Responsibilities
The Clinical Coordinator (CC) is a pivotal leader in rehabilitation, serving as the central point of contact to orchestrate comprehensive care and community reintegration for patients. Much like case managers in inpatient rehab hospitals, the CC drives an efficient, effective, and transdisciplinary rehabilitation program , ensuring every patient's needs are met with precision and personalized attention.
Responsibilities Include:
- Manages the provision of individual patient services and collaborates with clinical staff to develop rehabilitation plans.
- Coordinates the ongoing provision of rehabilitation services.
- Formulates comprehensive, transdisciplinary rehabilitation treatment plans, clinical outcomes, goals/objectives and service delivery for referred patients in conjunction with the patient/care partner and the clinical team.
- Completes CC evaluation and develops patient initial, progress and discharge reports and monitors the patient's status on an ongoing basis.
- CCs are responsible for completion of patient clinical outcome tools based on diagnosis.
- Monitors patient, family, physician and payer/case manager, referral source satisfaction with the program on a regular basis.
- Leads interdisciplinary case conferences, team meetings, and communicates Treatment Plans/Progress Reports on a regular basis to the physicians, external case manager/payor and/or patient/family members.
- Schedules/facilitates interdisciplinary case conferences with the clinical team, payers/case managers, physicians and family members.
- Communicates regularly with patient, family/ care partner, payer/case manager and physician regarding patient progress and any important issues that arise.
- Facilitates acquisition of necessary equipment to enable achievement of agreed upon functional goals (i.e. HMO, insurance carrier, etc.) with funding source.
- Participates in medical provider meetings to report/resolve medical issues for person served.
- Monitors projected hours of service to be delivered to the patient and reports hours/subsequent changes to the local leadership (Executive Director/Director of Clinical Management/Program Manager).
- Negotiates with funding sources for extensions of service beyond initial authorizations.
- Acquires and tracks orders from physician for all provided ongoing services after the initial orders are obtained.
- Adheres to standards of the Company's Code of Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.
- Attends, participates in and/or conducts internal staff development programs, obtains continuing education as required by Company policy and regulations. Maintains and enhances clinical practice skills.
- Performs other duties, as assigned.
Educational and Experience Requirements:
- Minimum of Master's or Doctorate degree in Occupational Therapy, Physical Therapy, or Speech-Language Pathology.
- Maintains relevant professional licensure/registration/certification in the state of practice.
- Current certification as Certified Brain Injury Specialist (CBIS) preferred.
- Current BLS (Basic Life Support) First Aid and CPR certifications.
- Minimum of two-year's experience in a rehabilitation setting with neurologically impaired patients
- Prior case management and/or insurance administration experience preferred.
- Demonstrates excellent organization, problem solving, decision making and communication skills.
- Organized and able to function under minimal supervision.
- Has reliable transportation and able to travel to/from multiple assignments/locations on a daily basis.
- Ability to lift fifty (50) pounds. Moving, lifting, or transferring of patients may involve lifting of up to 100 pounds.
- Ability to stand for extended periods.

Position Summary: Responsible for the supervision of the staff members assigned to the shift or specialty team and for the successful operational and administrative functions of the shift. FUNCTIONS:The Clinical Coordinator for Nursing is a faculty member whose role is to function as a Clinical Coordinator for undergraduate and/or graduate students matriculating in Nursing at Howard University in addition to other faculty responsibilities.SUPERVISORY ACCOUNTABILITY:N/APRINCIPAL ACCOUNTABILITIES:Under the direction of the Chairperson & Asst. Chairperson in the Division of Nursing, the Clinical Coordinator/Faculty responsibilities include but are not limited to:Obtaining affiliation agreements with desired healthcare facilities based on program clinical needs.Obtaining clinical site requests from course faculty. Engaging with facility to request student rotations.Verifying nursing students' compliance with the facility's health requirements (PPD, vaccinations, etc).Communicating with course coordinators regarding students' health requirement compliance status.CORE COMPETENCIES:Evidence of effective use of computer technologyDemonstrated experience in online and/or distance learning, as well as curriculum design and implementation in higher education is a plus.Candidate will also have a commitment to teamwork, and organizational skills with attention to detail, thoroughness, excellent communication, and practical decision-making.Documented research experience. *This description is not meant to be inclusive of all aspects of employment*MINIMUM REQUIREMENTS:Masters degree in healthcare related field (required).
