Join the Redwood Crossings Team or the Sequoia Crossings Team in supporting stable housing for local neighbors that are chronically homeless. You can make a difference putting your compassion in motion with The ARCHES Project! We have multiple positions available, multiple shifts, weekdays and weekends.
This position is located at a Permanent Supportive Housing (PSH) program that provides affordable and stable housing to people who experienced chronic homelessness. The person in this position will meet with residents one-on-one to perform needs assessments, develop support plans, and identify resources that will help support each resident based on assessments. The Redwood Crossing Case Manager will coordinate and maintain a healthy relationship with other community providers in order to ensure each resident gains access to the services that they need.
MINIMUM QUALIFICATIONS The requirements listed below are representative of the knowledge, skill, and/or ability required.
- High School Diploma or GED and 2 years of social service experience. Equivalent combination of education and experience may be accepted.
- Knowledge of/or experience with: the principles of coordinated assessment, working with unsheltered or vulnerable populations, as well as best practices in homeless service delivery is preferred.
CERTIFICATES, LICENSES, REGISTRATIONS
- Driver qualifications are required for this position. Valid driver license, insured vehicle available for use on the job, and a satisfactory drive record for the previous three years are required.
- Candidate must pass a comprehensive MWVCAA background screening prior to employment.
- Basic Life Support/First Aid Certification is required within first 30 days of hire.
- Food Handler's Card or ability to obtain within 30 days.

Under the leadership of the Director, Home Health, the Home Health Case Manager I is an active member of the Home Health team that delivers professional end-of-life care and services consistent with the strategic vision, goals, philosophy and direction of the Home Health Department and CRHS. The Home Health Case Manager reviews patient and family needs and develops age-appropriate and culturally sensitive nursing care for patients of all ages in their homes. The Home Health Case Manager I formulates an appropriate nursing plan of care with goals mutually set by the RN and the patient and/or caregiver. The Home Health Case Manager I formulates an appropriate and safe aide plan of care and supervises the aide every 14 days in the patient’s home and assesses the effectiveness of interventions and education to the patient and his/her family.
Basic Qualifications:
- Education:
Requires an Associate’s degree in nursing.
- Experience:
Requires one to two years of work-related experience or any equivalent combination of education, training, and experience.
- Licensure, Registrations & Certifications:

- Reports significant changes in the patient’s condition promptly to the attending physician.
- Utilizes nursing skills to independently administer intermittent safe, timely and effective nursing care and therapeutic treatment in the patient’s home as ordered by the physician.
- Evaluates and documents the effectiveness of the plan of care as developed by all members of the interdisciplinary team working with the patient.
- Actively participates in Home Health interdisciplinary group meetings and home health care conferences.
- Submits completed documentation in accordance with established standards for timeliness and accuracy.
- Complies with Home Health quality, satisfaction, and regulatory compliance standards to ensure they are either met or exceeded.
- Works continually to identify new approaches that improve the quality and timeliness of Home Health care.
- Follows all Hospice policies and procedures and ensures compliance with all federal and state laws, regulations, and JCAHO requirements.
