Position Description: The Registered Nurse II Children First, Nurse-Family Partnership is responsible for are assigned responsibilities for providing direct or indirect professional nursing services in a variety of settings. Children First, Oklahoma's Nurse-Family Partnership Children First Program (oklahoma.gov), is offered free of charge to first-time, low resource mothers. This program focuses on women who have had no previous live births. A specially trained nurse provides personal visits in the family's home or other location beginning early in pregnancy and continuing through the child's second birthday. Nurses complete nursing assessments utilizing the Nursing Process. Nurses provide education related to pregnancy, health, community resources, parenting, safety, child development and other topics as needed. They also support the mom as she works toward attaining life goals that will ensure a more secure future for the family.
Position Responsibilities/Essential Functions:
* Prepares for and actively participates in reflective supervision, case-conferences and team meetings.
* Monitor and assess the health status of first-time mothers and infants to age two utilizing the Nurse-Family Partnership Guidelines, program guidelines and Physician Approved Protocols.
* Works with the client/family to develop long-term and short-term personal and family goals.
* Develop a plan of care based on nursing assessment. Provide nurse case management and health education.
* Deliver professional nursing care in the client's home, or other setting.
* Completes electronic documentation in time frame and format as outlined by the Children First Program.
* Assure quality health care through use of various measures such as record review, peer review, direct observation, assessment of individual, family, and community for the desired outcome.
* Assist with activities in support of public health emergency response.
* Works with families, following a comprehensive program plan that focuses on the client's development of personal health, parenting, environmental health, preventing unintended subsequent pregnancies, and finding or returning to work and school.
* Completes required training and ongoing education as outlined by the NFP Model and Children First Program Guidelines.
* Achieve and maintain caseload as outlined by the Children First Program Guidelines.
* Collaborates with internal and external partners.
* Being present in the office is an essential function of this job

Minimum Qualifications:
Requirements consist of possession of a valid Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or, Registered Nurse License recognized by the Enhanced Nurse Licensure Compact (eNLC) and one year of professional nursing experience, OR, a Bachelor's in Nursing and possession of a valid permanent Oklahoma license as a Registered Nurse as approved by the Oklahoma Board of Nursing or, Registered Nurse License recognized by the Enhanced Nurse Licensure Compact (eNLC). Up to date CPR certification.

Physical Demands and Work Environment:
Work is performed 60-75% in the families' homes or locations of the client's choice (approximately two to three visits per day), and 15-25% in the office when prepping documents. May be exposed to communicable diseases, bodily fluids, toxic substances, and medicinal preparations. Varied activities include sitting, walking, stooping, lifting, bending, pushing, and reaching. Exposed to inclement weather conditions while driving to and from office, homes, training, meetings, etc. Flexible schedule, which includes working occasionally after normal hours of 8:00 am - 5:00 pm, such as evening visits to meet the needs of the individual family. Work-related travel is required, travel time averages 5-25% based on the geographical area covered, and the location of training and meetings. A valid driver's license is required. Use of personal vehicle with up-to-date insurance may be required.
