As a Procedural Registered Nurse, work as a licensed professional using the nursing practice model to oversee patient care. Responsibilities include assessing patients, identifying nursing diagnoses, and developing personalized care plans. Regularly assess patient responses. Contributions help patients navigate healthcare safely by understanding their needs and the system. This prevents complications and delays. Delegate tasks based on Texas nursing laws, patient conditions, and staff skills.

The Patient Navigator will serve as a patient liaison to help patients and family members "navigate" the complex healthcare systems. The person in this position works with a multidisciplinary team providing improved comprehensive healthcare services to patients. This position requires advanced expertise to identify and implement improvement processes. Internal and external contacts include nursing staff and management, physicians, patients, families, community agencies, and professional peers.
QUALIFICATIONS
Required : Medical Assistant Certification from one of the following (AAMA, AMT, NHA, NCCT); Ability to work well as part of the interdisciplinary team in the treatment and care of sick and/or terminally ill patients and their families.
Preferred : 2 years' experience working with patients and their families. Associate degree or higher in a healthcare related field.
Lic/Reg/Cert: Medical Assistant Certification.

ESSENTIAL JOB FUNCTIONS

- Maximizes treatment adherence by coordinating with patients and a multidisciplinary team.
- Tracks and reviews orders, medical tests, and medical procedures and follows those results to provide timely decision-making information for the providers.
- Functions in an organized and time conscious manner.
- Maintains necessary professional standards for employment.
- Understands the side effects of procedures and treatments and coordinates the management of side effects under the directions of the physician.
- Supports the patient during difficult decision-making periods.
- Participates in the appropriate division of these job functions with other patient navigation staff with direction and guidance from providers and management.

Experience:

Essential Functions:
- Responsible for greeting every patient in a courteous, professional, and timely manner every Time.
- Responsible for answering telephones in a friendly and efficient manner in conjunction with Guthries Telephone Standards. Screens telephone calls, takes messages and provides information.
- Responsible for scheduling and maintaining all patient appointments electronically. Verifies patient information at time of scheduling and assigns B# / MRN to new patients. Responsible for editing appointment schedule at the direction of practice management.
- Registers patient. Reviews, verifies and corrects patient demographic and insurance information along with scanning current insurance cards into the patients confidential medical record.
- Verify eligibility for major insurance carriers including but not limited to New York and Pennsylvania Medical Assistance to ensure accurate billing. Complete various types of insurance forms, pre?certifications and referrals.
- Possess the ability to inform, quote, and collect copayments, insurance deductibles, deposits, or unpaid balances at the time of registration as per Guthrie Medical Group, P.C. s policies and guidelines.
- Responsible for daily cash?out and balancing in accordance with the Patient Cash Control Policy.
- Performs liaison duties between patients, physicians, hospital staff, organizational departments, etc., keeping the departments fully aware of all necessary information. Keeps patients informed of any delays.
- Adheres to departmental and organizational policies and attends meetings/huddles as required.
- Requires the ability to prioritize daily tasks simultaneously in a fast?paced, ever changing work environment within a strong team structure.
- Maintains strict confidentiality related to patient health information in accordance with HIPAA compliance.

I. JOB SUMMARY/RESPONSIBILITIES
Responsible for coordinating clinical care of patient. Integrates all health care providers to ensure high quality patient/family outcomes, and establishes a consistent and accountable relationship with patient/family. Key responsibilities include, but are not limited to, initiation and development of the plan of care, patient/family education, utilization management and discharge planning.
II. TYPICAL PHYSICAL DEMANDS
Essential functions- Continuous: lifting up to 1/2 lb., seeing, hearing, talking, grasping, fingering. Frequent to continuous: standing, walking, stooping, forward reaching, handling. Frequent: pushing and pulling up to 30 psi, lifting up to 10 lbs., sitting, bending. Occasional to frequent: squatting, twisting. IV. MINIMUM QUALIFICATIONS
A. Education/Certification and Licensure:
1.Licensure: Registered Nurse, State of Hawaii
2.Diploma or Associate Degree in Nursing; Baccalaureate Degree in Nursing preferred.
3.Current BLS/HCP card in all areas; ACLS, NALS and/or PALS in designated areas.
