About the Job

Reporting to the Community Care Manager, the Community Health Worker is a member of the Grace at Home interdisciplinary care team, providing in-home support to Grace at Home patients and their families as they navigate the often-complex health care systems. This role serves as a vital link between patients, their communities, and care providers. Through addressing individual patients’ unique barriers to care and social determinants of health, the Community Health Worker ensures improved health outcomes and overall quality of care in coordination. They are a patient advocates, ensuring that their voices are heard and their needs are met within the healthcare system. This position is a community-based position that will require you to be in the community and meeting with patients face to face in their homes, at doctor appointments, libraries, or other locations within the community agreed upon by the participant and the CHW.

Responsibilities

  • Establish rapport with patients and their families to facilitate open communication and understanding of their health needs during transitions; providing patients with essential information about their health conditions and self-management strategies, as directed by the clinical team.

  • Conduct in-home monthly or bimonthly visits with patients based on risk stratification.

  • Conduct telehealth video visits with appropriate clinical or social resources to address the patient’s real-time clinical, behavioral, or social needs. Ability to utilize appropriate monitoring equipment for telehealth video visits.

  • Collaborate with healthcare providers, social workers, and community organizations to ensure that patients receive comprehensive care. This includes scheduling follow-up appointments, arranging transportation, and coordinating home health services.

  • Assist patients in navigating insurance and healthcare policies, accessing community resources such as financial assistance programs, housing support, and healthcare services to help them manage their health effectively.

  • Be sensitive to the diverse cultural backgrounds of patients, ensuring that care is respectful and tailored to individual needs.

  • Utilize evidence-based assessments and approaches to help patients identify and prioritize their health, address social challenges, and utilize community-based resources.

  • Establish and maintain working relationships with primary care providers, interdisciplinary teams, and community-based programs to ensure coordinated care for patients.

  • Engage in care coordination efforts to assist patients in identifying barriers to health and well-being and collaborate to address those challenges effectively.

  • Accurately document all visits and patient interactions in the electronic medical record (EMR) and team tracking systems.

  • Maintain detailed records to ensure continuity of care and assist in monitoring the progress of patients.

  • Perform other job-related duties as assigned.

Qualifications

  • Education: Associate degree or a combination of education and experience required; B.S or B.A. preferred.
  • Experience: 2+ years in the healthcare arena or exposure to health care systems required. Experience as a Community Health Worker or equivalent supporting vulnerable populations required.
  • License/Certification: CPR certification is required. CHW certification is a plus.
  • Communication: Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to Grace at Home’s team and patients with the ability to build trust and rapport, in-person and over the phone, with individuals from diverse backgrounds.
  • Relationships: Ability to build and effectively manage relationships internally and externally.
  • Culture: Good judgment, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial company.
  • Unrestricted driver’s license in home state required.

  • Ability to work collaboratively with healthcare teams and community organizations to address the complex needs of participants.

  • Experience working with electronic medical records (EMR) and care management system.

  • Strong knowledge of HIPAA regulations and ability to handle sensitive and confidential information with discretion.

  • Experience providing drug and alcohol screening, assessment, linkage, and integrating substance use interventions for dually diagnosed clients, or willingness to be trained in these areas preferred.
  • Strong relationship-building skills with the ability to engage individuals and families who may initially be reluctant to participate preferred.
  • Knowledge of, commitment to, and passion for serving adults and families living in Chicago communities experiencing concentrated socioeconomic disadvantage and high rates of community violence preferred.
  • Demonstrated ability to collaborate effectively with outreach teams, hospitals, justice system partners, and community stakeholders to support participants preferred.
  • Master’s degree in social work, Counseling, Psychology, or a related field from an accredited college or university required.
  • 2 + years of related post-Master’s experience at an intermediate skill level, required.

