**Job Duties & Responsibilities:**
+ Provides direct outreach to Tribes and youth surrounding mental health/behavioral health issues and substance use treatment.
+ Consults and collaborates with mental health providers on the assessment and treatment of psychological disorders, substance use disorders, and psycho-social problems.
+ Participates in the intake process by interviewing clients and/or their family members to obtain family, social, employment, medical, and mental health treatment history to determine appropriate level of behavioral health care.
+ Provides crisis intervention and behavioral health interventions (including behavioral health case management).
+ Coordinates continuity of care and other treatment related services with caseworkers, probation officers, and community and healthcare workers.
+ Coordinates mental health support services to clients and families by linking them to other community resources to maximize client self-sufficiency and independence significant to client's continuum of care.
+ Participates in the development of individual and group treatment plans designed to address mental, emotional, and family disorders and dysfunction.
+ Participates in treatment team, staff, and professional meetings to discuss client progress.
+ Assists with alcohol & sexual abuse programs, as needed to enhance services for patients.
+ Provides specialized services to the Behavioral Health Department, such as special program coordination and management.
+ Assists in developing community resources for preventive mental health services. Maintains cooperative professional relationships with local community agencies significant to the continuum of care for clients. Conducts outreach and community-based service provision.
+ Prepares and regularly maintains progress notes, closing, and/or transfer summaries on clients and related duties as required which will be directly signed off by Behavioral Health Director.
+ Engages in individual and/or group supervision while in pursuit of licensure.
+ Must be able to travel.
+ Performs other duties as assigned.

About the Role
We are seeking a Skill Builder to join our Foster Care Program. This role is required under NYS Department of Health 29-I regulations and plays a vital part in helping children and youth in foster care develop the life skills they need to thrive. The Skill Builder works one-on-one with children to build social, emotional, and daily living skills that promote independence, resilience, and success in home, school, and community settings.
The Qualified Candidate will have:
- Master’s degree in social work, mental health counseling, marriage and family therapy or related degree. Clinical licensure is strongly preferred.
- Valid New York State driver’s license and insurance and reliable transportation for frequent travel throughout Western New York, primarily Erie County
- Availability to work a flexible schedule that includes regular evening hours for home visits and community events and participation in the program’s telephonic on call rotation
- Accessibility for consultation outside of regular business hours during emergency situations involving children on caseload
- Strong computer/technology skills, attention to detail, and organization
- A calm, compassionate demeanor in person and on virtual platforms and telephone
- Commitment to family-centered, trauma-informed care

Shift Information: Requires the ability to accommodate a flexible schedule that includes both traditional business hours and regular evenings to match the availability of foster parents and children on caseload. Although this occurs infrequently (less than 5 times per year on average) the Skill Builder must be accessible for consultation outside of regular business hours during emergency situations involving children in care.
As a Skill Builder, you can expect to spend approximately 75% of your time out of the office performing Skill Building for children in their home, day care or school settings and transporting children to and from sessions and approximately 25% of your time in the office writing progress notes, assessments, treatment plans and related tasks.
A day on the job as the Licensed Behavioral Health Practitioner- Skill Builder-
- Complete assessments and develop treatment plans for children in foster care using available historical information and current input from the parents/caregivers, foster parents, child (as appropriate), daycare, school, medical providers
- Provide individualized skill-building sessions for children and youth in foster care, following each child’s treatment plan.
- Teach and reinforce skills such as communication, problem-solving, emotional regulation, conflict resolution, and self-care.
- Support children in developing healthy peer and adult relationships.
- Integrate skill building activities with other available community or school resources for the benefit of the child and their foster parents to achieve a stable placement while also focusing on reunification
- Utilize interventions drawn from evidence-based psychotherapeutic methodology
- Structure interventions to decrease problems and increase developmentally appropriate pro-social behavior
- Document progress and maintain compliance with program, agency, and regulatory standards.
- Collaborate with case planners, foster parents, and treatment teams to reinforce skill development across environments.

**General Summary**
Licensed Professional Counselor, Licensed Clinical Social Worker or Licensed Marriage and Family Therapist provides individual, group and/or family therapy. Provides preadmission screening to determine appropriateness for admission. Interviews patients and/or families to obtain psycho-social histories. Collaborates with the treatment team to develop a treatment plan and coordinates discharge planning.
**Duties and Responsibilities**
**Essential Functions:**
+ Conducts individual or group therapy sessions. Assesses patients' progress and modifies or extends treatment program accordingly.
+ Provides therapeutic intervention with patient and family including individual, marital, parental, family and group therapies.
+ Interviews patients and family members to obtain information about home environment, family relationships, health history, and patient's personality traits.
+ Documents current, accurate observations of care to the patient in appropriate records.
+ Coordinates care of assigned patients.
+ Assists in the implementation of education and training experience for interns and other designated students.
+ Prepares and maintains written documentation as required by the profession and the department.
**Common Expectations:**
+ Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
+ Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
+ Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
+ Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
+ Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
**Minimum Education:**
+ Master's degree required in counseling, psychology, social work or human service-related field required
**Work Experience:**
+ More than 1-year related experience required

Situated on more than 200 acres, WellSpan Philhaven's main campus in Mt. Gretna, PA, is a tranquil place with walking trails nestled within a picturesque forest and adjacent to an operating farm.
