During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits.  The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

What to Expect :

• Conduct Adult/Geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols

Type: PRN - Flexible-Nurse Practitioner or Physician Assistant

PAY:  Earn from $2,300 up to $1,500 per month (based on an average of 20-100 completed visits per month and pay of 115.00 per completed assessment).  Compensation range is based on availability and assessment completion.

Location: Home, SNF, and other community environments in Ithaca, New York

Hours : Days, Evenings, Nights, Weekends, Flexible Scheduling

Must Haves:

• Master’s Degree required OR commensurate experience and satisfactory completion of NP licensure
• Master’s Degree required OR commensurate experience and satisfactory completion of PA licensure
• Current RN and NP or PA licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority
• Board certified by the NCCPA, AANP, ANCC or the AACCN in a Matrix approved specialty

Base Rate is 28.00 - 35.00 with an additional .75 cent shift differential for evenings and nights

Corning Center  is actively seeking motivated Licensed Practical Nurses  for our Skilled Nursing Facility located in Corning, NY. The ideal candidate will have a pleasant demeanor and excellent communication skills!

Duties:

• Collecting required information from new Residents to be admitted
• Recording health details of Residents; including vitals & temperature
• Administering medications and injections to Residents as needed
• Treating and dressing wounds and bedsores as needed
• May be required to supervise Certified Nursing Assistants (CNAs)
• Helps Residents get dressed & take care of personal hygiene
• Monitors Residents’ food and liquid intake and output

Requirements:

• Must be able to work as a team member
• Valid LPN license (any state)
• In good standing with State Registry
