Must be able to work one summer and one winter holiday Must be able to rotate on an on call schedule JOB and UNIT SUMMARY: The Home Health division of Advocate Health Care is the largest home health and hospice services in Illinois and the RN will provide home care services to the homebound population and establish strong relationships with patients and caregivers in order to assist the patient in reaching their maximum potential. Advocate Home Health Services helps patients return home to continue the care that began in the hospital. Our team helps patients and caregivers become independent when managing their condition. The delivery of high level nursing care is critical to allowing the patient to remain at home and to preventing rehospitalization. All medical needs will be coordinated by the RN who is case managing the patient and utilizing the resources of the team, (i.e. social worker, CNA, MD, etc.) Document and connect with the care team thru point of care documentation. Required to make visits to private residences which may not be accessible to people with disabilities. THE ADVOCATE HOME HEALTH NURSE: Assumes primary responsibility for the assessment, planning, implementation, and evaluation of nursing care for assigned patients in the comfort of their own homes. Set goals which demonstrate a holistic approach to patients and families. Assumes leadership responsibilities at the unit level. Demonstrates a mastery of clinical skills. Nurse/Patient Relationship: Develops and maintains a therapeutic relationship throughout the healthcare continuum. Support staff on the unit includes Stroke Coordinator, Respiratory Therapy, physical and occupational and speech therapy, case management, nutritional services, social work, pharmacy, pastoral care, and community nursing service
The unit is managed by the Nurse Manager with a designated charge nurse each shift. The house nursing supervisor provides leadership during off hours and on weekends. Neurologists, Neurosurgery, and hospitalist support is available either in-house or by pager 24/7.
Flexible self-scheduling, with designated third weekend commitment and accommodating three holidays per year.
Performs a variety of direct patient care procedures to assist the unit manager, professional staff and physicians in providing patient care focused on the needs of the patient.
Meets the comfort and hygiene needs of the patient by providing care services such as bathing, oral hygiene procedures, hair care, bed pan/urinal use, skin care and garment, linen and bedding changes. Distributes meals and beverages, takes vital signs, and collects lab specimens.

Individuals working as Patient Care Assistants at Loyola University Medical Center prior to 1997 are exempt from the above guidelines and are grandfathered into the Patient Care Tech roles upon successful completion of an in-house orientation and training program for the Patient Care Tech position.

Job Description
Performs a variety of direct patient care procedures to assist the unit manager, professional staff and physicians in providing patient care focused on the needs of the patient.
GENERAL PATIENT CARE - Meets the comfort and hygiene needs of the patient.
TECHNICAL CARE - Assists with completion of patient testing and treatment procedures, sets up and primes IV tubing, 12-lead ECG, simple tracheostomy suctioning and care, changes dressings, performs cardiopulmonary resuscitation, obtains wound and throat cultures.
EQUIPMENT - Assists physician and professional staff in equipment supply and set up.
TRANSPORT - Assists with ambulating patients with assistive devices such as crutches and with transfer/transport procedures.
COMMUNICATION - Performs required documentation and communicates in a competent and effective manner to meet patient and/or family needs and regulatory requirements. Maintains strict confidentiality of patient, employee and department information to ensure patient rights.
QUALITY IMPROVEMENT - Utilizes time and resources effectively to promote quality and cost outcomes.
