Required Experience and Competencies

• Each scribe works 2-3 shifts per week (including weekends and a holiday).
• Ability to stand, walk and follow a physician for extended periods of time.

• 1 year commitment at this site, continuous one year, not a summer position.

The Opportunity

• Supports and represents educational program(s) at assigned site.
• Provides general administrative support to include, but not limited to, meeting agendas and minutes, managing calendars, drafting correspondence, handles phone calls/messages, filing, copying, and faxing.
• Purchases office equipment and supplies as needed.
• Assists with scheduling and supporting educational program events including lectures, recruitment interviews, orientation, retreats, as well as various meetings and other program-related events.

This includes ensuring all providers are completing their Vituity and hospital Medical Staff Office (MSO) initial credentialing and re-credentialing as applicable and in a timely manner.

Required Experience and Competencies

• 1-2 years of experience working with Graduate Medical Education Programs required.

We are excited to share the base salary range for this position is $2.05 - 27.02, exclusive of fringe benefits or potential bonuses. This position is also eligible to participate in our annual corporate Success Sharing bonus program, which is based on the company's annual performance. The Patient Care Technician (PCT) serves in a combination role and is competent to perform as a Patient Care Assistant (PCA), Telemetry Technician (TT) and a Unit Secretary (US). • Telephonic and field-based outreach to engage clients in our care management program.
• Establishes close relationships with and serves as a point of contact for clients.
• Deliver weekly or monthly health education and promote self-management to clients.
• Communicate with Care Team members (Care Coordinators, Community Health Worker, Primary Care Physicians and other health care providers) to facilitate client care.
• Observe, report, and assess client self-administration of medication.
• Identify resources for clients to overcome barriers to care, such as transportation, housing, and childcare arrangements.
• Remain aware of current services offered by service providers, such as mental health, housing, and employment assistance.
• Maintain strict confidentiality in accordance with agency policies.
• May meet with clients after primary care physician appointments to review and update care plan with the Care Coordinator

• Meet with Care Team (including, but not limited to, Care Coordinator, Community Health and primary care provider) to discuss client care issues and needs and facilitate client health care.
• Maintain documentation of all client encounters and complete reporting requirements according to organization standards
• Track client information, schedules, files, and forms in a confidential manner.
• Track client attendance at medical appointments and patient navigation sessions and initiate outreach and missed appointment procedures, as necessary.
• Attend and represent the organization at professional conferences, in-service trainings, and meetings at the request of or with the approval of supervisor.
• Interest in working with underserved, homeless populations.
• Physical demands associated with office work.

• Some evening work may be required.

• Minimum high school degree, some college education preferred.
• Strong understanding of cultural competency with the target population
• Bilingual (English/Spanish) preferred.
• Computer literacy desirable
