**Position Summary & Highlights:**
+ The Athletic Training Outreach Coordinator will support in the development and growth of the Saint Alphonsus Sports Medicine program while maintaining efficient workflow operations in a designated work environment.
+ Assignments may involve work of a confidential nature and require a basic knowledge of the policies, practices, and procedures relative to the Sports Medicine Program.
+ This position will be a key part in the development of outreach athletic training workflow processes, coordination of patient care in collaboration with the providers and administrative team.
+ This role will also facilitate independent marketing, facilitate community services specific to the Saint Alphonsus Sports Medicine Program (e.g., concussion program outreach, community events, screenings, and education).
+ Saint Alphonsus Rehabilitation Services (STARS) ( provides medical coverage for a variety of youth and adult organized sports, including high school athletics, club sports, rodeos, and more. Our athletic trainers and sports medicine physicians are available to be on-site to help prepare athletes for events as well as assess and treat injuries on the field.
**What You Will Do** **:**
+ You Will know, understand, incorporate, and demonstrate the Organizational Mission, Core Values, and Vision in behaviors, practices, policies and decisions.
+ The Athletic Training Outreach Coordinator will effectively and efficiently oversee the coordination of the Saint Alphonsus Sports Medicine program in designated work environments which may include:
+ Physician clinic aspect of the Sport Medicine Program including building relationships with sports medicine/orthopedic providers and staff;
+ Coordinate with leads regarding and staff schedules, workflows, and processes/procedures;
+ Coordinate onboarding and training of new colleagues, as well as maintenance of clinical competencies for sports medicine staff with Sports Medicine Supervisors.
+ Facilitate the management and processing of referrals into the clinics from independent marketing pathways such as schools, youth organizations, referring MDs, PTs, and community or outreach ATCs.
+ Facilitate community services such as Concussion program outreach, organizing community events, screenings, and education.
+ Work closely with Sports Medicine Supervisor(s) and other Saint Alphonsus leadership on collaborative activities.
+ Remain flexible to oversee additional clinical programmatic needs as they develop.
+ Able to provide clinical services consistent with the Certified Athletic Trainer as required:
+ Develop and maintain efficient clinical work-flow processes in a unique medical practice setting.
+ Independent decision making with minimal oversight in a unique clinical setting.
+ Coordinate patient care and facilitation of referrals for ongoing care, tracking referrals, chart prep, record retrieval and scheduling.
+ Develop and maintain a functional system for patient care related correspondence with physicians, referral sources, potential or current patients.

+ This position will work out of our Boise office (next to the Saint Alphonsus Regional Medical Center) and will support ATCs that work across the Treasure Valley from Boise, ID to Ontario, OR.

Requirements** **:**
+ Bachelor's degree required, master's degree or doctorate degree preferred.
+ Licensed as an Athletic Trainer, as defined by their primary work state (Idaho or Oregon), issued prior to hire date. Those colleagues hired before January 1, 2020 must provide licensure or proof of application in process in their secondary state by August 28, 2020. Individuals hired in this role after January 1, 2020, must provide licensure or proof of application in their secondary state by August 28, 2020 or within six months of hire, whichever date is later.
+ Athletic Training BOC Certification required.
+ Three (3) years of relevant and recent athletic training experience preferred.
+ Basic Life Support for Healthcare providers certification from either AHA, ARC or the Military Training Network is required at hire in all units, unless currently certified in ACLS and/or PALS or as defined in the SAHS Certification Crosswalk.

JOB PURPOSE:
Perform entry-level general and routine cooking and kitchen tasks under direct supervision, ensuring the smooth operation of the kitchen.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Preparation and Setup:
- Gather mis-en-place for recipes as directed.
- Assist with culinary prep work for supervisors and chefs.
- Measure or weigh ingredients in recipes, scaling portions as required.
- Set up the service line before meal service.
- Cooking and Serving:

+ Assists Sports Performance Leadership with staff education as it relates to sport.

The hourly rate for the Physical Therapy role at Center For Elders Independence is $ 47.76 to $71.62 per hr. Hourly rate based on the market for the Physical Therapy position, as well as experience, skills, abilities and work history

GENERAL DUTIES AND RESPONSIBILITIES:
Collaborates with the primary care provider team and the interdisciplinary team in evaluating agency participants by applying muscle, nerve, joint and functional ability tests.In conjunction with the interdisciplinary team, participates in the development and implementation of the participant plan of care to maximize a participant’s ability to be independent, balanced against their ability to be safe.
Instructs participants and caregivers in the care and use of mobility-related devices such as wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
Works with vendors to obtain rehabilitative equipment and supplies for participants.
Instructs the interdisciplinary team and family on the participant’s physical therapy program and aspects of physical therapy in which they may work with the participant.
As directed by the plan of care treats participants to relieve pain, develop or restore function and maintain maximum performance, using physical means, such as exercise, soft tissue and joint mobilization, functional training, gait training, neuromuscular re-education, heat, sound, water, light, or electricity
Conducts initial assessments of all participants.
Completes reassessments on participants per CEI policy.
Observes and records, and reports regularly to the interdisciplinary team, on the participant’s response to physical therapy and any changes in the participant’s condition.
Instructs, supervises and evaluates therapy program aides in the implementation of the participant’s individualized rehab plan of care.
Provides in-service training to other members of the health team.
Attends the interdisciplinary team conferences and actively participates in the development of the plan of care.
Evaluates participants’ home environments and makes appropriate recommendations as needed.
Attends continuing education and actively participates in meetings and trainings as required. Maintains required certifications.
Participates in coordination for 24-hour care delivery to participants.
Delegates to and monitors the physical therapy care provided by Physical Therapy Assistants as appropriate and in accordance with California regulations.
Documents thoroughly, accurately, and timely, and in accordance with Physical Therapy practice standards and CEI policy.
Maintains department in a clean and orderly manner. Ensures supplies are ordered and ready. Keeps equipment in good working condition.
Maintains the confidentiality of all company procedures, results, and information about participants, clients or families regardless of setting.
Handles potentially infectious specimens with appropriate biohazard precautions, and practices Standard precautions.
Maintains a safe working environment. Follows CEI’s Safety Policies and Procedures.
Maintains a courteous, helpful and professional attitude on the job. Displays a willingness and ability to be responsive to all customer groups.
Performs other duties as assigned in a positive and helpful manner.
QUALIFICATIONS:
Possession of a minimum of a Bachelor’s degree and is a graduate of a program of Physical Therapy approved by the Commission on Accreditation in Physical Therapy Education.
Currently licensed as a Physical Therapist by the Physical Therapy Board of California.
Preferred, one (1) year of experience working as a Licensed Physical Therapist with the frail elderly population.
Current CPR and First Aid certification on file.
A valid California driver’s license, current vehicle insurance, and access to a reliable vehicle is required.
Ability to travel to multiple work sites and participants’ residences, required.

Unlike other healthcare plans, CEI is not a "fee-for-service" plan. It is a “capitation” healthcare plan. CEI is paid a set amount for each person enrolled in our program, whether or not that individual seeks care. We are a growing company that offers stability and continues to thrive.
