Position Summary:

Responsibilities: Essential Duties

  • Provides education/training to our existing or potential customers on the use of B. Braun Medical's products.
  • Works with Sales Representatives/CNC to advance the sales process and maximize customer satisfaction through in-service education and training programs (Mentoring Program), clinical and product trouble-shooting, and follow-up activities.
  • Product focus revolves primarily around the Infusion Therapy and Introcan Safety product line but may include any of the product portfolio.
  • Functions as a clinical and product expert representing B. Braun Medical in the healthcare community.
  • Conducts and leads in-services and educational sessions relative to BBM products during evaluations, conversions and trouble-shooting.
  • Serves as a product resource expert/mentor for other BBM educators while in accounts performing training/educational activities.

Essential Functions of the Clinical Counselor:
  • To provide high-quality clinical direction and leadership to staff in all program settings, and fostering a positive youth/human development environment.
  • To complete assessments, comprehensive plans including community and specialized services in support of assigned clients using positive youth development, trauma-informed, and culturally appropriate frameworks.
  • A minimum of 50% of the working hours will be spent providing quality in-person individual or group counseling services and mental health case management, including understanding medication, doctors' appointments, according to the youth's identified level of service.
Qualifications for the Clinical Counselor:
  • Required a Master's degree in human services, counseling, social work, or other related field.

  • Possess sound judgment and knowledge of adolescent development and behavior.

The Dermatology Clinical Assistant will master the entire patient process through check-in, patient rooming, scribing, check-out, and patient navigation.

Job Duties and Responsibilities:

Core Front Office Functions:

  • Actively participates in maintaining and/or improving quality improvement initiatives regarding Physician Billing computer system and patient satisfaction survey.
  • Maintain high level of competence on Physician Billing computer system.
  • Maintain patient and physician office schedules.
  • Answer telephones within three rings, prioritize calls and coordinate service with the answering service.
  • Establish and maintain accurate patient records and record filing system.
  • Receive/transfer patient records with required records release.
  • Types, files, and distributes correspondence as directed.
  • Collect and process payments at time of service.
  • Coordinate as necessary with billing office.

Core Clinical and In-Room Assistant Functions:

  • Conducts pre-visit planning by reviewing and updating the patient's medical record prior to the appointment with recent test results and correspondence and determines needed services per practice protocol.
  • Confers with the physician, NP or PA regarding any incomplete patient tests/consults or other incomplete orders to the patient visit.
  • Prepares patients for the examination and treatment by escorting them from the reception area to the exam room prior to the provider entering the exam room.
    • Height, weight, and vital signs
    • Update medication list
    • Update allergies in record
    • Update personal, family, and social history
    • Inquire about smoking status
    • Obtains the Chief Complaint for the visit
  • Perform provider documentation of patient information, following all guidelines for documentation and billing following SLPG guidelines.
  • Demonstrates skill in documenting and maintaining patient care records in a complete, thorough, and timely manner.
    • Enter provider orders in EMR per physician instruction accurately.
  • Clarifies and corrects any documentation under the direction of the provider.
  • Completes all documentation real time and within the same date of service
  • Communicate to provider when chart is complete and ready for review and sign off.
  • Provides other patient care services as needed/directed:
    • Keeps exam rooms clean
    • Maintaining clinical supplies in exam rooms
    • Cleans and autoclaves contaminated instruments
    • Disposes of Bio-Hazardous waste and regular waste per practice protocol.
    • Assist with in-room procedures
    • Patient and family education
  • Maintains supplies:
    • Medical supplies
    • Disposing of expired Sample medication
  • As appropriate:
    • Assists with scheduling outpatient testing
    • Pre-certifying patients for procedures and referral processing

Physical and Sensory Requirements:

Requires sitting, standing, and walking for extended periods of time (up to 8 hours at a time). Requires continual use of fingers for patient care, writing, and computer entry. Routinely uses upper extremities; occasional requirement to lift up to 25 pounds. Occasionally push cart with supplies up to 30 pounds. Occasionally push wheelchair with patient weighing up to 325 pounds. Regularly requires ability to stoop, bend, and reach above shoulder level. Requires ability to hear normal conversation and good general, near, and peripheral vision.

Education:

High school diploma.

Graduate of MA program, LPN, or Surg Tech preferred.

Training and Experience:

Formal MA training or one year of clinical experience.

Current CPR within 6 months of hire.

Job Summary
The ambulatory clinical pharmacist is responsible for assuring the provision of safe, efficacious, cost-effective, and high-quality medication therapy disease management to patients in the ambulatory setting. The pharmacist is embedded within a multidisciplinary clinical team, works under a collaborative practice agreement with the physician(s), and provides direct patient care, via either in-person or telephone interventions. The pharmacist will consult with the patient and physician(s) to identify, resolve and prevent medication related problems, as well as establish mutually agreed upon treatment goals. The pharmacist assesses each of the patient's medications for indication, effectiveness, safety, tolerability, and adherence issues and provides care in a way that maximizes patient satisfaction. The pharmacist will titrate/manage/monitor the patient's medication therapy to optimize clinical outcomes and will identify and address gaps in care.
Job Duties
+ Assumes responsibility for the delivery of high-quality medication disease management services for the specific patient care area in which the pharmacist is assigned.
+ Demonstrates proficiency in medication and disease management, as well as a thorough understanding of the medications, clinical conditions, and current treatment guidelines for the conditions in their assigned clinical areas.
+ Gathers appropriate clinical, imaging, and laboratory data necessary to make appropriate decisions on the management of the patient's medication therapy and related diseases and conditions.
+ Assesses patient's medication therapy regimen for appropriateness, effectiveness, tolerability, safety, and potential for adverse events.
+ Make clinically appropriate changes to regimen which will maximize effectiveness and patient satisfaction, while working to minimize potential for both acute and chronic complications Provides in-services, in-depth drug information, and other educational and informational materials to members of the medical, nursing and other healthcare professionals.
+ Provides patient with appropriate educational materials and information in order help them understand and actively participate in their care.
+ Provides appropriate documentation of each encounter in the electronic health record and conveys that information to the referring physician and next provider of care as appropriate.
+ Attends, participates, and upon request, presents at department educational programs.
+ Teaches and trains pharmacy interns, pharmacy residents, students, pharmacy technicians and new pharmacists, regarding information in all appropriate areas of pharmacy practice, policies, and procedures.
+ Assists the Ambulatory Clinical Pharmacy leadership team in implementation of new clinical programs and sites including building and organization of the new program, education and certification of all staff members Coordinates and supervises work-place activities and resources; promotes professional competence; assures that services rendered, and work performed are in accordance with departmental standards, state and federal regulations, and billed appropriately.
+ Advises, assists, and supervises the work of clinic assistants and pharmacy interns; assumes final responsibility for accuracy of their work.
+ Works with facility, medical and nursing staff, support staff, and clinic and region administration where necessary to resolve issues, incorporate clinic services seamlessly into site workflow, and to continually demonstrate the value of the ambulatory clinical pharmacy program.
+ Assists with the review and revision of departmental policies and procedures, as requested.
+ Applies departmental policies and procedures to complex situations and identifies areas for which policies and procedures need to be developed, improved, or revised.
