**Job Summary and Qualifications**
As a Patient Care Technician you will be a crucial member of our collaborative nursing care team, delivering hands-on care and helping create a safe, welcoming environment for patients. **What will you do in this role:**
+ Obtains and records initial and ongoing patient information throughout the cycle of care, including admissions data, vital signs, height and weight, blood pressure, oxygen saturation, intake and output, calorie counts, and other patient information as outlined by policy.
+ Documents observations, interactions, patient responses to performed activities, and any physical or behavioral changes at the time of occurrence. Reports observations of patients to the RN/Team Lead.
+ Helps with mobility by regular turning and positioning, dangling, and ambulating patients. Helps with the use of walkers, crutches, canes, and wheelchairs. Transfers patients to and from other units, departments, or entities. Helps with discharges. Demonstrates proper body mechanics.
+ Provides or helps with non-medicated care, including non-sterile dressing changes, skin protection, application of heat or ice, and ventus compression devices.
+ Obtains or helps in the collection of specimens for diagnostic or waived tests. Ensures accuracy of all labels and transfer of specimens.
+ Helps patients with activities of daily living including eating and drinking, bathing, shaving, oral care, grooming, and elimination. Changes bed linens as needed.
+ Performs hourly rounding and responds promptly to call lights and patient requests. Aims to anticipate patient needs and resolve them proactively
**What qualifications you will need:**
+ 1 year of Hospital Experience
+ Patient Care Tech Training, or Star Tech Program or (CNA) Certified Nursing Assistant must be obtained within 60 days of employment start date

**Job Description**
The Director of Patient Care Services (Director) at Saint Luke's East Hospital (SLEH) assumes accountability for the overall planning, coordination, and delivery of acute care services within the assigned departments. Most of these departments are nursing in nature but also may include other clinical services, within patient care services, some in a matrixed relationship.
The Director is responsible for utilizing and modeling the Saint Luke's values of kindness, respect, excellence, safety and teamwork. The Director is responsible for stewardship of resources (staffing, space, equipment, and supplies) for the assigned departments, including cost center budget management.

Working collaboratively with the Medical Staff, the Director also assumes a leadership role in planning, developing and implementing programs, processes, and services which support the mission, vision and strategic direction of the entity, system and enterprise.
This role has 6 direct reports. They are the managers of the following departments: Emergency Department, Medical Specialty Unit, Medical Tele Unit, Progressive Care Unit, Respiratory Therapy and our Wound Program. This role also has a matrixed relationship with several other smaller departments.
A strong candidate would possess excellent leadership skills, previous acute or critical care management experience, the ability to assess opportunities and to propose innovative solutions, independently and be a change agent.

- Assesses and analyzes injured, acute, or chronically ill members/clients medical and/or vocational status; develops a plan of care to facilitate the member/client's appropriate condition management to optimize wellness and medical outcomes, aid timely return to work or optimal functioning, and determination of eligibility for benefits as appropriate.
- Communicates with member/client and other stakeholders as appropriate (e.g., medical providers, attorneys, employers and insurance carriers) telephonically or in person.
- Prepares all required documentation of case work activities as appropriate.
- Interacts and consults with internal multidisciplinary team as indicated to help member/client maximize best health outcomes.
- May make outreach to treating physician or specialists concerning course of care and treatment as appropriate.
- Provides educational and prevention information for best medical outcomes.
- Applies all laws and regulations that apply to the provision of rehabilitation services; applies all special instructions required by individual insurance carriers and referral sources.
- Testifies as required to substantiate any relevant case work or reports.

**Required Qualifications**
- Active and unrestricted Kansas Registered Nurse license
- Must live within a 30 mile radius of Kansas University Medical Center. Zip code for KU is 66160
- 3 years clinical practical experience preference: (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members.
- 2 years Case Management, discharge planning, and/or home health care coordination experience
- Ability to travel 75% plus, will be reimbursed for mileage and parking.

- Effective communications, organizational, and interpersonal skills.
- Ability to work independently (may require working from home).
- Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications.

- Willing and able to obtain multi state RN licenses if needed, company will provide.

This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Develops, implements, and supports Health Strategies, tactics, policies, and programs that ensure the delivery of benefits and to establish overall member wellness and successful and timely return to work. Services and strategies, policies and programs are comprised of network management, clinical coverage, and policies.
Community Care Case Manager use a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality, cost effective outcomes.
Additional responsibilities to include but not limited to the following:
- Acts as a liaison with member/client /family, employer, provider(s), insurance companies, and healthcare personnel as appropriate.
- Implements and coordinates all case management activities relating to catastrophic cases and chronically ill members/clients across the continuum of care that can include consultant referrals, home care visits, the use of community resources, and alternative levels of care.
- Interacts with members/clients telephonically or in person. May be required to meet with members/clients in their homes, worksites, or physician's office to provide ongoing case management services.
- Assesses and analyzes injured, acute, or chronically ill members/clients medical and/or vocational status; develops a plan of care to facilitate the member/client's appropriate condition management to optimize wellness and medical outcomes, aid timely return to work or optimal functioning, and determination of eligibility for benefits as appropriate.
- Communicates with member/client and other stakeholders as appropriate (e.g., medical providers, attorneys, employers and insurance carriers) telephonically or in person.
- Prepares all required documentation of case work activities as appropriate.
- Interacts and consults with internal multidisciplinary team as indicated to help member/client maximize best health outcomes.
- May make outreach to treating physician or specialists concerning course of care and treatment as appropriate.
- Provides educational and prevention information for best medical outcomes.
- Applies all laws and regulations that apply to the provision of rehabilitation services; applies all special instructions required by individual insurance carriers and referral sources.
- Testifies as required to substantiate any relevant case work or reports.
- Conducts an evaluation of members/clients' needs and benefit plan eligibility and facilitates integrative functions using clinical tools and information/data.
- Utilizes case management processes in compliance with regulatory and company policies and procedures.
- Facilitates appropriate condition management, optimize overall wellness and medical outcomes, appropriate and timely return to baseline, and optimal function or return to work.
- Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes, as well as opportunities to enhance a member's/client's overall wellness through integration.
- Monitors member/client progress toward desired outcomes through assessment and evaluation.
**Required Qualifications**
- Active and unrestricted Kansas Registered Nurse license
- Must live within a 30 mile radius of Kansas University Medical Center. Zip code for KU is 66160
- 3 years clinical practical experience preference: (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members.
- 2 years Case Management, discharge planning, and/or home health care coordination experience
- Ability to travel 75% plus, will be reimbursed for mileage and parking.
**Preferred Qualifications**
- Excellent analytical and problem-solving skills
- Effective communications, organizational, and interpersonal skills.
- Ability to work independently (may require working from home).
- Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications.
- Efficient and Effective computer skills including navigating multiple systems and keyboarding
- Willing and able to obtain multi state RN licenses if needed, company will provide.
- Bachelor's degree preferred.
- Certified Case Manager is preferred.
- Additional national professional certification (CRC, CDMS, CRRN, COHN, or CCM) is preferred, but not required

**Pay Range**
The typical pay range for this role is:
$54,095.00 - $116,760.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
