Duties and Responsibilities:
Through an on-site interdisciplinary team of professional staff, JASA’s Community Guardian Program serves as the court-appointed legal guardian of hundreds of adults across the five boroughs, providing them with critical services and supports with the goal of keeping them in their respective communities and as independent as possible. JASA's Community Guardian Program serves some of the most vulnerable New Yorkers, including the seriously and persistently mentally ill, the homeless and formerly homeless, and people facing eviction.
Key Responsibilities:
- Maintain oversight and monitor the status of all clients on caseload.
- Evaluate and assess client’s capacity to manage their own affairs and protect themselves from financial exploitation.
- Advocate on behalf of the client.
- Seek appropriate guidance from the supervisor in situations requiring clarification and
- consultation especially for cases that meet the high risk criteria.
- Monitor the disbursement of monies as issued through JASA relief funds or client financial management services to secure needed goods and services.
- Screen the client for program eligibility, conducts triage for emergency interventions,
- ascertains short-term needs, and provides appropriate information and referral services
- to clients, families, and collaterals. Refer to, and maintains a cooperative relationship with, other community agencies to meet the needs of clients.
- Conduct referrals for the direct provision of those benefits including housekeeping, home health aides, public assistance, Medicare, Medicaid, SNAP, legal aid, protective services, medical and psychiatric evaluations, etc.
- Develop and implement a service plan to provide a full range of social services geared towards the client’s direct needs.
- Utilize HRA and agency databases to input client data, case records and other client related documentation.
- Screen client eligibility for benefits and entitlements.
- Complete required case records, reports and statistics within mandated timeframes,
- utilizing computers and mobile devices as required.
- Conduct in-home, facility and community visits within the 5 boroughs to evaluate client functioning, monitor risk, and assess current services in place.
- Attend court hearings and represents the program at community conferences and meetings.
- Emergency Schedules—In certain crisis situations, such as “cold weather days” requiring visitations of clients lacking heat, or scheduled appointments for emergency medical or psychiatric evaluations, initiating emergency home care services, or implementing court-ordered protective actions.
- May be required to report to work outside of normally scheduled work week, including evenings, weekends and holidays. Work closely with law enforcement and ACT/AOT Teams as needed for assistance.
- Participate in staff trainings.
- Perform such other duties as assigned.
Qualifications:
- Graduate of an Accredited College or University with a Master of Social Work or equivalent Master’s level degree in counseling, psychology, gerontology or related field.
- Good writing and record keeping skills
- Must be computer literate, including the ability to learn and operate software.
- Ability to travel within the 5 boroughs to client homes or current place of residence.
- Excellent organizational and time management skills.
- Understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients and families.

Position Overview: RN Case Managers for a weekday contract opportunity in the inpatient unit. This role is ideal for local Registered Nurses with strong hospital-based care coordination skills looking to make an immediate impact on patient transitions and discharge planning.
Key Responsibilities:
- Oversee patient care transitions, discharge planning, and resource coordination
- Collaborate with physicians, social workers, and interdisciplinary teams to support optimal care outcomes
- Conduct utilization reviews and ensure compliance with payer and regulatory requirements
- Monitor patient progress, advocate for appropriate services, and facilitate referrals
- Maintain accurate documentation and case management records
- Active NYS Registered Nurse (RN) license required
- Associate's Degree in Nursing
- AHA BLS Certification

Job Description
The Nursing Clinical Manager (NCM) for Case Management is responsible for the day to day management of the Case Managers. This includes, but is not limited to, daily supervision with the case managers, staff recruitment, retention, labor relations, daily staffing, employee performance evaluation, performance improvement, and staff education.
The NCM Manager serves as a resource/mentor for Case Managers, Social Workers and other institutional personnel, and participates in the development and implementation of new Institutional initiatives. The NCM reports to and assists the Director of Case Management in providing ongoing education to Case Managers and other hospital personnel as needed. The Nursing Clinical Manager collaborates with Appeals Management, Managed Care Contracting and other institutional departments to ensure contractual compliance and optimal reimbursement by Third Party Payers.
The Nursing Clinical Manager for Case Management has a collaborative reporting relationship with the Nursing Director for medicine and attends the monthly Nursing Leadership meeting.
**Qualifications**
Education Requirements
BSN, Masters Degree in Nursing or health related field.
Experience Requirements
Minimum of 5 years of case management experience and 5 -7 years of experience as a clinical nurse.

Certification: PALS (in ED; PACU; IR; Peds ICU, and APNs Pediatric Oncology depts.)
**Responsibilities**
1. Attends interdisciplinary rounds on units as warranted.
2. Meets with Case Managers, as warranted, to review medically complex and long stay cases; refer to physician advisor as indicated.
3. Collaborates with Social Work Supervisors to resolve discharge planning issues on assigned units.
4. Regularly reviews avoidable day data and provides follow-up with Case Managers on accuracy and completeness of avoidable days.
5. Oversees Case Managers interactions with physicians, nursing, social work, payors, patients and families.
**Clinical Leadership:**
1. Develops, implements, and maintains professional standards.
2. Ensures that the overall Case Management program supports patient, unit and clinical service objectives.
3. Provides operational and clinical oversite to case managers to insure that all day to day activity supports the mission of efficient and clinically appropriate patient care.
4. Demonstrates integral involvement in the Nursing Department PI/QA program. Participates in monthly Nursing Department meetings.
5. Works with unit case managers, clinical nurse managers, physicians, and social work to facilitate Interdisciplinary Rounds to ensure compliance with the patient management plan and the appropriate length of stay for each patient.
6. Conducts rounds on the inpatient units as warranted to determine and improve hindrances to appropriate length of stay.
7. Collaborates with the Director of Case Management to maintain, review and update all Case Management specialty policies in accordance with hospital, professional and regulatory standards.
8. Supports the overall strategic goals of the Hospital and the Nursing Department.

Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $134616 - $173355 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.

Job Summary
Reports to and is under direct supervision of Case Management Department. Provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Promotes continuity of care and cost effectiveness through the integrating and functions of case management.
