The Home Health Occupational Therapy (OT) Care Manager is a professional therapist who coordinates and directs the home care patient's services based on individual patient needs. The OT Care Manager is responsible for independent management of the Home Health patient population requiring the use of advanced assessment, teaching and decision-making skills. The OT is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. Relevant knowledge and experience is consistently applied to new patient populations. The OT Care Manager cares for a caseload of home health patients requiring occupational therapy as the primary service by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician. S/he educates patients, families, caregivers and community providers to safely perform care. · Assesses physical, functional, psychosocial, social, spiritual, educational, developmental, cultural, cognitive status and discharge planning needs of the home care patient utilizing interview observations and physical exam techniques. Assesses the home environment for safety, infection control, and community resource needs. Reviews patient history and physical, diagnostics and laboratory data. Reviews available information obtained by other team members. Reports abnormal items and results to the physician as appropriate and reviews with patient family. Accurately and timely documents these assessments.

· Utilizing assessment data, formulates a patient specific plan of care along with the patient, family and physician which is feasible within the physical, financial and emotional resources of the family. Establishes individualized, realistic, measurable patient centered goals in consultation with the patient, family and other health care providers including goals to improve or stabilize patient’s medical condition, functional abilities and promote independence. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient when developing the plan of care.

The expertise and experience you’ll need to succeed:

Minimum qualifications:

· Master's Degree in Occupational Therapy

· 1 year relevant clinical occupational therapy experience

· Occupational Therapy

· Active American Heart Association BLS

· Valid in state Driver’s License with current car insurance

Preferred qualifications:

· Recent, relevant experience in a Medicare-certified home health agency as a case-manager

Deliver professional nursing care in the operative setting to patients undergoing surgical or other invasive procedures.

• Perform scrub functions including the selection and handling of instruments and supplies used during procedure. 
• Perform circulating functions including monitoring, recording, and communicating patient condition and managing overall nursing care of patient before, during and after procedure. 
• Directly assists operating physician with surgical tasks including hemostasis, suturing, and wound exposure as well as patient positioning.
• Prepare operating rooms and surgical instruments and equipment for use.

Licensure / Certification / Registration:

• Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job
  transfer date required.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of
  hire.

Coordinate unit staffing and provide leadership while ensuring implementation of clinical objectives and quality patient care for one or more assigned nursing specialty areas: Cardiovascular/Cath Lab, Surgical Services, Emergency/Triage, Endoscopy, Adult or Pediatric/NeoNatal Critical Care, Labor & Delivery/Women & Family.

Responsibilities:

• Assign, direct, educate and monitor nursing and support associates during assigned shifts.
• Meet with staff and exercise independent judgment to plan the shifts, services, and patient care.
• Assess daily patient care needs and develops and distributes patient care assignments, ensuring that population appropriate patient care is given and is accountable for maintaining productivity standards and justifying variances to unit manager or designee.
• Respond to complaints about patient care and manages through established channels.
• Facilitate availability of adequate material, resources and supplies.
• Set priorities appropriate to daily patient/physician dynamics.
• Provide input into hiring, training, directing, development and evaluation of staff.

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse obtained prior to hire date or job transfer date required.

Education:

• Required professional licensure/certification AND 1 year of cumulative job specific experience required.
