Essential Functions & Responsibilities
- The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities he or she is responsible for;
- Medical directorship functions include attending and participating in monthly quality assurance and process improvement meetings; participating in quality improvement initiatives; providing guidance to facility staff; reviewing and revising (if necessary) facilitys clinical guidelines; insuring compliance with state and federal regulations, training facility staff; and supervising facility clinical staff.
- In addition to the medical director role, the physician also serves as the attending physician for the residents residing at each facility;
- The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;
- Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;
- Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes
- Provider documentation is completed in Theorias proprietary electronic health record (EHR) system, ChartEasy
- PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems
- Participating in Theorias Chronic Care Management, Transitional Care, RPM and other medical quality initiatives and programs
- Physician must be able to supervise collaborating nurse practitioners and physicians assistants
- Physician must be able to utilize electronic/online medical tools such as EHR systems and state/county medical systems utilized for Death Certificates and administrative functions.

Shift Structure

Compensation and Benefits

+ Supports a production-oriented environment by maintaining electrical equipment through established troubleshooting techniques and methodologies, with expertise in low and medium voltage equipment preferred.
Electrician
+ Supports general electrical maintenance tasks, including wiring repairs, lighting, and circuit breaker upkeep, to ensure continued operational efficiency.
+ Assist s with interpreting and utilizing electrical one-line diagrams, schematics, and operates electrical testing equipment to troubleshoot and resolve issues.
Machinist/Millwright
+ Assists Craftsmen with the repair and maintenance of refinery machinery, including pumps, compressors, turbines, valves, gearboxes, and other critical refinery equipment, both in the shop and in the field.

Garage
+ Assists with routine maintenance tasks, including oil changes, tire rotations, and vehicle inspections, and support the servicing of heavy equipment.
+ Helps maintain a clean and organized garage environment, ensuring tools and equipment are properly stored and the work area is safe and free of debris.
EDUCATION AND EXPERIENCE
Minimum (Required) Qualifications:

- Ability to repetitively push and pull objects weighing up to 50 pounds without assistance while kneeling; squatting; reaching above the head; reaching the waist; reaching below the knees; and bending at the knees.
- Ability to repetitively push and pull manual transport equipment (for example, pallet jack, hand-truck) containing product loads a minimum of 100 yards without assistance.
- Ability to repetitively grasp and manipulate objects of varying size and weight requiring fine motor skills and hand-eye coordination.
- Ability to exert oneself physically over sustained periods to complete job activities.
- Ability to read the information in small, medium, and large print.

Qualifications
- Must be 18 years of age.
- Must be eligible to work in the United States.
- Must have a valid driver's license.
- Must have current vehicle liability insurance.
- Must have a driving record with no major moving violations in the last three (3) years.
- Must provide and maintain a personal vehicle for use during employee working hours.
- Preferred Qualifications:
- 1-year experience working in replenishment or as Merchandiser.
- 1-year experience working in grocery, retail, consumer goods, warehousing, or related field.
- 1-year experience working in physically demanding fields such as construction, food service, landscaping, manufacturing, military, nursing, or related experience.
- 1-year experience working with manual or powered pallet jacks.
- Straddle stacker certification.
- Powered pallet jack certification.
- 1-year experience working under little or no supervision.
- 1-year of college coursework in business, marketing, communication, or related area.
