**As a Hospice Patient Care Manager, You Will:**
* Report directly to the Executive Director, Administrator, or Senior Patient Care Manager
* Oversee and direct patient care services to ensure the delivery of high-quality hospice care
* Manage visit scheduling, including reassigned, missed, declined, or rescheduled visits-making decisions based on clinical judgment and priority
* Triage incoming phone referrals and medical concerns in a timely, compassionate, and professional manner
* Lead or facilitate Interdisciplinary Group (IDG) meetings, including presenting patient updates, reviewing charts, coordinating care plans, and documenting minutes
* Serve as a mentor, trainer, and clinical support to associates, supporting development, performance, and compliance
* Ensure all care delivery meets federal, state, and local regulatory standards, as well as company policies and procedures
* Support performance improvement initiatives, quality assessments, and operational efficiencies within your location
**About You**
**Qualifications - What You'll Bring:**
* Current RN license in the state of employment
* Graduate of an accredited nursing program
* Minimum of 3 years of direct patient care experience, including 2 years in a hospice or home-based care setting
* Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC)
* Knowledge and compassion for terminally ill patients and their families
* Ability to lead a diverse care team while adapting to the emotional and clinical needs of patients from various backgrounds
* Strong interpersonal, organizational, and leadership skills
* Competency in clinical documentation and technology systems
**Preferred Qualifications (Not Required):**
* Bachelor of Science in Nursing (BSN)
* Previous leadership, team management, or supervisory experience in hospice or home health
* Familiarity with EMR systems and telehealth platforms

Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).
**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**
**Patient Related**
Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
+ Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.

+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.

+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.

+ Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.

Required: Practitioner must have completed the number of years in a residency program approved by the Accreditation Council for Graduate Medical Education and met all other requirements for eligibility to receive board certification recognized by either the American Board of Medical Specialties or the American Osteopathic Association in the primary medical specialty (or subspecialty) for which the Practitioner is applying for Clinical Privileges. If five (5) or more years have elapsed since the Practitioner completed such residency program, the Practitioner currently must have board-certification recognized by either the American Board of Medical Specialties or the American Osteopathic Association in such specialty (or subspecialty).

Meets requirements of the Moses Cone Health System Medical Staff by-laws and obtains the appropriate level of medical staff privileges.
The Practitioner must have a current, and valid DEA registration number, with no involuntary limitations on prescriptive authority (schedule limitations) resulting from DEA disciplinary actions.
Each Practitioner shall maintain in force professional liability insurance as determined by the Moses Cone Health System Medical Executive Committee and Board of Trustees; currently $1 million each incident and each occurrence, and $3 million aggregate. The Practitioner must provide proof of professional liability insurance coverage, with a current certificate of insurance or a current copy of the policy. The professional liability insurance coverage shall be with a company approved by the North Carolina Insurance Commission and otherwise satisfactory to the Board of Trustees.

We are educators, researchers, and advocates, dedicated to meeting the needs of our patients and their caregivers. UNC Hospitals RN Incentive Program - UNC Health Careers ( plans, implements, and evaluates wound, ostomy and continence care throughout inpatient and outpatient areas. Functions primarily as a clinical consultant and provides direct patient care and education to highly complex patients and provides staff educational programs in areas of expertise.
Responsibilities:
1. Administrative: Updates, revises and maintains ostomy, wound, continence and skin care product formulary in conjunction with Central Distribution and PMC. Evaluates products for their efficacy and cost effectiveness. Stays informed of product innovations from the manufacturers of ostomy, wound, continence and skin care products. Maintain a mechanism for documentation patient caseload. Update and maintain appropriate data collection criteria and tools when appropriate. Maintains and utilizes current documentation standards for ostomy, wound, continence and skin care patients.
2. Clinical Consultant: Serves as consultant to medical staff, physicians and nursing staff for patients with wound, ostomy and continence conditions. Provides assessment and recommendations for wound, ostomy and continence patients. Develops patient plan of care and monitors for effectiveness. Provides patient and family education. Coordinates discharge plans with the provider and case management. Serves as clinical consultant for wound, ostomy, continence and skin care needs for committees, task forces, projects on the unit level, hospital level and healthcare level. Attends professional offerings to further enhance clinical skills and to be current on clinical standards related to wound, ostomy, continence and skin care.
3. Educational Consultant: Provides educational programs for health care providers including nurses, physicians and students in the UNC Health Care system. Provides educational resources to keep staff updated to advances, new policies and procedures related to WOC practices and related quality and patient safety issues. Serves as the clinical expert for orientation material on wound, ostomy and skin conditions for new nursing staff. Serves as a clinical preceptor to nursing and medical staff and students. Serves as a clinical preceptor to WOC Nurse students. Serves as the clinical expert for LMS modules related to WOC Nurse practice. Creates, reviews and revises all procedures and protocols related to WOC nurse practice.
4. Research Participant: Participates in research related to WOC nurse practice including wound, ostomy, skin and continence conditions. Participate in data collection including audits of pressure injuries. Interpretation of data.
