Job Summary
The Community Bridges Inc. (CBI) Community Based Outreach Team Behavioral Health Technician (BHT) is responsible for responding to calls dispatched by the CBI Access to Care Line and for transports from CBI facilities. The Community Based Outreach Team BHT is responsible for the in-field assessment of each referral to determine the appropriateness of care in consult with the CBI Triage RN when there is a question of need due to the severity of the level of risk in ASAM Dimensions 1, 2 and 3. The Community Based Outreach Team BHT is responsible to ensure that all documentation of Crisis Intervention Notes, Trip Tickets, and Mobile Crisis Intervention Field Forms on each patient prior to the end of his or her scheduled shift.
Skills/Requirements
- Highschool diploma or GED required. Associate degree or higher preferred.
- Minimum of 6 months of health care related experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations preferred.
- Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101 and Clinical Policies and Procedures.
- Emergency Medical Technician certification (valid within the state of Arizona) is preferred for this position.
- Current AZ Driver's License (valid and in good standing).

Responsibility Summary
Provides individual, family and group therapy and case management to clients within agency policies and procedures and practices. Assesses the social service needs of clients and makes appropriate referrals.

- Provides direct individual and family therapy to a minimum caseload of 15 clients including comprehensive evaluation, treatment planning, effective treatment implementation, and discharge planning within the guidelines established by the Family Hope Center.
- Facilitates a minimum of two to four therapy or psychoeducational groups (child, parent, combined child and parent, or family) per year (e.g., Hope and Healing, ICDP).
- Performs a variety of casework duties, including obtaining, analyzing and evaluating data necessary to implement client services.
- Actively participates in reflective, clinical and administrative supervision with the clinical supervisor at a frequency to be determined by the therapist and supervisor together.
- Actively participates in Family Hope Center department meetings and case consultations.

- Maintains accurate, complete and timely client records as required by the Family Hope Center and ChicagoCAC.

- Participates in Family Hope Center community events and actively fosters supportive community environment within Family Hope Center for clients and families.
- Promotes trauma resiliency among the Family Hope Center team, ChicagoCAC staff, and MDT staff by participating in and contributing to projects to foster the trauma informed environment, e.g., Breathing Room.
- Facilitates trainings for Family Hope Center students.
- Performs other duties as assigned.
Discretion
The Therapist has a mid-level range of discretion and independent decision-making.
Supervisory Responsibilities
The Therapist has no direct supervisory responsibilities.
Relationships
The Therapist works closely with the Clinical Supervisor, Family Hope Center team, Advocacy department team and manager of PATHH and intensive case management.
Qualifications
Minimum requirement of a Master's Degree in Social Work, Counseling, Psychology, or another human services related field. Must be eligible to receive LCPC, LCSW license or PsyD. Minimum of three years of experience providing trauma-focused evidence-based therapy to children and adolescents. Strong problem-solving, organizational, written, oral, and communication skills needed. Demonstrated awareness of multiple cultures and cultural impact on the delivery of service a must. Flexibility in schedule based on the needs of clients is required. Vehicle, insurance, and valid driver's license are required. Evenings and Saturdays may be required. Fluency in Spanish is required.

The Community Based Therapist is responsible for providing home-based individual or family therapy and crises intervention for the purposes of family preservation, placement stabilization, reunification, and/or immediate crises stabilization. - Make necessary referrals to meet client's health, well-being, education, mental health, and employment needs (i.e., Department of Public Health, Department of Family & Children Services, Georgia Department of Labor, employment staffing agencies, local colleges and tech schools, etc.)
- Facilitate and/or participate in Team Meetings for all clients/members/residents/young adults.

Job Summary:
The Community Based Therapist is responsible for providing home-based individual or family therapy and crises intervention for the purposes of family preservation, placement stabilization, reunification, and/or immediate crises stabilization. The Therapist must maintain a flexible work schedule that may include working hours other than regular work schedule and evenings, weekends and holidays. The Therapist should demonstrate interpersonal skills and the ability to connect with and engage with families. The Therapist will be expected to work with families with a risk of CPS and DFCS involvement, children who are in the foster care system, birth parents, and foster parents.
This is a contract role, and contract employees are not eligible for company benefits.
Re sponsibilities :
- Serve high-risk families, including those whose children are at substantial risk of placement or who are in placement and cannot be reunified without in-home services.
- Provide services primarily in client homes to include individual counseling, family counseling/training, drug treatment and support services for the parent/caregiver and/or child, domestic violence counseling, anger and stress management /counseling, enhanced behavior management for children, and grief management including loss/separation issues.
- Administer biopsychosocial assessments, Caregiver Strain assessments, Child and Adolescent Function Assessment Sale, Bonding/Attachment assessments, and any other validated assessment tools specific to service needs.
- Schedule sessions at times that are convenient for families, and when problems are likely to occur, including evenings, weekends and holidays.
- Collaborate and advocate with formal and informal community resources and systems, while teaching clients to advocate for themselves.
- Identify, develop, and implement appropriate cognitive and behavioral intervention strategies that are sensitive to the unique characteristics of each individual or family.
- Ensure that clients files, progress notes, service plans and data collection points are submitted timely accurately documented and accurately reflect work being done with family.
- Be fully prepared for weekly case review and supervision.
- Assist administrative staff in collection of information for program effectiveness.
- Promote a positive relationship and healthy boundaries with each client with a non-judgmental and strength-based approach.
- Respond in a positive and respectful manner to diversity, among the agency's service population and staff.
