**PURPOSE AND SCOPE:**

Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies procedures and training and in compliance with regulations set forth by the corporation state and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the FMCNA commitment to the Quality Enhancement Program (QEP) and CQI Activities including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMC Quality Enhancement Goals (QEP).
**DUTIES / ACTIVITIES:**
CUSTOMER SERVICE:
+ Responsible for driving the FMS culture through values and customer service standards.

+ Evaluate vascular access for patency and report any unusual findings to nurse supervisor.
+ Obtain necessary pre and post treatment vital signs and weight. Report unusual findings to nurse supervisor.
+ Perform vascular access evaluation pre-treatment and report unusual findings to nurse supervisor.
+ Perform vascular access cannulation and report any difficulties in cannulation or unusual findings to nurse supervisor.
+ Perform administration of Heparin as delegated or as allowed by state law.
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy and report any unusual findings to nurse supervisor.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures
+ Evaluate patient prior to termination of venous access - standing & sitting blood pressure.
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient for prior to discharge and report any unusual findings to nurse supervisor.
+ Perform and record Pre and Post dialysis evaluation weight and vital signs with initial identification.
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled including Narrow Venous Limits (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and if applicable progress notes.
+ Report any significant information and/or change in patient condition directly to the nurse supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the nurse supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures provide appropriate intervention as needed document and report any unusual findings to the nurse supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
MAINTENANCE/TECHNICAL:
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface chair equipment and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
+ Perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare organize and efficiently utilize supplies and equipment to prevent wastage.
CLERICAL & ADMINISTRATIVE:
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
+ Enters all treatment data into the designated clinical application accurately and in a timely manner.
+ Review treatment sheets for completeness ensure nursing signatures are documented and ensure omitted entries are completed or corrected where appropriate.
+ Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
+ Prepare lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
+ Collect label appropriately prepare and store lab samples according required laboratory specifications.
+ Ensure collection of lab specimens by appropriate lab courier.
OTHER:
+ Perform additional duties as assigned.
**ADDENDUM FOR PATIENT CARE TECHNICIANS WORKING IN THE HOME PROGRAMS (NOT HOME ASSIST):**

+ May assist HT RN on Home Visit as directed

As a Patient Care Technician (PCT) at US Renal Care, you will be an integral part of a cross-functional team providing care and delegated activities of treatment to patients. The PCT will work under the supervision of a Registered Nurse (RN) or Charge Nurse (CN) for patients with renal failure.
What you'll be doing
Patient care. You will work directly with patients to provide safe, comfortable, and high-quality dialysis treatment. Maryland
- Must possess current Certified Nursing Assistants -- Dialysis Technicians (CNA-DT) certificate from Maryland Board of Nursing OR
- Hired at USRC location recognized by state of Maryland as an approved dialysis technician training program and obtain Maryland CNA-DT certificate within three (3) months from date of hire OR
- Out-of-state applicant must have active BONENT certification and provide proof of initial application for CNA-DT certification.
New Mexico
- Must have New Mexico dialysis technician certificate at time of hire OR
- Hired at USRC location recognized by state of New Mexico as an approved dialysis technician training program and obtain New Mexico state certification within six (6) months of successful completion of training program OR
- New Mexico certification of hemodialysis technician is required for out of state applicant with active state or national hemodialysis certification; must obtain New Mexico state certification prior to working as a Certified Hemodialysis Technician.
- After January 1 st , 2024, all initial applications for the certified hemodialysis technician will require a national hemodialysis technician certificate.
- The applicant must submit proof of a valid national hemodialysis technician certificate from a recognized organization.
- Hemodialysis technician students must have a current basic life support credential prior to and while the student has contact with patients
OHIO
- Must have Ohio dialysis technician intern certificate at time of hire OR
- Hired at USRC location recognized by state of Ohio as an approved dialysis technician training program and submit application for Ohio dialysis technician intern certificate to board no later than four (4) weeks prior to completion of the approved training program.
- Must obtain Ohio Board of Nursing Certificate (OCDT) not less than twelve (12) months and not later than eighteen (18) months of enrolling in the USRC OCDT program.
OREGON
- Must possess current Certified Hemodialysis Technician (CHDT) certificate from Oregon Public Health Division at time of hire OR
- Must obtain Oregon dialysis technician provisional certificate within three (3) weeks of successful completion of training program and obtain Certified Hemodialysis Technician (CHDT) certificate from Oregon Public Health Division within eighteen (18) months. An Oregon Provisional Certification is valid for six months and can be renewed for one additional six month period.

