- Community Holistic Defense for Juveniles and Emerging Adults: Free legal representation for indigent clients grounded in holistic defense practices which acknowledge that quality of representation and criminal legal outcomes are improved for youth when their unique developmental needs are simultaneously addressed by an integrated, interdisciplinary team of attorneys and social service providers.
- Community Housing: LCLC owns and operates 20 units of studio style housing for emerging adult men, 18-25 years old, inside the Deer Community Justice Center. In addition, LCLC has created a network of approximately 275 private landlords and funders that place other LCLC clients and their families in subsidized affordable housing units.
- Restorative Justice Community Court: LCLC was the lead agency that gathered numerous community and cook county court stakeholders together to design and launch the first Restorative Justice Community Court in the country in North Lawndale in September 2017. This court is based upon the tradition of restorative justice circle keeping and other restorative justice practices to enable repair and community integration for emerging adult men, 18-25, facing qualifying felony criminal legal charges. LCLC hopes to add a juvenile call to this court to serve minors 17 and younger under the same model.
- Community Holistic Juvenile Diversion: Since 2020, LCLC has worked with the Mayor's office and the Chicago Police Department to close the Juvenile Intervention and Support Center (JISC) and create a new model of juvenile diversion for Chicago. LCLC is one of eight service providers in the city and focuses on serving youth from the west side diverted from the courts by the Chicago Police Department.
LCLC looks to continuously innovate our program portfolio to build upon new best practices for serving minors and emerging adults and better serve our communities.
Position Summary
LCLC's Outreach Worker is responsible for helping to reduce community violence and working with juveniles and emerging adults, 25 and under, who are of the highest risk of violence to help prevent shootings and retaliatory violence. More specifically, the responsibilities of outreach include but are not exclusive to being a part of the Community Holistic Defense team working in collaboration with attorneys and case managers identifying new potential clients and engaging existing clients in services, maintaining a caseload of 15 clients, mediating conflict, canvassing, monitoring and responding to community violence, serving as a liaison between LCLC and the community, and promoting the LCLC way of working to transform the lives of young people we serve and the criminal justice system as a whole. This individual will work on a rotating schedule of 10a-6p Mon-Fri; Tues-Saturday(DR1) 10a-6p along with being on call to respond to community violence up until 9p. Travel is required.
Responsibilities
- Work with Juveniles and Emerging Adults (25 and under) who are of the highest risk of violence successfully engaging and/or re-engaging them in services and maintaining a caseload of up to 15 clients.
- Maintains contact with clients on a weekly basis, at minimum.
- Participates in Holistic Intake process with attorney and case management & completes eligibility screen for outreach services.
- Complete the safety assessment and safety intervention plan that will be used to help identify appropriate services, safety risks, and needed resources, sharing findings with case management & legal.
- Complete all required documentation in a timely manner utilizing the agency database.
- Ensures timely information about potential and current clients and their safety is clearly identified and communicated, both verbally and in writing, to attorneys, case managers and Director.
- Encourage and assist clients with completing individual goals identified in their work with case management.
- Demonstrates supportive behavior and act as a role model displaying positive appropriate interactions.
- Works with clients individually and in group setting on violence prevention skills (i.e., anger management, conflict resolution).
- Coordinates and facilitates activities for workshops and groups for clients (peace circles, recreation, life skills).
- Have knowledge and a clear understanding of street dynamics, trauma and the effects of trauma and operate from that perspective when interacting with clients, their families and the community.
- Gain and maintain the trust of the community to help prevent shootings and retaliatory violence.
- Serves as support and advocate for the clients and their families, identifying and relaying the need for resources to case management.
- Canvassing the community and working in other unstructured situations.
- Collaborate and network with other Outreach programs and workers to coordinate strategic responses to violence.
- Monitor and respond to shootings or other forms of community violence in North Lawndale received through CPIC notifications, Citizens app and word of mouth as well as responding to violence in surrounding communities when necessary.
- Willingness to work later hours during the week and during the weekends.
- Coordinates and lead community events for clients, their families and North Lawndale community.
- Market and promote the work of LCLC in the North Lawndale community magnifying our Holistic services to attract and identify potential clients in services.

- Firsthand knowledge of North Lawndale and other communities served and knowledge of current street conflicts.
- Some experience or training in crisis intervention, conflict resolution/mediation, and trauma preferred.

Under the supervision of the Syringe Access Program Manager performs assigned duties relating to the implementation and evaluation of Outreach Services
Duties include but are not limited to the following:
- Conduct outreach to the substance abusers and high risk communities. These communities include women and men who are IDUs and those who engage in substance use.
- Street Outreach will be conducted in at least teams of two in identified high risk areas.
- Outreach will be provided to service providers in the area to introduce SAP and ARCH services.
- Provide relevant literature to clients and members of the community. Engage IDUs and those who engage in substance use in dialogue pertaining to risk behaviors, harm reduction and testing options for HIV, Hep C, STDs and availability of PrEP. Identify obstacles for the provision of services and assist with referrals.
- Actively seek and identify individuals or groups requiring the agency's services.
- Provide harm reduction and health education.
- Serve as a resource for members of the community who are IDUs and those who engage in substance use already to ensure access to needed care and treatment services.

