Reviews and approves Case Conference/IDG Coordination notes submitted by case managers and attach to Episode Detail Report. Enters Case Conference/IDG coordination notes within 48 hours of the Case Conference/IDG meeting. Contacts physicians to obtain orders for continued service provision or add-on services.
- Reviews orders as they appear on the action screen. Approves or declines as appropriate. Follows up with Licensed Professional as necessary when editing an order. For hospice, approves level of care orders and ensure appropriate criteria is met.
- Enters and approves all orders for tracking purposes and route to Medical Records to be sent for physician signature. Ensures any corrections are made by the Licensed Professional who wrote the order prior to approving the order. Updates the client’s medication profile and schedule as applicable; via the order. Ensures all orders that address frequency have appropriate calendar modification completed.
- Ensures that there are existing orders for requested medical supplies.
- Follows up on orders from the Order Tracking Report when the MRS is unable to retrieve the unsigned order.
- Enters detailed Non-Admit information into HCHB in Coordination Notes if no visit was made. Ensures the Clinical Manager approved the non-admission.

- Complete review of evaluation packets, OASIS/HIS and 485/Plan of Cares (POC). Assures consistency with the comprehensive assessment and the 485/POC. Reviews both the data submitted from the field staff to ensure accuracy and follows up on any documentation that requires correction.
- Ensures that patient is appropriate for homecare/hospice based on required elements, including homebound status for homecare and eligibility guidelines for hospice.

- Complete review of evaluation packets, OASIS/HIS and 485/Plan of Cares (POC). Assures consistency with the comprehensive assessment and the 485/POC. Reviews both the data submitted from the field staff to ensure accuracy and follows up on any documentation that requires correction.
- Ensures that patient is appropriate for homecare/hospice based on required elements, including homebound status for homecare and eligibility guidelines for hospice.
- Ensures service utilization correlates with assessment data.
- Processes the OASIS/HIS and 485/POC, and verifies the correct start of care date and episode date range.
- Processes any unlisted supplies, medications, activity, functional limitations, allergies, etc. that appear on the action screen to assure completion of the 485/POC.
- Ensures that appropriate Care Types and Nursing Diagnosis Pathways (NDPs) are selected based on clients medical condition and staffs assessment data.
- Reviews ROC and Discharge Assessment Data to ensure accuracy and follows up on any documentation that requires correction.
- Processes the ROC and Discharge OASIS/HIS and orders.
- Conduct OASIS/HIS and documentation portions of the Job Specific Orientation in collaboration with Clinical Field Staff Supervisor and HomeCare Support Analyst.
- Assures that the clients plan of care is completed and reassessed by the appropriate health care professional when there is a significant health status change in the clients condition, at the physicians request and after hospital discharge. Assures appropriate documentation is completed for all patients transferred to an inpatient facility.
- Identifies additional training needs for employees including OASIS/HIS documentation.
- Participate in on-call rotation
- Provides for ongoing supervision and evaluation of assigned Homecare staff.
- Reviews Infection Control reports for all clients. Provides/documents all follow up related to Infection Control Reports (both client and employee).
- Reviews Occurrence Reports. Provides/documents all follow up related to Occurrences.
- Participates in community awareness programs as necessary.
- Takes administrative on-call when required as a support to the on-call nurse and provides software management related to processing intakes, reviewing evaluation packets, and scheduling, moving, reassigning visits to ensure the smooth function of the agency during off hours.
- Follows up on OASIS/HIS Assessments that cannot be processed due to Licensed Professional document deficiencies.
- Serves as a back up to the Clinical Field Staff Supervisor (CFSS).
- Communicates information among departments as necessary for quality care and process improvement.
- Assists timekeeper with approvals and scheduling of CTO time.
- Analyzes and identifies excess staff, or need for additional staffing, according to approved staffing ratios and submits to Clinical Manager for approval/action.
- Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
- Perform other related duties as assigned.
Qualifications
Minimum Education :
None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.
Minimum Experience:
One (1) year of home health or hospice experience required. Three (3) years as home health field staff nurse preferred with broad experience in use of high tech equipment; knowledge of Medicare guidelines. Excellent communication skills; Conflict resolution skills; Effective teambuilding skills; Organization and Delegation skills; Ability to confront difficult situations effectively. Preference may be given to individuals with supervisory, management experience.
Licensure Requirement:
Must have and maintain a RN license in state of Tennessee. OASIS certification (may be obtained within one (1) year of employment). Hospice and Palliative Care Certification required for Hospice (may be obtained within one (1) year of employment). Employee must have and maintan a Tennessee drivers license and state mandated minimum insurance coverage. Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure. OASIS certified (will provide if not already certified) strongly preferred.
