Key Position Details:

  • Full time position (80 hours every two-week pay period)
  • 8-hour, day shifts

  • This position is on the inpatient Adult - mental health unit(s) at Abbott Northwestern Hospital
  • Every patient on the inpatient mental health units is assigned a mental health social worker during their hospital stay
  • The Inpatient Mental Health Social Worker works very closely with the multidisciplinary inpatient care team including the psychiatrist/nurse practitioner, nurse, psychologist, internal medicine provider, clinical therapy group staff, mental health coordinator, pharmacy and additional team members as needed
  • In this role, the social worker will meet with patients to complete a comprehensive bio-psycho-social assessment, assist with civil commitment/court related paperwork, exams and hearings as needed, coordinate with outpatient supports/providers/care team/ family and help to develop a comprehensive discharge plan for each patient to ensure they have the support needed for a successful transition back into the community
  • For those seeking licensure - For full-time employees, we offer 4 hours of Clinical Supervision per month (2 Individual clinical supervision hours and 2 Group clinical supervision hours)
  • Possible cross coverage on child adolescent unit, but none currently
  • NOTE: This position does not require WI Social Work license or Care Management certification

Job Description:

Provides patients and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.

Principle Responsibilities

  • Delivers professional and thorough social work services, including psychosocial assessment and intervention planning.
    • Identifies patients who require social work assessment and intervention through high risk screening, interdisciplinary team meetings and individual referrals.
    • Conducts assessments that address bio-psycho-social issues for age, population and health specific needs which results in individualized plans of care.
    • Provides support and counseling to patients and families.
    • Provides information and assistance for identified financial or social needs.
  • Coordinates complex transition plans with patients, families, health care team and community providers.
    • Utilizes the electronic medical record to monitor, document and communicate patient progress toward goals and progression of the social work plan.
    • Collaborates and communicates with interdisciplinary team anticipating needs to move the plan of care forward.
    • Provides support and information to patient and families regarding transition plan.
    • Maintains knowledge of government and private payer networks and services to assure appropriate transitions.
    • Collaborates with community and health care resources based on need to coordinate care for the patient.
  • Advocates for patients and families by supporting patient rights and accessing protective services.
    • Demonstrates awareness of patient rights and ethical decision making; provides advocacy to support patient and family.
    • Assures appropriate reporting of vulnerability or suspected abuse as mandated by law.
  • May participate in care system process that prevent readmissions.
    • Plans and participates in transition conferences with patients and families.
    • Utilizes tools and technology to identify and intervene with patients who are at risk for readmission.
    • Ensures that a complete clinical handoff occurs for at risk patient, which may include referrals.
  • May collaborate with the health care team to promote appropriate length of stay.
    • Utilizes tools and technology to support appropriate length of stay management.
    • Facilitates timely referrals and transfers of information.
  • Other duties as assigned.


Required Qualifications

  • Bachelor's degree in Social Work

Licenses/Certifications

  • Licensed Social Worker - MN Board of Social Work required

The Recovery Counselor will assess the client's personal, medical, emotional, social, and environmental situation to plan for treatment course through home, community, and office visits. He/she/they will provide individually-based motivational treatment and support to clients to assist them in their recovery from mental illness. The Recovery Counselor will be an advocate for clients, and link them to community services as needed. He/she/they will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.

The Recovery Counselor will provide at least 75% of all community support services in the community. He/she/they will complete case management tasks with clients within a shared caseload. The Recovery Counselor will be responsible for documenting all services provided to clients according to agency policy and state requirements. The Recovery Counselor will also provide crisis/on-call coverage as directed.

