Position Summary:

The Patient Care Technician (PCT) performs direct patient care tasks under the direction of the Registered Nurse and performs various tasks as competency documentation designates. Work occasionally requires internal contacts outside the work group to exchange relevant information. Communication with external contacts involves providing or giving factual information

Education/Experience:
  • High School Diploma or High School Equivalency Diploma.
  • One year of clinical experience is required or successful completion of the intern program
QUALIFICATIONS:
  • Certified Nursing Assistant (CNA) certification, Medical Assistant (MA) certification, or one of the

    following:
    • A nursing student or LPN who has completed fundamental nursing skills coursework
    • An Emergency Medical Technician (EMT) certification

    o BLS certification provided by AHA


Salary Range: Minimum $17.37/hour - Maximum $6.91/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a 0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.

**Fundamental Components & Physical Requirements**
+ Acts as a liaison with member/client /family, employer, provider(s), insurance companies, and healthcare personnel as appropriate.
+ Implements and coordinates all case management activities relating to catastrophic cases and chronically ill members/clients across the continuum of care.
+ Interacts with members/clients telephonically and in person. Required to meet with members/clients, worksites, acute/post-acute inpatient facilities to provide ongoing case management services.
+ Assesses and analyzes injured, acute, or chronically ill members/clients medical and/or vocational status; develops a plan of care to facilitate the member/client's appropriate condition management to optimize wellness and medical outcomes, aid timely return to work or optimal functioning, and determination of eligibility for benefits as appropriate.
+ Prepares all required documentation of case work activities as appropriate.
+ Interacts and consults with internal multidisciplinary team as indicated to help member/client maximize best health outcomes.
+ Facilitates appropriate condition management, optimize overall wellness and medical outcomes, appropriate and timely return to baseline, and optimal function or return to work.
+ Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes, as well as opportunities to enhance a member's/client's overall wellness through integration.
+ Monitors member/client progress toward desired outcomes through assessment and evaluation.
**Workspace Expectations**
+ This is a field-based role with 25-50% travel required; candidates must have a dedicated workspace at home free of interruptions.
+ Dependents must have separate care arrangements during work hours, as continuous care responsibilities during shift times are not permitted.
**Required Qualifications**
+ Minimum 3+ years clinical practical experience preference: (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members.
+ Minimum 1 year in CM, discharge planning, hospice, and/or home health care coordination experience or experience working at Baptist Medical Center.
+ Registered Nurse with active state license in good standing within the region where job duties are performed is required.
+ Ability to travel within a designated geographic area for in-person case management activities as directed by Leadership.
**Preferred Qualifications**
+ Excellent analytical and problem-solving skills.
+ Effective communications, organizational, and interpersonal skills.
+ Ability to work independently (may require working from home).
+ Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications.
+ Efficient and Effective computer skills including navigating multiple systems and keyboarding.
+ Willing and able to obtain multi state RN licenses if needed, company will provide.
+ Bachelor's degree preferred.
+ Certified Case Manager is preferred.
+ Additional national professional certification (CRC, CDMS, CRRN, COHN, or CCM) is preferred, but not required.
**Education**
+ Associates degree with equivalent experience, applicant would be required to obtain a Bachelor's degree within 3+ years as part of role development, state licensing laws may apply.

The typical pay range for this role is:
$54,095.00 - $116,760.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
