JOB PURPOSE : Provides cultural mediations between health care, social services and the community. Helps individuals increase health knowledge and self-sufficiency through a range of activities such as outreach, client navigation and follow-up, community health education and information, informal counseling, social support, and advocacy. Assists with gaining access to needed services and build increased linkage and retention in substance use, mental health, and medical services to individuals, families, and groups.

EDUCATION AND EXPERIENCE REQUIRED: Minimum two- years' experience in the delivery of social, mental, or health related services, preferred .

  • May be required to interact with other consumers, referral sources, outside agencies, and family members to determine needs and actions to be taken.

  1. Performs Community Outreach to community organizations, individual community members, and business impacted by substance use.
  2. Completes CHW documentation and weekly reports in accordance with DSHS CHW standards.
  3. Actively participates as a CHW team member to ensure the program standards are being met in an efficient and professional manner.
  4. Provides individual and group Community Health Worker services
  5. Regularly participates in the development of all other activities planned to further the primary purpose of the CHW Grant
  6. Maintains and submits accurate data and documentation on reproduction of any reproduced, purchased, or borrowed media for the program.
  7. Thoroughly networks with communities to promote interacting with individuals, businesses, faith-based institutions, and other community organizations and group-based presentations as assigned.
  8. Establishes rapport with individuals to help determine emotional status and need for follow-up services.

How You Make An Impact:

TYPICAL DUTIES
  • Assists various public health professionals and technical staff in providing a wide variety of services in the area community public health.
  • Assists in teaching basic health practices to clients and families, particularly in areas of communicable disease transmission, prevention and treatment; may assist in re-enforcing basic public health concepts of nutrition, hygiene, sanitation and related.
  • Assists in conducting field investigations to locate persons possibly infected or who test positive for communicable diseases, including HIV infection, and those with whom they have had contact; assists in providing follow-up counseling as to treatment options and resources available; makes referrals as appropriate.
  • Assists in interviewing, gathering and assembling related information and preparing reports; maintains appropriate records and files.
  • Translates various written material and instructions of other staff for non-English speaking clients and families; assists staff in working with clients of non-traditional cultural patterns and dealing with various cultural barriers.
  • Participates in development of programs and delivery of services for high-risk groups; provides information and counseling regarding HIV antibody and other communicable disease testing.

MINIMUM QUALIFICATIONS

Experience: One year as a Community Health Outreach Worker Trainee.

OR PATTERN II

Experience: One year responsible work experience providing community based health services, working with community based organizations that provide health related services to culturally diverse groups or working with high risk groups in area of communicable diseases.

License: Possession of a valid California Driver's License.

Special Requirements:

If required by nature of assignment:

1. Must obtain an HIV pre and post-counseling certificate issued by State of California within one year of employment.

2. Ability to fluently speak, read and write Spanish or one of the Indo-Chinese languages.

3. Must obtain a Certified Phlebotomy Technician I certificate issued by the California State Department of Health Services within six months of assignment.

Bereavement Leave : 3 days of paid leave for the death of qualifying family member, 2 additional days of accrued leave for death of employee's spouse, domestic partner, parent or child.

Merit Salary Increase : New employees will receive the starting salary, which is the first step of the salary range. After employees serve 52 weeks (2080 hours) on each step of the range, they are eligible for a merit increase to the next step.

Job Sharing : Employees may agree to job-share a position, subject to approval by a Department Head and the Director of Human Resources.

Educational Reimbursement Program : Eligible employees may be reimbursed for career-related course work up to a maximum of 850 per fiscal year. Eligible employees enrolled in an approved four (4) year College or University academic program may be reimbursed up to 800 per semester for a maximum of 1600 per fiscal year.

Parking Supplemental Downtown Stockton: The County contributes up to 20 per pay period for employees who pay for parking and are assigned to work in the Downtown Core Area.

School Activities : Employees may take up to 40 hours per year, but not more than eight (8) hours per month, to participate in their children's school activities.

Selection Procedures

Applicants who meet the minimum qualifications will go through the following examination process:
  • Written Exam : The civil service written exam is a multiple choice format. If the written exam is administered alone, it will be 100% of the overall score. Candidates must achieve a minimum rating of 70% in order to be placed on the eligible list.
  • Oral Exam : The oral exam is a structured interview process that will assess the candidate's education, training, and experience and may include a practical exercise. The oral exam selection process is not a hiring interview. A panel of up to four people will determine the candidate's score and rank for placement on the eligible list. Top candidates from the eligible list are referred for hiring interviews. If the oral exam is administered alone, it will be 100% of the overall score. Candidates must achieve a minimum rating of 70% in order to be placed on the eligible list.
  • Written & Oral Exam : If both a written exam and an oral exam is administered, the written exam is weighted at 60% and the oral exam is weighted at 40% unless otherwise indicated on the announcement. Candidates must achieve a minimum rating of 70% on each examination in order to be placed on the eligible list.
  • Rate-out : A rate-out is an examination that involves a paper rating of the candidate's application using the following criteria: education, training, and experience. Candidates will not be scheduled for the rate-out process.
Note: The rating of 70 referred to may be the same or other than an arithmetic 70% of the total possible points.