RESPONSIBILITIES
  • Assess client's personal, medical, emotional, social, and environmental situation through home, community, and office visits.
  • Coordinate the establishment of an individual recovery plan with the client, client's support system, and other care providers.
  • Complete daily progress notes based on services provided to clients in a timely manner reflected in Trilogy's documentation policy and agency standards
  • Complete all necessary documentation related to client care e.g. IMCANS, LOCUS, tracking forms in the Electronic Medical Record
  • Recognize and act on opportunities to move clients to appropriate levels of care; provide referrals to additional services as indicated.
  • Assist clients in identifying signs and symptoms of de-compensation, assess for crisis situations and or the need for stabilization through hospitalization. Work collaboratively with clients to identify coping skills to assist in managing symptoms and stressors experienced.
  • Provide psychoeducation, medication training and monitoring to clients according to Trilogy policy
  • Coordinate with team nurse/pharmacy to ensure client medication accuracy
  • Documenting in real time on medication administration record (MAR)
  • Assist clients in learning and improving independent living skills; i.e. personal hygiene, housekeeping skills, nutrition, and shopping for food and personal items
  • Educate and assist clients with applying for entitlements; i.e. Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and LINK
  • Understand the representative payeeship process per agency policy and providing individualized client money management services.
  • Accompany and transport clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, and medication.
  • Advocate on client's behalf and empower clients to advocate on behalf of themselves when appropriate
  • Communicate effectively within the team model for a multi-disciplinary approach to client care.
  • Actively participate in team communication via multiple electronic platforms
  • Engage regularly in daily team meetings to consult and collaborate on shared caseload
  • Provide on-call and crisis coverage as assigned.
  • Perform other related duties and/or projects as assigned
QUALIFICATIONS
  • Must have one of the following requirements:
    • High School Diploma or GED and two years of supervised clinical experience in a mental health setting.
    • Associate's or Bachelor's degree in any field.
    • Licensed Practical Nurse under the Nurse Practice Act (225 ILCS 65)
    • Certificate of Psychiatric Rehabilitation from a DHS-approved program, a high school diploma/GED, and two years' experience of experience in a mental health setting
    • CRSS certification through IAODAPCA
    • Family Partnership Professional certificate from and in good standing with IAODAPCA
    • Occupational Therapy Assistant licensed under the Illinois Occupational Therapy Practice Act (225 ILCS 75) with one-year experience in a mental health setting
    • High school diploma or GED and five years of supervised clinical experience in a mental health setting
  • Valid Illinois driver's license with daily access to a well-maintained vehicle with 1,000,000/ 3,000,000 liability insurance
  • Experience working with people with mental health and/or substance abuse diagnoses preferred
  • Experience in community-based services preferred
  • IM+CANS certification preferred and may be required to obtain IM+CANS certification within 30 days of hire

Social Work Clinician (MSW) needed for a contract opportunity with Yoh’s client located in Palo Alto, CA!

What You’ll Be Doing:
  • Collaborate with interdisciplinary health care teams to treat and plan for the social and medical needs of patients by consulting with other providers and making referrals
  • Devise, develop, and implement treatment plans as necessary
  • Conduct initial assessment of all new patients to evaluate social, emotional and physical needs by applying social work theories and techniques; interview and evaluate patients and family members; identify factors which may interfere with obtaining maximum benefits from treatment
  • Develop and implement patient management plans, including the coordination of referrals to specialty and social services, the recording of progress notes and the provision of continuity of care
  • Manage assigned caseload; prepare and maintain records and make written and oral reports to document case activity concerning patient care, counseling and therapeutic activities in accordance with department, hospital, medical center, and/or health system policies
  • Participate in monitoring and evaluating the quality and appropriateness of patient care
  • Provide supportive counseling, grief counseling, crisis intervention, and other appropriate therapeutic services to assist patients and families in coping with presenting symptoms and problems such as, for example, coping with illness, accepting medical recommendations, and loss of function
  • Provide education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation
  • Schedule: 5x8s (Monday – Friday), dayshift
What You Need to Bring to the Table:
  • Masters degree in Social Work (MSW) from an accredited college or university; OR be within forty-five (45) days of completion of the requirements for graduation with an MSW from an accredited college or university; candidates who meet this qualification and furnish a letter from the Dean of the School Work where they are a student in good standing attesting to this may receive a conditional offer of employment; such new hires must provide proof of possession of an MSW degree from an accredited college or university within forty-five (45) days of their date of hire, as a condition of continued employment
  • Ability to apply theories, principles and practices of medical social work and of social services delivery systems in a hospital or medical center setting
  • Ability to gather data needed to assess needs of patients and families
  • Ability to manage a caseload, recall case details; maintain confidentiality of information; determine the appropriate course of action in emergencies and other stressful situations
  • Ability to provide written case narratives and reports
  • Knowledge of and ability to apply effective assessment, interviewing, and counseling techniques
  • Knowledge of and ability to apply social work theories, principles and techniques
  • Knowledge of hospital and medical center resources to access and provide for patient care needs, and act on behalf of client needs to obtain necessary services
  • Knowledge of needs, problems, attitudes, and behavior of hospital patients and their families
  • Knowledge of computer systems and software used in assigned work area