THE OPPORTUNITY
Neighbors in Action, formerly known as the Crown Heights Community Mediation Center operates out of a storefront since the summer of 1998, and provides anti-violence programs, youth programs, and resource links to community residents. Its mission is to encourage communication and understanding, prevent future conflicts, and help foster stronger, healthier neighborhoods.
Neighbors in Action (NIA) seeks an Outreach Worker Supervisor. Reporting to the S.O.S. Program Manager, the Outreach Worker Supervisor oversees the Crown Heights Outreach Workers. S.O.S. Crown Heights is a replication of the Cure Violence program, a gun violence prevention model that aims to reduce and prevent shootings through the use of public health strategies. The Outreach Worker Supervisor will work individually and as part of a team to prevent neighborhood shootings and killings. They provide conflict mediation services and provide direction to the team.
Responsibilities include, but are not limited to:
- Work closely with the Program Manager to plan the day-to-day and week-to-week activities with and for the outreach staff;
- Have daily communication with each member of the outreach team;
- Meet weekly at a regularly scheduled time for supervision sessions with each team member;
- Lead the outreach workers and collaborate with the Program Manger in researching causes of shootings/killings and assist in mediating underlying conflicts and prevent retaliations between individuals and groups;
- Identify and build relationships with individuals who are at high risk of involvement in shootings and killings to help them rethink their attitudes and beliefs about violence and redirect their lives in more positive directions;
- Maintain a small caseload of five participants;
- Create and monitor a risk reduction plan with each participant;
- Plan, prepare, attend and help to facilitate participant events including violence prevention and life skills workshops in local schools, parks, and community-based organizations;
- Lead outreach workers in identifying and cooling "hot spots" for shootings and violence (e.g., conducting eyeball surveys with residents and community-based organizations to identify areas frequented by high risk individuals);
- Conduct outreach to the community to build strong relationships with youth, residents, businesses, and community groups;
- Work with the Program Manager to provide program participants with support and linkages to programs including wrap-around services (e.g., job programs, GED, drug treatment, and mentoring);
- Attend weekly supervision sessions with the Program Manager and report regularly to the Program Manager on all violence reduction strategies, relevant community activities, personnel issues, or communications with press, politicians, or law enforcement;
- Document all work including shootings and other acts of violence prevented, case notes, referrals, contacts with participants, and changes in participants' status;
- Collaborate with the Program Manager in planning and executing the community mobilization strategy;
- Assist the Program Manager in organizing community rallies in response to shootings and deploying the outreach workers to increase its visibility when shootings/killings take place in the target area;
- Maintain a schedule of all approved S.O.S. Hospital Responders (including the Coordinator) that tracks the rotation of responses to hospital calls;
- Monitor Hospital Responders' log entries to ensure that the data regarding is accurate and up to date; and
- Additional tasks as necessary.
Qualifications: High school diploma as well as a minimum of 4 years' experience or the equivalent in a related area, knowledge of mental health, youth engagement, and community organizing. The candidate should reside in or have an in-depth knowledge of Bed-Stuy, especially the target area (Nostrand Avenue to Marcus Garvey Boulevard, Kosciuszko Street to Madison Street) including individuals and groups in the neighborhood who are known to have had past involvement with gangs and/or gun violence, influential people in the neighborhood and the area's hotspots. A stable residence within or nearby the target area (highly preferred). Excellent communication skills and basic computer skills. Experience with (or eagerness to develop) administrative skills in reporting, and documentation. Ability to walk 40 blocks. One or more years of supervisory experience is preferred. Penn Community Violence Prevention (PCVP) is a strategic evidence-based public health approach to reduce and prevent shootings and killings in areas with a high prevalence of gun violence. Skilled outreach workers are a key ingredient to the success of this initiative and are instrumental to facilitating both community norm change, and positive behavior change among high-risk individuals. For this initiative, PCVP staff will implement the program with fidelity in three feeder schools of Bartram H.S. and the concentrated areas of violence in West/Southwest Philadelphia. The PCVP Outreach Worker will provide community and school-based violence prevention activities including school presentations/assembly's, trainings, mediating conflicts, canvassing the community, adult community outreach and mobilization, assisting in shooting responses, case management/referrals, and accurately documenting all activities in a timely manner. They will also manage a caseload of 15-30 clients that are at high risk for violence as they work to detect and interrupt violence, mentor at-risk individuals towards prosocial paths and away from street violence and anti-social networks and change attitudes and social norms that promote violence in vulnerable communities. This position will participate and assist in all related PCVP community outreach events. This is a 3-year grant-funded position. Continuation of this position is contingent on funding.
The duties of the Outreach Worker include Scheduled work hours are between 7 am - 8 PM, Monday - Friday, with weekends as needed.
This is a non-traditional work setting doing street outreach. It will require working in inclement weather.

