**Job Summary and Qualifications**
The Virtual Patient Logistics Center (VPLC) Assessment Specialist (AS) supports hospital functions within the emergency department and inpatient medical units. Per Physician or Nurse Practitioner/Physician Assistant order, Assessment Specialists provide level of care assessments and recommendations (LOC) for patients presenting in acute mental health crisis and with other behavioral/mental health concerns. The Assessment Specialist will confer with the ordering physician, clinical hospital staff, and patient collaterals for the purpose of appropriate evaluation of risk and safety planning recommendations. This position functions within a remote centralized team model and will offer support to all in-scope division partners across the HCA enterprise.
As the VPLC Assessment Specialist you will:
+ Respond to requests for assessment in facilities and locations serviced by an in-scope Patient Logistics Center (PLC).
+ In partnership with the Patient Logistics Specialists (PLS), new requests for level of care assessments will be passed to the VPLC AS via the case queue.
+ Follow internal processes and work independently to quickly initiate ownership of a case from the queue.
+ Adhere to confidentiality guidelines appropriately and ensure EMTALA and HIPPA compliance at all times; compliance concerns will be reported to leadership immediately.
+ Coordinate communication necessary for a thorough assessment, including but not limited to collateral and key stakeholders responsible for the patient/knowledgeable of the sequences leading to the assessment, hospital staff and leadership, and local PLC staff.
+ Ensures efficiency in throughput for the emergency department, requesting additional resources when needed based on volume, and works with the local PLC staff to move patients to appropriate dispositions quickly.
+ Respond to assessment as soon as possible and no later than 60 minutes from the initial request.
+ Confer with ordering physician regarding the appropriateness of a patient's legal status based on patient's presentation and engagement during assessments.
+ Must be knowledgeable of all state mental health/civil commitment laws for each state supported by VPLC.
+ Document all Initial Assessment, Re-Assessment, or Chart Review in 1 of the 3 EHRs utilized by the division supported by the LOC Team.
+ Detailed Risk Assessment will be completed within the 1 of 3 EHRs utilized by the facilities.
+ Safety Plan recommendations are likewise documented in the patient's EHR chart for the use of the ordering physician, if they so choose, upon patient disposition.
+ Downtime paper work will be utilized if EHR is not available and sent to clinical stakeholder for utilization and record keeping at facility.
+ Copy of assessment and all clinical information to be saved to TCIQ for record retention.
EXPECTATIONS INCLUDE BUT ARE NOT LIMITED TO
+ Complete assessments in a timely manner and provide high quality clinical care:
+ Initial Assessments - 90 minutes or less on average
+ Chart review 30 minutes or less on average
+ Clinical interview 30 minutes or less on average
+ Documentation 30 minutes or less on average
+ Ability to engage with patients remotely via tele-video platform and implement skills necessary for proper clinical evaluation of patient
+ Works collaboratively with hospital departments to assist managing behavioral health assessment needs.
+ Coordinates and participates with appropriate interested parties and physicians through the patient admission, treatment and discharge process in the acute hospital setting.
+ Tracks monthly individual performance via key performance indicators and quality metrics shared by leadership
+ Engages in clinical learning opportunities offered internally as well as meets the required continued education expectations of particular licensing board
KNOWLEDGE, SKILLS AND ABILITIES
+ Possess a thorough knowledge of current behavioral health concepts, principles and methods of treatment.
+ Able to navigate technology solutions with minimal guidance/support
+ Comfortable with Telehealth solutions and providing care through virtual platform
+ Possesses good multitasking skills to navigate 5+ systems during a given shift to ensure quality care is provided to patients
EDUCATION
+ Master's Degree in the field of Mental Healthcare
EXPERIENCE
+ Prefer 2 years' experience in a behavioral/mental healthcare setting performing clinical duties, including assessment
+ Prior management/supervisory experience in a healthcare related field preferred
CERTIFICATE/LICENSE
+ Must hold and maintain licensure as a mental health professional (i.e. LCSW, LPC, LMHC, LP, etc) within state of residence

The Virtual Patient Logistics Center (VPLC) Assessment Specialist (AS) supports hospital functions within the emergency department and inpatient medical units. Per Physician or Nurse Practitioner/Physician Assistant order, Assessment Specialists provide level of care assessments and recommendations (LOC) for patients presenting in acute mental health crisis and with other behavioral/mental health concerns. The Assessment Specialist will confer with the ordering physician, clinical hospital staff, and patient collaterals for the purpose of appropriate evaluation of risk and safety planning recommendations. This position functions within a remote centralized team model and will offer support to all in-scope division partners across the HCA enterprise.
