skill, appropriate preventive and rehabilitative nursing procedures, and instructions to assist the client in learning appropriate self-care techniques. When assigned as case manager, the staff nurse is responsible for coordinating all aspects of care related to that patient.

The care is moderate to high acuity providing care with a high complexity of interventions which includes Leukemia care, Autologous Stem Cell transplants, Donor cellular infusions, post allogeneic complications, Graft vs. Host disease and a focus on Clinical trials. The multidisciplinary group of care providers includes Bone Marrow transplant physicians, oncology nurses, Case managers, coordinators, social workers, spiritual care, pharmacy and psycho-social support.

Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life-limiting illness. We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying. Thoracic Surgery is a fast paced surgical stepdown unit with 6 PCU beds and 18 floor beds. We care for complex thoracic surgeries, including lung transplant, thoracotomy, esophagectomy, thymectomy, hiatal hernia repair. We are a Dedicated Education Unit, specializing in training nurses. This unit provides experience for nurses who are interested in promoting to critical care, including hemodynamic monitoring, IV drip titration, chest tube management and critical thinking skills.

Role Purpose

Provides direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care.
