OurLegacy is designated by the Centers for Medicare and Medicaid Services (CMS) as the only organ procurement organization (OPO) in east central Florida and is accredited by the American Association of Tissue Banks (AATB) as a tissue recovery agency. OurLegacys scope of operations extends to each of the following hospitals within the AdventHealth Hospital system: Altamonte, Apopka, Celebration, Daytona Beach, Deland, East Orlando, Fish Memorial, Kissimmee, New Smyrna Beach, Orlando, Palm Coast, Palm Coast Parkway, Waterman, Winter Garden and Winter Park. OurLegacy is seeking a passionate and innovative Community Engagement & Partnerships Specialist to join our team. In this position, you will play a crucial role in coordinating our brand presence, fostering community engagement, and developing strategic partnerships. Under the general direction of Manager, Multicultural Affairs and Partnerships, the Community Engagement & Partnerships Specialist strategically plans, develops, coordinates, and evaluates community and corporate relations activities to increase donor designation rates within the OurLegacy donation service area. Through networking, strategic partnerships and collaboration, this person will develop and maintain a positive culture of donation in their key area and maintains a favorable public image for OurLegacy. This individual will also actively participate in industry organizations to keep abreast of national efforts, effect change as opportunities present themselves, increase knowledge of best practices, maintain relationships, and develop collaborative opportunities.
**You Will Be Responsible For:**
+ Demonstrates a strong understanding of OurLegacy, the organ procurement organization industry, the organ, eye, and tissue donation process, transplantation and research opportunities.
+ Independently develops and executes strategic community outreach marketing programs to increase organ and tissue donation awareness in target communities.
+ Develops and maintains a comprehensive relationship target list for assigned territory, including community-based organizations, faith-based organizations, healthcare associations, school-based organizations, government officials, and others.
+ Professionally plans, coordinates, and attends meetings, cultural programs and events in assigned territory to promote organ and tissue donation awareness in target communities.
+ Maintains and strengthens current relationships and identifies and cultivates new relationships with local businesses, organizations, associations, faith-based organizations, school-based organizations, and community groups to raise awareness about organ and tissue donation.
+ Knowledgeable and stays abreast of community events, attending as appropriate to continue to build trust and OurLegacy brand within assigned territory.
+ Fosters relationships with community leaders and government officials in assigned territory to increase their involvement and advocacy around organ and tissue donation.
+ Identifies the assets, needs, issues, and concerns of the residents in their assigned territory to help develop culturally relevant and engaging marketing materials.
+ Researches and coordinates appropriate speaking opportunities for OurLegacy executives and staff at professional associations, business organizations, Chamber of Commerce events, community events, and other events to promote organ and tissue donation.
Qualifications
**What Will You Need:**
**EDUCATION AND EXPERIENCE REQUIRED** **:**
+ Bachelors Degree in communication or related field, i.e. public relations, marketing, public health, and/or journalism
+ 2-years public relations experience in community relations working with community-based organizations, healthcare agencies, volunteers, and civic and faith leaders.
+ Exceptional communication and organizational skills
+ Ability to write for and communicate with a diverse constituency, including but not limited to, donor families, volunteers, high school and college staff and faculty, hospital personnel, medical professionals, media, driver license/tax office staff and customers, and corporate and community partners
+ Fluent in social media trends, calendars and content management systems
+ Self-motivated, systematic in approaches and flexible to the needs of 24/7 organization

The Community Liaison is a trusted link between our team and our local providers to facilitate service referrals that provide solutions and care for their patients. Representing Home & Health Care Management, the Community Liaison will cultivate provider relationships, coordinate outreach events, oversee patient testimonials and ensure referral pipelines are actively nurtured and monitored. This role may assist with staff recruitment as requested by the HR department. This Liaison role is critical to sustaining both direct patient demand and long-term provider relationships.

- Conduct market research to find answers about consumer requirements, habits, and trends
- Brainstorm and develop ideas for creative marketing campaigns
- Assist in outbound or inbound marketing activities by demonstrating expertise in various areas (content development and optimization, advertising, events planning etc.)
- Build and maintain relationships with local providers (i.e. Orthopedic offices, Cardiologists, wound care offices, all hospital based affiliated practices, etc.)

Schedule:

Position Detail:
The Community Liaison serves as a vital connector between our healthcare services and the community. This role is responsible for driving business development and managing both inbound and outbound communications. The Community Liaison acts as the local market leader for B2B (business-to-business), B2C (business-to-consumer), and DTC (direct-to-consumer) engagement, representing our organization to referral partners, patients, and caregivers.
Key Responsibilities:
- Business Development & Outreach:
- Identify opportunities for in-services in the health care community and coordinate their implementation with appropriate persons
- Build and maintain strong relationships with referral sources such as hospitals, physician offices, senior living communities, and other healthcare providers.
- Identify and pursue new business opportunities to grow patient census across service lines.
- Represent the organization at community events, health fairs, and professional networking functions.
- Patient & Family Engagement:
- Educate patients, families, caregivers, and legal representatives (POAs, guardians) on the benefits and services offered through Hospice.
- Serve as a compassionate and knowledgeable resource to guide families through the enrollment process.
- Communication & Coordination:
- Develop relationships with medical staff offices, hospitals and nursing homes; act as a consultant to medical staff and hospital committees, as required to provide hospice and end-of-life information
- Facilitate seamless communication between referral sources and internal clinical teams.
- Ensure timely follow-up on referrals and maintain accurate documentation of outreach activities.
- Market Leadership:
- Provide formal and informal education opportunities with physicians, physician office staff, hospital social workers/discharge planners/patient unit staff, nursing home staff and staff in community agencies.
- Collect and analyze data of referral patterns from all referral sources, to include physicians, social workers, discharge planners, hospital vs community physicians and SNFs.
- Maintain a thorough knowledge of the community’s health care market, competition, consumer demands and governmental policies which influence the agency’s outreach goals and access for patients/families
- Act as the face of the organization in the local market, promoting brand awareness and trust.
- Monitor market trends and competitor activity to inform strategic planning.
Skills and Experience Required
- Proven experience in healthcare marketing, sales, or community outreach—preferably in home health, hospice, or a related field.
- Demonstrated ability to meet or exceed performance goals and key performance indicators (KPIs). This includes:
- Referral volume targets
- Patient admission and conversion rates
- Community engagement metrics
- Timely follow-up and documentation standards
- Relationship development with referral sources
- Strong interpersonal and communication skills, with the ability to engage diverse audiences.
- Knowledge of Medicare/Medicaid and other payer sources is a plus.
- Knowledge of Hospice eligibility and levels of care is preferred
- Knowledge of customer referral source (CRM) management systems for activity tracking
- Ability to work independently and collaboratively in a fast-paced environment.
- Valid driver’s license and reliable transportation required.
Preferred candidates will have a track record of using data to drive decisions, improve outreach strategies, and contribute to overall business development goals.
