As an Advanced Practice Provider (Nurse Practitioner, Acute Care Nurse Practitioner, or Physician Assistant) in our Transplant Program, you will provide high-quality, patient-centered care under the guidance of specialty physicians. You will play a critical role in the diagnostic and therapeutic management of patients by obtaining medical histories, performing physical examinations, ordering and interpreting diagnostic tests, and formulating and executing treatment plans. In addition, you will deliver comprehensive follow-up and health maintenance care in alignment with established clinical protocols.
Responsibilities:
Conduct patient assessments and manage pre- and post-transplant care.
Collaborate with physicians and multidisciplinary teams to develop and implement individualized care plans.
Monitor transplant patients for complications and coordinate care transitions.
Educate patients and families about procedures, medications, and long-term care requirements.
Maintain detailed documentation in accordance with regulatory and institutional standards.
Education & Experience Requirements:
For Physician Assistants :
Graduate of a PA program accredited by the Commission on Accreditation of Allied Health Education Programs.
At least one year of experience in the transplant or relevant acute care specialty preferred.
For Nurse Practitioners (NPs) and Acute Care Nurse Practitioners (ACNPs) :
Master’s or Doctoral degree in Nursing from an accredited program.
Board certification as an NP or ACNP.
One year of experience in acute care or transplant preferred.
Licensure & Certifications:
Current licensure to practice as a PA or NP in the State of Texas.
National certification (e.g., NCCPA, ANCC, or AANP).
Current AHA BLS certification (or higher, as required).
Valid DEA license or eligibility to obtain within 90 days of hire.

We are looking for a full-time, Advanced Practice Provider (NP or PA) in the Foley Cancer Center. Our comprehensive community cancer center manages patients in multiple settings: cancer clinic, inpatient, emergency room, and telehealth. Working collaboratively with physician and nurse partners to provide patient and family-centered care. M-F 8:00am – 4:30pm, no weeknight call, one weekend per month on call coverage.
Under the direction of the Medical Director and/or physician, the Advanced Practice Provider (Nurse Practitioner or Physician Assistant) provides healthcare services to patients by following established standards, procedures and practices in their area of specialty. Obtains detailed history and performs physical or psychological examinations. Performs or orders X-rays, electrocardiograms, laboratory tests and other diagnostic procedures, and interprets results. Administers such therapeutic procedures as injections, immunizations, suturing and wound care. Provides physicians with assistance during surgery or medical procedures. Instructs patients on prescribed therapeutic regimens, home care and health maintenance. Evaluates outcomes of direct patient care and consults with other specialists as required. Maintains required records and writes progress notes on patient charts as to status, treatments and procedures. Provides preventative health care services to long-term patients and arranges consultations and referrals.
Minimum Education
- Masters Degree in Nursing or Graduate of an American Medical Association approved Physician Assistant educational program.
Minimum Work Experience
- li>2 years of Nurse Practitioner or Physician Assistant experience in specialty area being sought (i.e., Pain Management, Orthopaedics, Emergency Medicine, Psychiatrics, Endocrinology or Oncology).
Required Licenses/Certifications
- Licensed with the Vermont Office of Professional Regulation with advanced certification by a national certifying organization recognized by the Vermont Nursing Board’s scope of practice.

**The Opportunity** :
We are hiring an APP in our CVICU at the Plaza. We are looking for an APP that is hardworking and has a passion for critical care. This is a full-time position that works a combination of night shifts and day shifts.
This position works closely in a collaborative relationship with the Neuro Critical Care physicians. A positive attitude and working well with all members of the multi-disciplinary team is crucial. ICU background is preferred, new grads will be considered. The level of autonomy increases with experience. The physicians expect to be highly engaged in patient care. The expectation is the APP will work towards being independently credentialed to place radial arterial lines.
**The Schedule:**
The schedule is 3 shifts per week. Night shift begins at 5:00 pm and ends at 6:00 am with some days extending longer depending on patient load. Expectations are one weekend every three weeks but could require more due to staffing needs. Approximately two Holidays per year are expected. Applicants must be prepared to work undesirable shifts. The unit is 21 beds. Staffing ranges from 1 to 2 APPs on night shifts. Orientation for new graduates is approximately 6 months, and they will be enrolled in the Fellowship program.