  • Prior experience working within a community violence prevention, trauma recovery, or outreach model preferred.
  • Familiarity with Medicaid documentation and performance-based contracting requirements preferred.
  • Additional certification or training in trauma-informed care, motivational interviewing, or harm reduction approaches preferred.
  • Understanding of trauma-informed care principles, behavioral interventions, and violence dynamics in urban communities preferred.
  • Experience providing drug and alcohol screening, assessment, linkage, and integrating substance use interventions for dually diagnosed clients, or willingness to be trained in these areas preferred.
  • Strong relationship-building skills with the ability to engage individuals and families who may initially be reluctant to participate preferred.
  • Knowledge of, commitment to, and passion for serving adults and families living in Chicago communities experiencing concentrated socioeconomic disadvantage and high rates of community violence preferred.
  • Demonstrated ability to collaborate effectively with outreach teams, hospitals, justice system partners, and community stakeholders to support participants preferred.
  • Master’s degree in social work, Counseling, Psychology, or a related field from an accredited college or university required.
  • 2 + years of related post-Master’s experience at an intermediate skill level, required.
  • Current Illinois LCPC (Licensed Clinical Professional Counselor) or LCSW (Licensed Clinical Social Worker) required.
  • Must become IM+CANS certified within 3 months of hire and maintain re-certification annually required.
  • Ability to maintain mobility and manage a community-based caseload, including school visits required.
  • Demonstrated ability to work effectively with diverse individuals and groups required.

ADDITIONAL REQUIREMENTS:

  • Satisfactorily pass all relevant screenings and criminal background checks.

TRANSPORTATION/TRAVEL REQUIREMENTS:

  • Driving for work required with a personally owned vehicle. Travel between sites required.

CTS Health Services is a community mental health services center that specializes in a holistic approach to health and wellness for children, parents, and adults of varying circumstances and mental challenges. We provide one-on-one behavioral interventions for our clients and their support circles to help our clients navigate through life within their community at their highest level.


What We Are Looking For:

We are urgently seeking passionate, authentic, and motivated individuals to provide mental health therapy / counseling with an emphasis on prevention for the Community Support Team . The mental health therapist will work with individuals and groups to develop treatment plans and activities to promote optimum mental health, help individuals and their support systems deal with addictions and substance abuse; family, parenting, and marital problems; suicide; stress management; problems with self-esteem; and issues associated with aging and mental and emotional health, etc.


* Occasional traveling will occur in order to meet consumers where they are and in the community *


Responsibilities and Duties of a Qualified Mental Health Professional (QMHP) Community support team (CST) Therapist, not limited to:

  • Maintain billable and non-billable therapy hours as instructed.
  • Perform physical and mental assessments and behavioral screening of clients and update as required.
  • Encourage clients to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships.
  • Guide clients in the development of skills and strategies for dealing with their problems.
  • Prepare and maintain all required treatment records and reports with a high level of confidentiality.
  • Counsel clients and patients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changes.
  • Collect information about clients through interviews, observation, and tests.
  • Act as client advocates to coordinate required services or to resolve emergency problems in crisis situations.
  • Develop and implement treatment plans based on clinical experience and knowledge and update as required.
  • Collaborate with other staff members to perform clinical assessments and develop treatment plans.
  • Meet with families, probation officers, police, and other relevant parties to exchange necessary information during the treatment process.
  • Refer patients, clients, or family members to community resources or to specialists as necessary.
  • Counsel family members to assist them in understanding, dealing with, and supporting clients or patients.
  • Evaluate the effectiveness of counseling programs and clients' progress in resolving identified problems and moving towards defined objectives.
  • Plan, organize and lead structured programs of counseling, work, study, recreation, and social activities for clients.
  • Modify treatment activities and approaches as needed to comply with changes in clients' status.
  • Gather information about community mental health needs and resources that could be used in conjunction with therapy.
  • Monitor clients' use of medications.
  • When necessary attend scheduled meetings, including, but not limited to, executive team meetings, state team meetings, school and treatment/plan team meetings.
