Part-Time (24 hours)
Will be responsible for the delivery of care to cardiac rehabilitation patients. Reviews and revise, as directed, the cardiac rehabilitation plan of care by continuous monitoring and assessment of patients to include, but not limited to: modifications of risk factors, exercise tolerance, adverse responses to exercise, heart rate, and rhythm changes during exercise and recovery via visual assessment. Counsel patients and family members on the modification of risk factors for heart disease
Education:
• Graduate of an approved school of nursing or respiratory therapy program, or exercise physiology
• Current licensure in the state of North Carolina as a Registered Nurse or Respiratory Therapist
• Current BLS and ACLS certification
• Knowledge of EKG interpretation per cardiac telemetry
• Proficiency in operating EKG machines and interpreting basic EKG rhythms
Necessary Skills:
• Cardiac experience

• Maintain strictest confidentiality and adhere to all HIPAA guidelines/regulations

Position Summary
The Cardiac Rehab RN evaluates Cardiac patients and formulates a nursing plan of care; implements the plan; evaluates the patient's and families nursing care needs and provides health educational needs for patients. Manages patient care assignments; demonstrates professional and clinical competence; provides age specific care; supports program compliance and maintenance of all records and logs; provides daily clinical nursing services.
Education:
- High school diploma or equivalent, preferred
- Associate or Bachelors of Science Degree in Nursing from an accredited school of nursing.
- One year of recent and relevant experience in a Coronary Care, Intensive Care setting or Cardiac Rehabilitation
- Current Illinois RN license

The role you'll contribute:
Assists with the planning and implementation of outpatient cardiac rehab and education programs for adult and geriatric populations, aiming for optimal recovery and promoting life-long wellness through exercises and education. This role manages individualized, goal-directed nursing care using the nursing process in alignment with departmental and hospital policies and procedures. The position entails direct patient care, including telemetry monitoring, recording of vital signs, management of medical problems and emergencies, and providing education and counseling. The role is pivotal in assessing and evaluating patients' cognitive, social, functional, and psychological status and in developing and implementing cardiac rehabilitation treatment plans. It involves performing cardiac monitoring, instructing patients on proper use of exercise equipment, adjusting exercise prescriptions as indicated, and assessing patients' response to exercise. Responsibilities also include communicating with telemetry and nursing to mobilize patients for Phase I Cardiac Rehab, reviewing charts, teaching home programs, assessing and screening patients for cardiac rehab, and providing education to patients and their families. Furthermore, the role requires the development and management of program policy and procedures, facilitating delivery of care, and ensuring adherence to the Corporate Compliance plan and all rules and regulations of all applicable local, state, and federal agencies and accrediting bodies.

- Advises patients on anticipated gains and possible risks associated with the program, obtaining prerequisite written order diagnostics and informed consent.
- Develops individualized, result-oriented care plans in collaboration with clients and referring physicians.
- Evaluates new cardiac patients to obtain accurate medical history, identify cardiopulmonary risk factors, and determine psychosocial disease manifestation.
- Completes assigned data collection, document patient outcomes, and demonstrate progress towards benchmarks.
- Provides formal and informal instruction in rehabilitation concepts, focusing on cardiac anatomy, proper nutrition, medication usage, physical retraining, and lifestyle modification.
- Motivates patients and guides the rehabilitation process, modifying programs according to individual progress.
- Performs ongoing patient assessments, including monitoring EKG, blood pressure, pulse oximetry, breath sounds, incisions, and signs of congestive heart failure.
- Demonstrates competency in all procedures within the scope of practice, maintaining knowledge of development and treatment needs for adolescent, adult, and geriatric patients.
- Documents in accordance with insurance guidelines and post charges accurately in Cerner/series.
- Recognizes and responds to changes in patient condition, alerting physicians as necessary, and respond promptly to emergency situations.
- Assists with development of cardiac rehab policies and procedures.
- Provides clerical/housekeeping duties such as: follows department policies for cleaning of equipment, maintains supply of exercise programs and documentation forms for cardiac rehab, listens to answering machines and return calls, inventories equipment and supplies monthly, and floats to other sites as needed.

Qualifications
MINIMUM QUALIFICATIONS:
- Graduate of an accredited or NLN-approved RN program.
- BLS (or obtained during orientation)
- Current state nursing license
- Prefer one year full time or three years part time experience in acute care setting.
- ACLS, PALS, (or obtained within six months of employment) in required specialty.
- Handle with Care (or obtained within six months of employment) in required specialty
- Must successfully pass any pre-employment assessment(s) required by the facility.
