**Responsibilities**
Responsible for overall documentation, development, update, and review of the Interdisciplinary Team's (IDT) Plans of Care.
+ Document IDT Plan of Care using IDT feedback based on discipline specific assessments, re-assessments, and participant/caregiver interviews.
+ Assist IDT with maintenance and updates to Plan of Care per assessment schedule and as needed with changes to participant condition. Update changes in Electronic Medical Record (EMR).
+ Attend IDT meetings, monitor and evaluate outcomes, note changes to care plan and update meeting participants of information/changes to the care plan and ensures participants EMR record is updated and accurate.
+ Review consultant notes for changes, completion and accuracy of care plans to include reviewing hospital discharge summaries and extended nursing home care admission orders. Ensure reminders on upcoming re-assessments are in place, follow through occurs and is documented.
+ Communicate to direct care staff regarding their role in implementing the Plan of Care. May participate in interventions and report outcomes.
**Requirements**
Licensure / Certification / Registration:
+ Practical/Vocational Nurse preferred.
+ Registered Nurse preferred.
Education:
+ High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
+ Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.

The Graduate will gain clinical perspective of the roles and responsibilities of a leading PBM supporting health plan clients. Our ESI Health Plan Division (HPD) residency is a holistic program that provides resident a great foundation in managed care, with a focus in clinical strategy within client account management and as a core member of our HPD team. The resident will engage in multiple different projects and presentations that support our clients, account teams, and overall HPD team.