Job Summary:
The Patient Care Administrative Nurse Supervisor is an experienced Registered Nurse (RN) and is responsible for supervising and monitoring the Nursing department(s) on an assigned shift in accordance with nursing standards and practices as well as current Federal, State and local standards, guidelines and regulations.
This position ensures the appropriate delivery of direct and indirect patient care via the utilization of the Nursing Process and creates a positive environment for the professional practice of nursing (RN).
In addition, this position ensures activities foster continuous quality improvement to enhance patient care outcomes. This position also ensures patient flow throughout the organization.
This position provides leadership for multiple hospital departments in the absence of the RN unit manager, director, or administration.
QualificationsQualifications
- Bachelor’s degree in Nursing required
- Existing incumbents as of 3/1/24 must obtain BSN within 3 years of this date
- 3 years of acute care charge or supervisory experience required
- Progressive leadership experience
- Active Registered Nurse license from the State of Connecticut required
- Certified in Basic Life Support (BLS) provider credentialed form the American Heart Association (AHA)

Responsible for the operational and clinical management of a designated patient care unit(s) on a 24 hour basis. Oversees a span of control and financial responsibility of > 72 FTE's supervised + > 2.5 million in total expenses OR > 40 FTE's supervised + > 5 million in total expenses.
**Responsibilities**
+ Manages individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
+ Develops and adheres to departmental staffing, revenue and/or expense budgets. Responds to changes in the business which may affect the ability to achieve the budget goals.

St. Louis, MO ( Still University's Missouri School of Dentistry & Oral Health (MOSDOH) is seeking an exempt, full-time Patient Care Manager on the St. Louis, Missouri Campus. This position reports to the Director - Dentistry in Community. The Patient Care Program Manger will oversee and coordinate the complex activities required to fully execute a successful patient care program study. S/he will collaborate with the outreach hygienist to coordinate outreach events. This position will be responsible to screen potential participants against program criteria, enroll eligible patients, and serve as the patient navigator to ensure program compliance by participants. S/he will manage all aspects of the funded project for programmatic and fiscal compliance, and will collaborate with the PIs to achieve the project's stated aims, objectives, methodology, data collection/analysis, evaluation, and reporting.
**Duties and Responsibilities:**
+ Monitor patient assignments, appointments, transportation needs, and treatment plans.
+ Meet with patient care coordinators, students, and faculty to assess and troubleshoot patient care issues related to the grant program.
+ Navigate the Affinia Healthcare and NexGen systems to produce accurate reports.
+ Update PIs on status of the project and maintain project timeline.
+ Work with outreach hygienist to prepare for outreach programs and implementation of dental care for participants.
+ Assist with training stakeholders on the purpose of the education and patient care program.
+ Review paperwork/consents.
Requirements
**Education & Experience**
+ Four year college degree in dental hygiene, business, health care, social work, or health education. MPH or MBA preferred.
+ Registered Dental Hygienist or Dental Assistant. If not, possess some understanding of treatments plans and/or dental terminology.
+ Two to three years experience.
+ Experience with spreadsheets, EDR/EMR, treatment plans, and dental terminology.
+ Ability to run and read reports from NexGen.
+ Ability to track information and run reports, e.g. Excel or Google Sheets.
+ Some data entry required.
**Special Characteristics or Personality Traits**
+ Must be able to work with people with a wide assortment of personalities and abilities.
+ Will communicate with program participants with limited health literacy and with students and dental/medical professionals.
+ Dental knowledge is very helpful.