As the VPLC Assessment Specialist you will:
+ Respond to requests for assessment in facilities and locations serviced by an in-scope Patient Logistics Center (PLC).
+ In partnership with the Patient Logistics Specialists (PLS), new requests for level of care assessments will be passed to the VPLC AS via the case queue.
+ Follow internal processes and work independently to quickly initiate ownership of a case from the queue.
+ Adhere to confidentiality guidelines appropriately and ensure EMTALA and HIPPA compliance at all times; compliance concerns will be reported to leadership immediately.
+ Coordinate communication necessary for a thorough assessment, including but not limited to collateral and key stakeholders responsible for the patient/knowledgeable of the sequences leading to the assessment, hospital staff and leadership, and local PLC staff.
+ Ensures efficiency in throughput for the emergency department, requesting additional resources when needed based on volume, and works with the local PLC staff to move patients to appropriate dispositions quickly.
+ Respond to assessment as soon as possible and no later than 60 minutes from the initial request.
+ Confer with ordering physician regarding the appropriateness of a patient's legal status based on patient's presentation and engagement during assessments.
+ Must be knowledgeable of all state mental health/civil commitment laws for each state supported by VPLC.
+ Document all Initial Assessment, Re-Assessment, or Chart Review in 1 of the 3 EHRs utilized by the division supported by the LOC Team.
+ Detailed Risk Assessment will be completed within the 1 of 3 EHRs utilized by the facilities.
+ Safety Plan recommendations are likewise documented in the patient's EHR chart for the use of the ordering physician, if they so choose, upon patient disposition.
+ Downtime paper work will be utilized if EHR is not available and sent to clinical stakeholder for utilization and record keeping at facility.
+ Copy of assessment and all clinical information to be saved to TCIQ for record retention.
EXPECTATIONS INCLUDE BUT ARE NOT LIMITED TO
+ Complete assessments in a timely manner and provide high quality clinical care:
+ Initial Assessments - 90 minutes or less on average
+ Chart review 30 minutes or less on average
+ Clinical interview 30 minutes or less on average
+ Documentation 30 minutes or less on average
+ Ability to engage with patients remotely via tele-video platform and implement skills necessary for proper clinical evaluation of patient
+ Works collaboratively with hospital departments to assist managing behavioral health assessment needs.
+ Coordinates and participates with appropriate interested parties and physicians through the patient admission, treatment and discharge process in the acute hospital setting.
+ Tracks monthly individual performance via key performance indicators and quality metrics shared by leadership
+ Engages in clinical learning opportunities offered internally as well as meets the required continued education expectations of particular licensing board

Role Description The Admissions Specialist reports to the Admissions Director and serves on the Skyway support team. This person is a trusted leader who collaborates with senior management to embody and promote the mission of Skyway Behavioral Health. The Admissions Specialist is both a clinical and customer service role and it responsible for the performance of intake calls and patient assessments through collaborative efforts with attending physicians, case managers, and clinical treatment team members. Essential Functions/Core Responsibilities To ensure success as Admissions Specialist, candidate should have excellent knowledge of clinical assessment and case management principles and the ability to advocate for patients needsResponsible for the initial assessment process (which includes prior treatment history, previous medications, health screen, risk factors, and family dynamics). Provides treatment referrals which are consistent with the customer's medical and psychiatric status/symptomsCoordinates the admission process including appropriateness of the admission, program placement, completion of all required admission paperwork, pre-certifications and benefit verifications and coordination with the treatment team. Keep patients and clinicians informed on care and treatment options.Promote quality patient services and overall patient satisfaction. Responsible for resolution of any patient or customer concerns during the admissions process. Respond to telephone inquiries from persons seeking mental health care or information about mental health-related services.Ensures all policies governing pre-certification and authorization are followed to achieve clinical goals and objectives that minimize risk and align with Joint Commission standards.Establishes and maintains effective communications and working relationships with physicians, providers, and all colleagues. Minimum Requirements Two (2)years or more of direct intake, assessment, admission, and crisis intervention experience with a psychiatric mental health population required. Must be licensed in the state of Illinois as LPC, LSW, LCPC, LCSW (PsyD graduates may also apply) Master's degree in a Psychology related field. Knowledge of screening tools to assist in determining if the proposed services are clinically indicated and provided at the appropriate level or determine whether further evaluation is required.Exceptional organization skills, reliable and timely. The task involves completing 420 forms. The client will provide the websites, and for each form, participants will need to take three screenshots:
- one of the checkout
- one for changing the password
- and another specific task.
This is a straightforward task that can be done in the afternoons, and training will be provided. We are simply looking for reliable and responsible individuals.