Online Written Exams : Written exams may be administered online. Candidates will be notified of the examination date and will be responsible to complete the written exam within a given period of time (typically within five calendar days). Candidates are required to read the Online Exam Guide for Test Takers prior to taking an online written exam.

Eligible Lists : Candidates who pass the examination will be placed on an eligible list for that classification. Eligible lists are effective for nine months, but may be extended by the Human Resources Director for a period which shall not exceed a total of three years from the date established.

Certification/Referral : Names from the eligible list will be referred to the hiring department by the following methods.
  • Rule of the Rank : The top rank or ranks of eligibles will be referred for hiring interviews. The minimum number of names to be referred will be equal to the number of positions plus nine, or 10% of the eligible list, whichever is higher. When filling nine or more positions in a department at the same time, the top rank or ranks will be referred and the minimum number of names shall be two times the number of positions to be filled or 10% of the eligible list, whichever is higher. This applies only to open competitive recruitments.
  • Rule of Five : The top five names will be referred for hiring interviews. This applies only to department or countywide promotional examination.
Physical Exam : Some classifications require physical examinations. Final appointment cannot be made until the eligible has passed the physical examination. The County pays for physical examinations administered in its medical facilities.

Pre-Employment Drug Screening Exam : Some classifications require a new employee successfully pass a pre-employment drug screen as a condition of employment. Final appointment cannot be made until the eligible has passed the drug screen. The County pays for the initial drug screen.

Employment of Relatives : Applicants who are relatives of employees in a department within the 3rd degree of relationship, (parent, child, grand parent, grand child or sibling) either by blood or marriage, may not be appointed, promoted, transferred into or within the department when;
  • They are related to the Appointing Authority or
  • The employment would result in one of them supervising the work of the other.

Department Head may establish additional limitations on the hiring of relatives by departmental rule.

The Role

The Outreach worker is essential in locating, engaging and enrolling assigned Health Home eligible persons who have been identified by the NYS Department of Health to be in need of comprehensive and coordinated social services. Outreach activities are defined within reason as electronic searches, correspondence, telephone calls, home visits, field visits and/or agency visits. Transportation is provided for all outreach activities or is reimbursed within the parameters of participating Health Home providers. This work is carried out in support of the mission and goals of Samaritan Daytop Village.

Responsibilities

What You Will Do
  • Perform community-based outreach activities such as home visits, provider visits, telephone calls and correspondence.
  • Confirm patient information and coverage via EPACES.
  • Follow engagement activities from an assigned calendar provided monthly.
  • Document all patient outreach activities in the Health Home EMR (electronic medical record), complete a Time Log reflecting the outreach activity and submit to Intake Coordinator daily.
  • If an Intake is agreed upon with potential enrollee complete the Demographics Form, A FACT GP, and the correct Health Home releases form. Upon return to the office complete a Time Log and a Progress Note in the Health Home EMR.
  • Prepare and maintain Health Home intake packets for each individual Health Home.
  • Respond to calls from Health Home regarding patients at noted hospital Emergency Rooms.
  • Complete Intakes as assigned.
  • Complete Bottom-Up Screening Forms as needed.
  • Any tasks or duties assigned by Managing Staff and/or Program Director.

The SUV Stand Up to Violence is a gun violence prevention initiative run out of NYC Health+ Hospitals/Jacobi . The SUV program is an evidence-based, violence reduction initiative that treats gun violence as a disease, by identifying its cause and it's causes and interruptingits transmission. The program hires a community outreach team of credible messengers - individuals who are hired from the communities in which they work and have backgrounds similar to those with whom they aim to connect - mentor the highest risk youth with an emphasis on conflict mediation and violence prevention.

In order to prevent violence, methods and strategies associated with disease control are implemented:

1. Detecting and interrupting conflicts -Outreach Workers (OW) identify and mediate conflict between high-risk individuals and groups in order to prevent violence and ongoing acts of retaliation.

2. Treating the highest-risk individuals the OWsmentor and support high-risk youth and young adults(ages 14 to 28) and connect them to educational, vocational, recreational and social services.

3. Changing social norms.

Opportunity Details :

  • Recruit and manage a caseload of 15 "high risk" gang/crew members and provide case management services. Responsible for mentoring these participants with a focus on providing alternatives to high-risk and violent behavior.
  • Build and maintain relationships with local gangs/crews,stay up-to-date on rivalries and plans for retaliation,facilitate conflict mediation.
  • Canvass the target areas on foot and identify and diffuse "hot spots" for shootings and violence.
  • Link participants and individuals in the target area to opportunities and programs in the community including vocational, educational, recreational, and social services.
  • Plan and participatein monthly community activities (cook-outs, basketball tournaments, etc.) in order to strengthen community awareness and garner support.
  • Engage in community shooting responses (marches, vigils, etc.) within 72-hours after a shooting, in order to mobilize the community against gun violence and change community norms.
  • Document all participantinteractions and conflict mediations in the program's computer database.
  • Attend ongoing trainings, individual supervision, and weekly staff meetings.
  • Communicate effectively with the team including other OWs, and supervisors.
  • Adhere to program safety regulations and remain in contact with a supervisor at all times.
  • Work flexible hours (mostly evenings and weekends) and respond to shootings or emergency situations.
  • Respond to violent trauma incidents and see patients in the hospital, when needed.
