The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.
- Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
- Evaluates the patient’s functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care.
- Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
- Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.

Northern Dutchess Hospital, affiliated with Nuvance Health, is seeking a full-time outpatient Endocrinologist to join our established and busy employed practice in Kingston, NY with an APRN.
The ideal candidate will be board certified in Internal or Family Medicine and board certified/eligible in Endocrinology and looking to join a dedicated and collegial team with some teaching responsibility to residents and medical students. The position is completely outpatient. This position will have no call responsibilities. This is an adult only, Monday-Friday practice. There will also be opportunity to expand coverage to other area office locations as we continue to grow the practice to meet the needs of our large primary care referral base. There may be flexibility to work a few days of the week in Kingston and a few days in Poughkeepsie if mutually agreeable.

Job Description
Implements physician orders, administers medications, starts IVs, performs treatments, procedures and special tests, and document treatment as required by company policy and local/state/federal rules and regulations.
Orders, interprets, and evaluates diagnostic tests to identify and assess patients' conditions.
Assesses and evaluates patient needs for, and responses to, care rendered .
Applies sound nur sing judgment in patient care management decisions.
Provides primary and emergency care for occupational and non-occupational injuries and illnesses.
Administers over-the-counter and prescription medications as ordered.
Collaborates with the nur sing team to create a Plan of Care for all patients.
Directs and guides ancillary personnel and maintain standards of professional nur sing.
Minimum Qualifications
Graduate of a professional academic nur sing program in which a Diploma, Associate Degree or Baccalaureate Degree is conferred
Must hold current NYS Registered Nur se license
Obtains and maintains certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS)
Minimum of 1-3 years of previous clinical RN experience in Med-Surg or Progressive Care
Excellent communication, prioritization, organizational and time-management skills
Possesses knowledge of quality concepts, principles, and problem-solving tools and techniques and demonstrates ability to apply in support of departmental and patient care quality improvement.
