- Assess the clinical nutrition needs of residents/patients and develop and document the plan within the electronic medical record.
- Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities
- Provide nutrition education and counseling to residents/patients, families, and staff.
- Collaborate with interdisciplinary teams to determine a plan of care that improves residents'/patients’ overall health and well-being.
- Ensure adherence to CMS, State, and Federal regulations and company standards.
- Provide coverage in multiple buildings based on company needs, and as desired.
- All other duties as assigned.
Qualifications
- Registered by the Commission on Dietetic Registration. **
- Hold a Certification/License in good standing within the state of practice.
- Knowledge of the state survey process and compliance with nutrition care regulations (preferred, but not required).
- Strong organizational and communication skills.
- Maintain a positive attitude and effectively communicate with building occupants, administration, and HCSG leadership to perform daily tasks.
- Compliance with COVID-19 vaccination policies as required by state, local, and/or customers.
- If multi-site registered dietitian, must have valid Driver's License

Coordinates clinical activities of the Inpatient Wound Care Program which include but aren't limited to utilization of skills in organization, planning, implementing, evaluating, and providing care through use of hospital and nursing standards.

Delivers direct patient care appropriate to the age of patient served.
Assess and diagnose wounds, ostomies and continence disorders
Develop and implement treatment plans
Monitor and evaluate patient progress
Provide patient education on self-care and disease management
Collaborate with other healthcare professionals to ensure holistic care
Stay up-to-date with the latest research and technology in wound, ostomy and continence care
Document patient progress and treatment plans accurately and thoroughly
Provide emotional support to patients and their families
Manage and maintain specialized equipment and supplies
Advocate for the needs and rights of patients with wounds, ostomy and continence disorders
Manage complex wounds, ostomies and continence issues
Qualifications:
JOB REQUIREMENTS
Minimum Education
Graduate of a Program in Discipline - Required
Bachelor's Degree preferred.
Required Skills
Must be able to work independently
Addresses wound care consults timely
Ability to teach staff on wound care, staging, & documentation
Professionally interact with patients
Ability to teach patients how to manage their wound care
Facilitates Pressure Injury monthly meeting
Accurately assesses patient skin integrity
Prevent skin breakdown in hospitalized patients
Accurately stage and assess wounds
Microsoft office skills - Word, Excel, PowerPoint
Certifications:
â–¡ Basic Life Support (BLS) - to be obtained within 6 months
One to two years related experience and/or training preferred.
Required Licenses
(West Virginia, United States) Other
Current licensure by WV State Board of Nursing.
Minimum Work Experience
Wound Care Certification preferred.
Two years of experience in wound, ostomy and continence care required per Wound Ostomy Continence Nursing Certification Board.

Responsibilities
- Assess the clinical nutrition needs of residents/patients and develop and document the plan within the electronic medical record.
- Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities
- Provide nutrition education and counseling to residents/patients, families, and staff.
- Collaborate with interdisciplinary teams to determine a plan of care that improves residents'/patients’ overall health and well-being.
- Ensure adherence to CMS, State, and Federal regulations and company standards.
- Provide coverage in multiple buildings based on company needs, and as desired.
- All other duties as assigned.
Qualifications
- Registered by the Commission on Dietetic Registration. **
- Hold a Certification/License in good standing within the state of practice.
- Knowledge of the state survey process and compliance with nutrition care regulations (preferred, but not required).
- Strong organizational and communication skills.
- Maintain a positive attitude and effectively communicate with building occupants, administration, and HCSG leadership to perform daily tasks.
- Compliance with COVID-19 vaccination policies as required by state, local, and/or customers.
- If multi-site registered dietitian, must have valid Driver's License