Summary
Position is for 1 Part-time. As an Outreach Worker, you will have the opportunity to identify the needs of homeless, runaway, foster, and at-risk youth ages 11-24 and connect them with resources, crisis intervention, referral information, and tangible goods to provide for their basic needs. Services and support provided as an Outreach Worker enable vulnerable youth to become self-sufficient, providing them with low-barrier, unconditional support that meets basic needs, links them to safe housing, supportive services, and increases their ability to navigate their community and thrive.
Your Impact and Key Responsibilities
- Complete intakes and conduct individual assessments with youth who access street outreach services. Monitor and oversee youth in the Drop-in Center, assisting them with laundry, food preparation, accessing shower facilities and basic needs supplies.
- Develop and maintain knowledge of community resources and connect participants to appropriate employment opportunities, education, medical, mental health, and basic needs. Connect and transport youth to shelter, housing resources and other appropriate resources as needed.
- Plan and facilitate events for youth in the Drop-in Center, assisting with the development of a monthly activity calendar.
- Collaborate with volunteers and community partners to provide resources and supports to youth who access services. Deliver presentations in the community.
- Conduct shifts of street outreach, identifying areas where homeless, runaway, and at-risk youth congregate.

Street Medicine is an innovative and compassionate approach to healthcare, designed to meet individuals experiencing homelessness right where they are: on the streets, in shelters, or within underserved communities. Purpose of the position
This position is focused on conducting community outreach among unhoused populations. The Community Health Outreach Worker (CHOW) care coordinator will play a crucial role in identifying individuals with active HIV/HCV and STI cases and ensuring they are connected to appropriate care. This includes verifying that patients have received the correct treatment and follow-up care.
Staff selected for this role will contribute to a vital and sustainable street medicine program designed to serve unhoused communities in the Riverside County area. Often, the most vulnerable individuals experiencing homelessness have encountered repeated failures from institutions throughout their lives, leading to a deep mistrust of authorities, institutions, and healthcare providers. This mistrust, while initially a form of self-protection, can become a significant barrier to accessing care and resources that could significantly improve their quality of life.
The street medicine team will work to build trusting relationships with people who are in need of medical services, work to decrease the logistical barriers that block access to health care and provide care directly to the places where unhoused individuals live.
Key Responsibilities
- Conduct outreach activities within a specific health program and/or defined patient populations with the goal of engaging multiple local businesses, schools, and other relevant organizations in our public and community health services.
- Conduct outreach with patients to identify social determinants of health impacting patient's health and overall wellness
- Utilize coaching, motivational interviewing, and other evidence-based techniques to support patients in achieving their goals
- Utilize technology and digital resources to monitor ongoing care activities
- Identify barriers to achieving targeted clinical or social outcomes, and engage the care team to revise the care plan when necessary
- Documents all participant encounters; completes and submits monthly reports; maintains comprehensive electronic participant files. Documents activities, service plans, and outcomes achieved by study participants in an effective manner.
- Works collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the participant, providers, nurse case managers, agency representatives, research staff, supervisors, and office staff, from diverse cultural and socio-economic backgrounds. Works to reduce cultural and socio-economic barriers between participants and institutions.
- Engage eligible Enhanced Care Management (ECM) members.
- Accompany ECM members to office visits, as needed, and in the most easily accessible setting, within Plan guidelines.
- Health promotion and self-management training.
- Arrange transportation.
- Assist with linkage to social supports.
- Distribute health promotion materials.
- Communicatie with members to facilitate visits with the care coordinator.
- Connect ECM members to other social services and support the members may need.
- Advocate on behalf of members with health care professionals.
- Monitor treatment adherence (including medication).
Requirements
Essential Skills and Qualifications:
CHWs must have either lived or professional experience that aligns with and provides a connection between the CHW and the clients being served. This may include, but is not limited to experience related to incarceration, military service, pregnancy and birth, disability, foster system placement, homelessness, mental health conditions or substance use, or being a survivor of domestic or intimate partner violence or abuse and exploitation. Lived experience may also include shared race, ethnicity, sexual orientation, gender identity, language, or cultural background with one or more linguistic, cultural, or other groups in the community for which the CHW is providing services. Supervising Providers (the organizations employing or otherwise overseeing the CHWs with which the MCP contracts, as described below) are encouraged to work with CHWs who are familiar with and/or have experience in the geographic communities they are serving. Supervising Providers must maintain evidence of this experience.
The CHW serves as an advocate for the health needs of individuals by assisting community residents in effectively communicating with healthcare providers or social service agencies. Act as liaison or advocate and implement programs that promote, maintain, and improve individual and overall community health. May deliver health-related preventive services such as blood pressure, glaucoma, and hearing screenings. May collect data to help identify community health needs.
Staff identified to be a part of this operation will be a part of an important and sustainable street medicine program aimed at serving the unhoused populations in the Riverside County area. The most vulnerable people experiencing homelessness have likely been failed by institutions many times in their lives, and their mistrust of authorities, institutions, and individual care providers may represent an attempt at self- protection that, over time, becomes a barrier to accessing care and resources that could improve their lives.
