• Provides basic nursing assistance to participants as established in department's policies and procedures.( i.e.: toileting, feeding, transportation and bathing when needed)
• Documents treatments/procedures, observations, nursing care and participant care activities on appropriate forms per established department guidelines, when needed.
• Takes assigned participants monthly weight and reports results to PACE dietician.
• Ensures that resident's/participant's rights are adhered to.
• Participates in departmental activities, multidisciplinary team meetings, department meetings and in-services and follows established guidelines as needed.
• Completes monthly training in a timely manner.
• Assists with monthly outings when assigned.
• Participates in a structured rotation program that provides cross training for all PCA duties in the PACE program.

• High School Diploma preferred
• Graduate of a Certified Nursing Assistant Program

• 3 months clinical experience as a CNA preferred
• Minimum one-year experience working with a frail/elderly population

• Certified by the State of Florida as a Certified Nursing Assistant in good standing
• Current American Heart Association BLS Certification
• Valid Florida Driver's License & Proof of Insurance preferred
• Current CPR Certification

• Good communication skills
• Ability to speak read and write English and Spanish, knowledge of Creole a plus
• Ability to think and act calmly to meet unusual occurrences of the job
• Demonstrates a caring attitude toward PACE participants, participants, family members, employees, and other facility guests

The New York City Department of Youth and Community Development (DYCD) invests in a network of community-based organizations and programs to alleviate the effects of poverty and to provide opportunities for New Yorkers and communities to flourish. DYCDs Beacon Community Center is comprised of over 90 school-based programs serving young people enrolled in grades K-12 during afterschool hours; and young adults/older adults and families in the evening and on weekends. Through its network of community partners, Beacon Community Centers offers high quality programs that offer a strong balance of academic enrichment, recreation, cultural and skill building activities that build 21st century skills. Beacon community partners are awarded funds through a competitive Request For Proposal (RFP) process. Under the supervision of the Deputy Director with latitude for the exercise of independent action and decision making, provides leadership and support to the Department in ensuring compliance with all internal and external program standards, and to promote continuous quality improvement. Responsibilities will include but not be limited to:

• Analyze program statistical data & prepare reports, graphs and charts for planning purposes.
• Assist in the development of program materials for distribution. Materials include pertinent information concerning deadlines, policies and procedures.
• Monitor and evaluate Community-Based Organization (CBO).
• Review, provide feedback and approve annual scope of work.
• Conduct site visits during late afternoon, evening and weekends is a non-negotiable requirement.
• Provide technical assistance and direct work improvement or corrective action for underperforming CBOs.
• Evaluate contract performance reports, as well as agency fiscal documents to ensure contract compliance.
• Make summary reports to higher administrative levels along with recommendations to improve program performance.
• Knowledgeable of Youth Development and Youth Leadership standards and ability to develop and integrate practices into program development.
• Carry out special assignments, tasks and projects as assigned by senior management as needed.

Qualifications:

1. A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or 2. High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or 3. Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.

Functions in collaborative practice with physician(s) and advanced practice provider(s)(APP) in the Urgent Care Center (UCC) setting. Provides ambulatory nursing care as directed by physician(s) and advanced practice provider(s), including patient focused assessment, intervention, and education. Skillfully and consistently performs daily clinical duties per established policy and procedure.

Education:

• High School Diploma or equivalent required. Graduate of an accredited school of vocational nursing.
• Minimum of one year nursing experience in a clinic or office setting preferred. 1-year pediatric experience preferred.

Certification/Licensure:

• Current license to practice vocational nursing in the State of Texas.
• Current CPR certification.
• PALS must be obtained within 1 year of hire.

Compensation Details: The expected base salary for this position is starting at $53,000 base salary depending on experience. This position is also bonus eligible - based on specific and relevant business metrics.
