Scope of Position:Responsible for direct oversight of the Pathway of Hope (POH) Program, an intensive case management program to end generational poverty. Manage the ServicePoint client software database. Assist the Social Ministries Director with the oversight of the service extension program and emergency disaster services as needed. Provide on-call coverage support for the Hartford, Waterbury, and New Britain shelters. Supervise the housing and homeless programs and the service extension program in the absence of the Social Ministries Director. Ensure all programs are aligned with The Salvation Army’s mission and policies.

Responsibilities

  • Serve as a role model for all social service employees by displaying a passion for the mission.

  • Assist with annual audits of the social ministry program.

  • Ensure all policies and procedures are followed in all social service and service extension programming.

  • Maintain strict confidentiality.

  • Attend conferences, trainings, and meetings.

  • Support fundraising and positive community relations efforts.

  • Provide direct case consultation and technical assistance to case managers.

Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. SOCIAL WORK ASSISTANT (CASE PLANNER)



University Behavioral Associates (UBA) at Montefiore Medical Center in Bronx, New York is seeking a dedicated and organized bachelors- or masters-level individual with experience in social services to serve as a case planner for the Family Treatment & Rehabilitation specialized preventive program funded by the NYC Administration for Children's Services (ACS).



This community based, family centered program works closely with parents, children, and providers (including ACS) in order to ensure a strong, effective collaboration that supports families in focusing on child safety and well-being. Case planners primarily engage with families in the community through home and school visits, and other community based contacts.



Case planner duties include working closely with families involved with ACS due to mental health or substance use concerns. Families may also have legal involvement. The case planner will be responsible for case management, including conducting ongoing in-home assessments of family members, making referrals to appropriate clinical services, monitoring attendance and engagement at mental health or substance use disorder treatment programs, collaborating with families and community agencies (e.g., schools, probation, ACS). The case planner will also be responsible for ensuring coordination between families, providers, and the legal system, will participate in weekly individual supervision, and will document treatment efforts consistent with ACS and UBA/Montefiore guidelines. Evening and/or weekend availability may be required.



The preferred candidate will have demonstrated experience with child welfare, mental health issues, and/or substance use concerns, as well as experience working with families and the legal system. The preferred candidate will also possess excellent organizational and communication skills. Spanish-speaking strongly preferred.

Health and Human Services Agency: The Shasta County Health and Human Services Agency (HHSA) partners with communities to protect and improve the health and well-being of Shasta County residents. The HHSA promotes optimal health for all. ABOUT THE POSITION
Assistant Social Worker: Under direction, to carry a caseload involving the determination of need for basic social service functions for applicants or clients; to perform elementary social studies to identify a need for more intensive casework services; to provide limited casework services of a general tangible nature; and to do related work as required.
Social Worker: Under direction, to carry a service caseload and to provide social services; to conduct social studies and develop service plans which may include the need for specialized social services; to provide information about available services and to arrange for referral of clients to appropriate departmental or community human services resources; and to do other work as required.
Senior Social Worker: Under direction, to carry a service caseload and to provide social services; to conduct social studies and develop service plans which include the need for specialized social services; to provide information about available services and to arrange for referral of clients to appropriate departmental or community human services resources; or to lead internal reviews of all aspects of the In-Home Supportive Services (IHSS) program to ensure both internal integrity and to investigate potential fraud; perform related work as required.

DISTINGUISHING CHARACTERISTICS
Assistant Social Worker: Assistant Social Worker is the trainee class in the professional Social Worker series. A position in this class is distinguished from that of the next higher class of Social Worker in that the incumbent is given constant and detailed supervision in learning such items as departmental organization, social service programs, basic case study methods and casework services. As experience is gained, employees are assigned selected caseloads where consequence of error is minimal; case recommendations and casework methodology, however, continue to receive close review. In contrast, the Social Worker carries a full caseload, works with greater independence of action, and normally provides case services of average to complex difficulty.
Social Worker: This is the full journey level in the professional Social Worker series. Incumbents spend a substantial part of their time on the more difficult, sensitive, and specialized services where major case decisions are made independently and where consequence of error may be significant. Advanced casework skills, a high degree of program knowledge, judgment and creativity are required. Typical assignments include adoptions, child foster home placement, child protective services screening and intake, juvenile court case consultation and liaison, original licensing for child foster care homes, and initial assessment of adult protective services. This class is distinguished from the Senior Social Worker class in that the latter is expected to manage a difficult caseload at the highest level of skill.
Senior Social Worker: Senior Social Worker is the highest level professionally trained worker in the series performing the most responsible and difficult casework dealing with highly complex family and individual problems in Child Protective Services, Juvenile Court, Adoptions, Placement, or Adult Services. This class is distinguished from the Social Worker class by its specialization in child welfare services requiring casework management at the highest level of skill, or by its advanced knowledge of the technical aspects of a program combined with the highest level of skill and experience in dealing with the complex problems within the senior and disabled adult population.

THESE BENEFITS DO NOT APPLY TO EXTRA HELP EMPLOYEES

Holidays:
  • 12 paid holidays (96 hours) per year.
  • 1 Floating holiday annually.
Vacation:
  • 10 working days a year during the first three years of service.
  • 15 working days a year - 4 through 9 years of service.
  • 17 working days a year - 10 through 15 years of service.
  • 20 working days a year - 16 or more years of service.

Note - accrued vacation leave may not be used until after three months of employment. For purposes of vacation accrual, a working day is no more than 8 hours.

Employees may elect to receive a payment for Vacation hours (in lieu of use) for up to twenty hours - in five whole hour increments - of accrued vacation. *Subject to criteria as outlined in section 13.9C in the UPEC-General MOU.

Sick Leave:
  • 96 hours of sick leave are earned each year with no maximum accrual.
    • Employees terminating in good standing after completion of 5 years of continuous service will receive a pro-rata payoff of accumulated sick leave.
    • Unused sick leave may be converted to PERS service credit upon retirement.

Note - sick leave may be used immediately as accrued. Accrued paid leave benefits are used in conjunction with FMLA/CFRA as appropriate.

*Vacation and Sick Leave accruals are based on regularly scheduled working hours. Additional time will not be accrued on hours worked over 80 hours within a one pay period.
Bereavement:
  • 24 hours paid leave for each non-concurrent death in the immediate family.
Health Plan:
Shasta County contracts with PERS for medical insurance.
Depending on the plan selected, the County pays most of the employee premium, and contributes a portion of the dependent premium.
Contributions are made for a portion of a retiree's insurance premium.

Dental Plan:
The County pays a portion of the employee's and dependents' premium for dental insurance. The employee pays the full premium during the first six months of coverage.

Vision Plan:
The County pays the employee premium and employees may enroll dependents at their own expense.

Life Insurance:
The County pays the premium for life insurance for most employees. The amount of coverage varies by bargaining unit. For UPEC-General employees the current coverage amount is 25,000.

Additional Life insurance is available to purchase through Aflac and The Standard.

Retirement:
The County is in the Public Employees' Retirement System (CalPERS) coordinated with Social Security.
  • Members hired prior to May 8, 2011 are covered under the retirement formula of 2% @ 55 benefit.
  • Members hired after May 8, 2011 but before January 1, 2013 are covered under the retirement formula of 2% @ 60 benefit.
  • Members hired after January 1, 2013 are covered under the California Public Employees Pension Reform Act (PEPRA) with a benefit formula of 2% @ 62.
Please review the specific MOU for stipulated retirement formulas.

Employees pay 9.5% of his or her salary, on a pre-tax basis for a period of time as specified in his or her labor contract, or Personnel Rules as appropriate. Members hired prior to 2013 may have a different contribution percentage.

Miscellaneous:
  • Deferred Compensation plans available through specified companies with employer match up to 3%
  • IRC Section 125 plan available for pre-tax payroll deductions for contributions towards un-reimbursed medical expenses and dependent care.

SOCIAL WORK ASSISTANT (CASE PLANNER) University Behavioral Associates (UBA) at Montefiore Medical Center in Bronx, New York is seeking a dedicated and organized bachelors- or masters-level individual with experience in social services to serve as a case planner for the Family Treatment & Rehabilitation specialized preventive program funded by the NYC Administration for Children's Services (ACS). This community based, family centered program works closely with parents, children, and providers (including ACS) in order to ensure a strong, effective collaboration that supports families in focusing on child safety and well-being. Case planners primarily engage with families in the community through home and school visits, and other community based contacts. Case planner duties include working closely with families involved with ACS due to mental health or substance use concerns. Families may also have legal involvement. The case planner will be responsible for case management, including conducting ongoing in-home assessments of family members, making referrals to appropriate clinical services, monitoring attendance and engagement at mental health or substance use disorder treatment programs, collaborating with families and community agencies (e.g., schools, probation, ACS). The case planner will also be responsible for ensuring coordination between families, providers, and the legal system, will participate in weekly individual supervision, and will document treatment efforts consistent with ACS and UBA/Montefiore guidelines. Evening and/or weekend availability may be required. The preferred candidate will have demonstrated experience with child welfare, mental health issues, and/or substance use concerns, as well as experience working with families and the legal system. The preferred candidate will also possess excellent organizational and communication skills. Spanish-speaking strongly preferred.
